The only reason I am giving such a good review is because of 2 beacons of light. The first is the director of activities, Maria. Her presence has helped me to feel safe. The second beacon of lighti is the therapy team. They have been very understanding of my physical situation. These people have shown much compassion. I also include the maintenance team and thank them and those fore mentioned for what they have done for me.

My friend is a current resident while awaiting cardiac surgery following a hospital admission for sepsis, heart failure and a heart attack. I would never send a family member or loved one to this facility knowing what I know now. I had to call the OLC, adult protective services and the long term care ombudsman within the first two weeks of her stay. The staff has been unresponsive to my friend's request to have her adult diaper changed, did not have transport for an important medical appointment, was unresponsive to requests for medical evaluations for pain and low blood pressure, and I caught the staff in several lies. Please avoid sending you family members to this facility if possible.

Envoy rehabilitation and nursing home at 900 Virginia Avenue in Alexandria, Va. is a total nightmare. My husband was there April 15 to May 7, 2022 following a hospitalization for a fall. The nurses and aids were unkind and unhelpful. Yelling outside the door waking him up and disturbing him. They didn’t greet him nor did they help him to the bathroom. It took 20 minutes to respond to a call bell. I needed to hire someone when I couldn’t be there. There was no daily care: teeth brushing, washing hands after bathroom, hair. The water container was never cleaned. Maybe rinsed but ice was continually added in the container. Disgusting and unsanitary. In addition it wasn’t placed where he could reach it. The bathroom was filthy as well. The food was nauseating. I have pictures to verify. He lost 18 pounds since his last doctor visit a month ago. My husband was shutting down. No one even noticed he wasn’t eating or drinking. When I talked to the nurse administrator, about the weight loss she asked how I would know. After being married 44 years i knew. She said they weigh on Wednesdays. Never gave me the weight he was when he came in nor the weight during the time he was there. My husband was shutting down. He was misdiagnosed as having shingles so he was kept in his room without being able to leave. No windows to open. No walking. Even though I knew it couldn’t be shingles because it didn’t follow a nerve path but she showed half a picture to the doctor and over the phone the doctor agreed with her. I was unable to talk to a doctor. I talked to our private doctor who told me to talk to facility doctor. i was not ever able to talk to a doctor. The nurse told me to tell her what my doctor said. Very inappropriate and a violation of our privacy. One nurse refused to look at the rash as it continued to worsen. No pain. No itch. Staff asked me whether or not to give the shingles medicine. Me. I couldn’t even talk to the doctor. Very bad medical care. This place is a totally awful. Skinny people living in hospital gowns in dark rooms. Heartbreaking. Social services need to go visit the first floor of Envoy to help these people we left behind. The first floor was where this all happened. Thank you. Marilyn Siwek

My friend lives in this facility - an accident of fate! He is a brittle diabetic who had a leg amputated and then COVID 19 hit and he could not come home. The food-if you can call it food- is often unrecognizable and is inedible. He is a diabetic but routinely receives starchy bread, powdered eggs, powdered mashed potatoes- and he is forced to either eat it and suffer the high blood sugar or toss it away. The food is inappropriate for any diabetic and his eyesight has become worse, his skin is worse. He has had COVID once and is likely to get it again because the floors and bathrooms are filthy. Two days ago he showed me a photo of human excrement on the hallway floor, left there for days. The shower area was no better- with a dirty diaper and bowel movement left on the shower floor! NOBODY tried to clean this dangerous mess up for days. He had roaches in his bed last night, and his room mate confirmed this. The state needs to shut this vile and substandard place down!

The building is clean and the food good I will give thence that much However, The staff that I have met so far are just awful, I am back on the Quarantine hall The Nurses are all from Africa They are very critical of the US@ Pauline withholds medicine from me all the time the male nurse I can’t understand a word he says Then there are the CNA staff They come I and get on their phones and don’t do enough to Warrenton a $50.00 check

During COVID I received regular updates about infections at Envoy. My relative was in isolation but the staff made sure we spoke or FaceTime regularly. I appreciate the peace of mind.

My mother has been a patient at Envoy of Alexandria for a few months rehabilitating after she had hip surgery. I have to say that, other than a isolated issue, I have been pleased with the care that she is receiving. The nursing ,PT and OT staff have been attentive to her needs. The case worker has done a great job keeping me informed and assisting me in the details necessary to transition her back home which will be in a few days Thank you for her care..

My cousin resided at Envoy of Alexandria, she was challenged by chronic ALZ. She walked in there able to address all of her personal needs, and communicate her concerns. But within 4 months, she was glued to a wheelchair and unable to address any of her personal needs or communicate to anyone. She received inadequate care with her chronic ALZ. The staff spends most of their time providing the patience with medications, but I witnessed minimum care. Often, there was no one available to feed her or take care of her immediate needs, bathing, grooming, and ongoing personal care.

