Held-up as a standard in dementia care, MOSAIC honors the personhood of each individual by understanding that dementia is a disability in only specific areas of the brain, not the whole person. As a result, our strengths-based programming works to utilize the parts of the brain that are still functioning properly leading to engagement and enhanced quality of life. When you visit Azura you will witness our everyday use of MOSAIC programming through creative expression, small group peer interactions, special interest clubs, one-on-one engagements with caregivers, outdoor activities, small group outings, daily themes, church services, supervised baking, music and much more! In addition, you will be visiting a home that was specifically designed to support and engage those with memory loss throughout their disease process.

We offer a wide range of Alzheimer's, dementia and memory care services at Azura Memory Care of Oshkosh including: 24-hour memory care; on-site respite care; high caregiver to resident ratio; on-site beautician services; electronic charting system; medication management; diabetic management, including sliding scale insulin; registered nursing services; in-home podiatry and dental services and offer on-site physician oversight. In addition, our home works in collaboration with area home health and hospice providers to assist with higher medical needs including wound care, physical therapy, occupational therapy, speech/swallow therapy and end-of-life care. Azura also provides daily behavioral and medical monitoring and is sit-to-stand and Hoyer lift capable.

I visited this facility

Azura Memory Care of Oshkosh was old, rundown, and not well-kept. There were no private bathrooms. And the hallway went in a circle, so someone with memory issues I found it very disturbing because you don't have a bathroom in your room, so I can just picture coming out of this hallway and just going around in a circle and not being able to find the bathroom because it's like a shared community bathroom. The staff was very helpful though. The facility seemed like it needed updating. The carpet seemed old, and the hallways were carpeted, which I don't think is a good idea. It doesn't clean that easily. It seemed like it was old carpeting, and the facility, in general, seemed outdated. The residents were just sitting in front of TVs. There were no group activities or no structured activities there at all.

I visited this facility

Azura Memory Care of Oshkosh was very interesting. They seemed to have a very nice philosophy about keeping the residents busy and making them feel useful. I was very impressed with their program. They said that you might see a resident sweeping the floor or helping the poor corals and that they might not be doing a perfect job but at least they're feeling their self worth. The rooms were just smaller than where my husband is now, and I would have liked a bathroom in the rooms. I think the staff member said they have 18 residents, so there were about 9 rooms on each side. For 9 people, there were four larger bathrooms, and the toilet seats were black because it has something to do with how the brain sees it (easier than a white toilet seat or something like that). So things are kind of set up that way. The other thing that impressed me is that you know they have a heart. They have a stray kitten that showed up in their yard. They bought fishes or food to feed it, got a kitten bed out there for it, and tried to tame it so that the residents can pet it. This kind of shows the quality of their compassion, and I thought that was very nice. I really liked the place. The facility was clean and nice. The staff that I saw were very friendly, and they all said 'hello' right away. I liked the location too. My husband is a farmer. We live in the country, and their backyard faces Sawyer Creek. They said once in a while they feed the deer, so it had that kind of wild and rural look.

I am a friend or relative of a current/past resident

I have my husband in Azura Memory Care of Oshkosh. It seemed like a perfect fit for him because all they do is memory care and Alzheimer's, and that's what his problem is. It's a smaller facility, which I like, and it's a home-like environment. He has only been there for a week, so he seemingly is adjusting as well as can be expected for right now, but I've been very pleased. That's their speciality, and that's what really triggered my interest in them. They also don't have a high turnover, which is great, and they seem to be very caring, too. With COVID, I haven't really seen a lot of the facility, but from what I've seen, it's very adequate, and it's very homey. In getting my husband registered and helping me with the whole thing, all of their regional staff did a wonderful job. They're very caring and very understanding, they're very good, and they have done everything they could do so far. I've only visited my husband once since he has been there. I can go only twice a week, so I'm going again tomorrow. As much as someone with dementia can tell you, he seems to like the food, so I'm sure it is good. It is not the most inexpensive place, but he takes a little more care, so the price is comparable to the other homes that I looked at. Also, I picked this one because of the size, and it seemed to be a wonderful place, too. The care, their philosophy, and what they do with people with dementia is outstanding.

