Having my dad in Chalet Rehabilitation And Healthcare Center was great. The facility was clean and the staff was friendly. He's got a shared room but they also have an option for a private room. He does some occupational, physical, and speech therapies as well. They have games and I've seen where they've had get-togethers, where the celebrations for the guests that's there, like maybe birthdays and stuff like that.

Don’t answer call lights, don’t pass out pain medication one time, no one ever answers the phone. Horrible patient care

Your family member will NOT be taken care of as they should. They’ll be left in their urine or be told they don’t help people to the restroom. The food isn’t edible for any resident there it seems. I spend about $1K every two months bringing food in to my grandma. An acceptable dinner from them is tots and corn muffin. Just avoid it at all cost.

My mom has been in the nursing home for 10 years and I was recently discharged from there after 4 1/2 months of therapy. There were times they were very short of help but they worked very hard to get things done. I made a lot of friends and I'm walking again which I'm very grateful to all of the therapist! They are serious about getting the job done but at the same time they are very nice. Everybody did a GREAT job and if I need rehab again I would go back there! The place was clean and the food was great (I gained 35 lbs.). Thanks everyone for making my stay enjoyable!!

Mom went to Decatur Township Center. It was highly recommended, but I wouldn't give it more than a 2 and a half. We were highly disappointed in them. She was barely out of her room. They took her to physical therapy then right back to her room. There wasn't much interaction. They only showed up for food and meds. At the time, we thought it was nice-looking and was kept pretty clean.

My grandfather stayed here. It was a never ending battle to get them to follow his diet restrictions. He was in kidney failure ad a diabetic. His Lasix (water pills) were as high as they could go. Still, a nurse would bring him a gallon jug of pickles every week and a candy bar every other day. No one would listen when we expressed our concerns. Even his cardiac doctor told us he would prefer to keep him in the hospital than to send him back to this place. It needs to be shut down before they kill someone because of blatant neglect of patients' medical needs.