This facility is horrible!!! I have work experience in the nursing field so I know first had. My father was transferred to Envoy from Sentera Hospital in February. I went to to visit him the second day he was there and the condition he was in was awful. The room very small (it was a two bed room on the 2nd floor) The room was very hot to the point my father was sweating horribly and They had the heating furnace right next to his bed on high. Due to my fathers condition to where he can not talk or move made me very upset the day I went to visit him that day. Also when I asked to speak to the head nurse. He was very uncaring and inconsiderate. I asked him if they can give him a suction machine and for someone to clean his mouth because his mouth was crusted and thick mucus/saliva was in mouth. The nurse had the audacity to tell me that he thinks my father doesn't need the suction machine. That was strike 3 for me!! The NEXT early morning I receive a call from Inova hospital saying my father has been admitted from envoy. The hospital told me he came in with a tempature of 107 and was breathing abnormally. My Point is DO Not trust Envoy of Alexandria. The staff is Horrible. The facility reeks of urine. The atmosphere is outdated from the moment you walk in the lobby to the condition of the nurses stations.They need some serious renovation!! I took a look at their room where the "rehab" is taken place. And I was very disappointed in the lack of equipment they have to help these residents. It mad me very sad the first time I walked in there...Eventho my father had a very short stay there, it is very unacceptable at the quality of care. Thank god he is in a better and more clean facility.. I advise you to take a tour and actually ask questions and do research before your loved one is admitted to this place.

HORRIBLE PLACE!!STAY AWAY IF AT ALL POSSIBLE!! I was a bit nervous about placing my father here after reading some past reviews but unfortunately it was all we could afford at the time with the help of the insurance co. The facility is run down and sub-par in my opinion. It always had an awful stench, the temperature in the building was always rather high, the food was equivalent to that of the local county jail. The administration staff are very cold and unwilling to help out. The only positive about this place and the reason for the 1 star was the nursing staff. They were very caring and attentive to my fathers needs. In closing, if you are able to have different options for your loved ones, I would suggest this place be left off the list.

Was there for 30 days, you can do better. The CNAs were poorly educated all mostly from west africa, rude and borderline mean. At least I could fend for myself but others less mobile were being ignored, I had to wait for over 2 hours once after I hit my call bell for someone to come. Nurses would leave premises regularly in middle of the night, so there was no one available if an emergency.

Online research suggests most skilled nursing homes are given poor ratings by upset family members. Envoy has poor ratings too. However, after learning online what questions to ask while considing rehab, a tour and meeting the staff, we chose to go there. Our experience was good. My father came for rehab. physical therapy was great, food was decent but the nurses could have been nicer.

This placed should be razed to the ground, preferably with the staff still inside. Imagine seeing your loved one taken there in their final days, to be shoved off into a corner and ignored. You try to call the phone in their room, but you get no answer. You arrive in the morning to find them fallen out of bed, covered in bruises, no call button to be found. You then summon the staff, who appear to absoultely not care at all. You request an ambulance be called, but the staff refuse. Ultimately, you have to call the police to get your loved one taken back to the hospital. All this time, you are treated rudely, indifferently, and as if neither you nor the patient matter to them at all. I would rather be pushed out into a cold, snowy forest than be taken here. This place is a disgrace to hospices and nusrsing homes and should be sold for scrap.

I was discharged on August 19, 2014 from Fairfax Hospital and sent to Envoy, room 223 for rehab, short term. The lady sharing the room was a long term patient screaming not to be thrown in the trash. There was black slime on the radiator AC vents. One could see the springs of the bed's spring box as there was no mattress on the bed. When friends checked the facility, they were shown a clean, beautiful suite - what a farce! I was lucky - I called my son and asked him to take me out - they kept us STANDING until they found a liability release form for me to sign. Could not use the bathroom, even though I was right after chemo, as the food tray for the lady blocked the door to the bathroom. The nurses are loud and nasty - CARING is not a word in the vocabulary of these people!

Stay Away From this Place!!! Please listen to me!!! I put my uncle in here in May 2013, my uncle couldn't talk, he had a treak in his throat. I would come see him 5-6 times a week. Family would see him a lot. Nursing home always had a sour smell. My uncle started to look like he was getting depress there. After being there for a few weeks, he stop breathing and was revived. Was at hospital for a month when it was time to be discharge from hospital and go back to Nursing Home, my uncle put up a fight. He finally told me what was going on. He said on more then one occasion at Envoy he would write and tell the staff he couldn't breath and they would tell him, don't worry you will be ok. That's what happen that night. I sent my uncle to anther Nursing Home and he pass away a couple of weeks later July 30, 2013.

My mother was treated terribly in this place. She fell so many times, without having anyone to assist her. She was also dropped off in the hospital without me knowing of her whereabouts until an hour later. When the hospital were ready to discharge my mother under Envoy's care, no one from this facility answered the phone or were willing to take her back. I was told the next day that my mother was not welcomed in this facility because they were not as equipped to handle her...As a sick person!!!! My question is: What is their job then if they can't handle and elderly sick person?? This is a horrible, unprofessional place to take your loved ones to. STAY AWAY FROM THIS ENVOY.

For the whopping self-paid fee of $295 per day, my parent was placed in a dismal, crowded room and barely received any care or attention. There are much better choices out there that charge significantly lower fees. If you truly love your loved one, look elsewhere.

I saw a lot of things here that I liked. The home is very, very nice and is I thought the patient rooms I saw were decorated nicely...like a home. More to the point, the nurses here are so nice to everyone and went out of their way to help me take my blood sugar because I became dizzy and light headed while I was here. They just took control, took my sugar, and gave me some orange juice to bring my sugar back up to normal. They all just fawned over me, and to be frank, I enjoyed the kindness.