I visited this facility

I was extremely impressed with Azura Memory Care of Oshkosh, especially the longevity of the staff. The rooms were adequate. The person who took me on the tour was well-informed and had excellent vocabulary. She gave us a lot of scientific background of their treatment program.

We want them out with us engaged in smiles, reading the paper, having a cup of coffee, making cookies, listening to music, and planning our next outing into the community. I am a friend or relative of a current/past resident

Mom has only been at Azura Memory Care for a few weeks, and we like the setting. Everything is geared specifically, and everyone is looking for something individual for their parent or loved one, and for us, that was the best place for her. I like the idea that the place was small, intimate, and more homey like. They did a very good job in sitting down with us and explaining everything. What we like about is that her room does not have a vacuum or kitchenette because for people with dementia, they don’t need all of that.

I visited this facility

Azura is a very small facility, they only have 8 residents, and they have small, hospital-type rooms with no bathrooms. It was homey, the director was super helpful, and he did a number of follow-up calls. It was quite expensive. You need to use private pay for 2 years before they make arrangements to use public funding.

We are so glad that you came to visit us at Azura Memory Care and that our Executive Director was helpful to you during your search. Our home was purposefully designed to support and engage our residents to continue to live life to the fullest despite their disease diagnosis. In fact, we have created a space where every aspect is meant to feel like home including a bedroom, which is just that a place for them to rest and relax, but not live. You see many with dementia won't come out of their room, unless you engage them with other aspects of home and personalized things that bring them joy. We encourage our residents to come out of their rooms and engage in exercise, help us bake the favorite chocolate chip cookies, sweep the floors, tend the garden and sit by the fireplace. We sing with them, converse with them, laugh with them and help them live rather than just exist. Care of this caliber is not inexpensive and on top of this our Team is specially trained through our exclusive MOSAIC training program on ways to understand those with dementia, communicate and approach them in a manner that cuts through the barriers of memory loss and to engage them personally to find joy in life. However, we are able to provide this service at less than $8.50 per hour, which includes all meals, housekeeping, laundry and linen services, assistance with daily cares like grooming, bathing and dressing, medication management, incontinence assistance, mobility and mechanical lift oversight, personalized activity programming and a trained professional to hold your loved one's hand and hug them when you are not there. Plus ,we continuously provide this exceptional service 24 hours per day, 7 days per week and 365 days per year. When you look at the amazing care your loved one would be provided at Azura Memory Care it's definitely worth every penny.

My mother moved into Azura Oshkosh in early March. The facility is well-maintained, the staff attentive, kind and caring, and the food home-cooked and delicious. The director, David, goes beyond what anyone would expect in helping residents and their families. I am so relieved that mom lives there, especially since I live out-of-state.

I am a friend or relative of resident

The staff at Azura Memory Care in Oshkosh are an absolute gift from heaven to our family - both to my mother, and the rest of us who have entrusted her care to them. I simply cannot say enough about the tender, encouraging manner in which they all interact with residents and their families - whether it's the middle of the night or at the light of day. They do, with never failing patience, what we wanted to do but could no longer with Mom's advanced dementia needs. The place is a warm, inviting, immaculate, organized home for residents with a reasonable, respectful flexibility that you would want in your own home. I cannot say enough about every experience we've had with every staff member, as well as the cooperation of their home office. We're blessed to have Azura Memory Care in our lives.

I visited this facility

We visited Harbor House Oshkosh while in the region looking at facilities for our uncle. We did not have an appointment, yet the staff was very accommodating and took us on a tour. The outside appeared very well maintained. The grounds were nicely landscaped, with nice seating areas and tree-lined walkways. The fitness center was nicely maintained with training as necessary, who assist the seniors as needed. There were many rooms for activities like the library, card and other games, exercise, crafting and many other hosted events. The generous living spaces were beautifully decorated with lots of fresh air and light. All units were well designed and efficient. The dining area was spacious with beautiful furniture. The menus all had healthy food options for all kinds of diets. Healthy snacks are offered all day and into the evening, as well as whenever residents request them. The food we saw looked and smelled delicious. This is a very nice living environment for seniors with helpful, caring and kind staff.
