Cornerstone is the BEST you will ever find. They treat the people that live there as family. We have regular care meetings. If there’s any issues or concerns they will call. I couldn’t ask for a better place for my mom.

I am/was a resident of this facility

Avoid this place! I had knee replacement and needed assistance to get to the bathroom. They rarely answered the call button in a timely manner and then left me sitting on the potty. They seemed irritated that I had to go every two or three hours. The food was awful....served on paper/plastic plates, was usually lukewarm and tasteless. My family brought food in for me. I received PT once in 4 days and no bath. (There was no shower in my bathroom.) The homemade shower chair the did have was in a tiny room with no way to elevate my leg. I did not receive the ice therapy or compression stockings as requested by my doctor. As a result I had major bruising on the underside of my leg. Good luck if you expect a private room. They said that would be an extra &79 a day. They waived the fee when I said we will go elsewhere. AND good luck if you have a TV that is larger than 12 x 14.....impossible for those who are hard of hearing to read closed captioning.....and the TV becomes important when one is bedridden! I left as soon as we could transfer out and found what a truly professional facility is supposed to be like!

As we face the hardest transition in my mom’s care, Cornerstone Gardens has provided her with excellent care. My mom’s care is tough; the team works together to help her get better. James and Kathy are great nurses who have been more than happy to answer my questions and take time to call me as needed. Falyn from PT is super sweet to my mom. Rhonda, the social worker has been very helpful with providing me with info about mom’s plan of care and community resources. Ryan, the director is great too. The facility is very clean.

My mom was here for rehab for 2 weeks. I suggest anyone putting their loved ones here to please surprise visit the nurses station! Pam head of nurses and Francis are very rude and uncaring!!!! The food was horrible and the beds were very uncomfortable! Very unorganized! The only reason I give it 2 stars is because it was clean and had a few good nurses aids! My mom waited for hours after pushing her call button for bathroom! Completely Not satisfied!!!

Aesthetically, the place looks great. Beautiful decorations, clean and friendly....HOWEVER, the actual facility and the quality of care is horribly overrated. There are a few folks I can name that are genuinely caring and will go above and beyond to listen to and address concerns. Pam is not one of them. Very poor communication skills for a head nurse. Ryan, in the same category. They live by watching videos that are only monitoring hallway traffic. So, what happens inside the room is anyone's guess. It seems that the caregivers are aware of this as well because I often pass rooms where they are sitting on their phones out of site of camera. When factual information is given to the "head nurse", Pam, and confirmed, ie: my loved one sat with the call button on for an hour to an hour and a half without so much as a peek in on her, I am told there were "five call buttons on at the same time." As well as- "WE ARE UNDERSTAFFED"...over and over and over and over again. When paying for the care of my loved one, this is not my issue. HIRE someone or stop taking in new patients. I say patients because that is exactly how they treat people even if they are coherent and understand what is going on. It seems that they want to fill the rooms with folks that do not have a say and do not know that they are being poorly treated. In addition, my loved one has been called stupid, tossed around and bruised, and made to feel bad about asking for help, all of which is documented. I have made numerous calls and have been argued with on every call. My loved one waits one hour minimum when her call button is pushed. I know this because we have timed them now on numerous occasions. All so that she can get help to the bathroom, of which she has now attempted twice on her own with the possibility of falling and hurting herself. I will say this...Susan, Jeremy, Eve, and even Elvis that cleans the room are respectful and assist when needed. Unfortunately, they are not always there. The laundry gal is wonderful. The gal that leads BINGO, the hairdresser, the gentleman that comes in at night with the red shoes- all wonderful and attentative. In addition, my loved one has not been eating because the food is so horrendous. Either too tough or so mushy that it does not have flavor. We take her to eat outside of the facility and she eats as if she has not eaten in days. I had high hopes for this facility. What I have seen now is that it is all smoke and mirrors. I did find the copy of the State report, hidden in the drawer up front, as my loved one pointed it out to read and I am most certain that this review will be no surprise. All of these issues have been brought to the attention of the facility and the outcome...I was asked by Pam..."well, why don't you move her?" Really???? That is your suggestion? I asked how she would feel if it was her mother waiting for an hour to have someone respond...Pam needs training, and so does Ryan. He has not once reached out to see what is happening. NOT ONCE in all my complaints. I have Pam's direct cell if anyone needs to be yelled at and/or called a liar. My suggestion to anyone considering this location is look around and do the research. Look at the reviews of this place. It is not worth the money or the worry of how your loved one will be treated. Again, look up the State findings. It's in the drawer up front, left side and about 2-3 inches thick. It's like a treasure hunt finding it...

My dad had a rod placed in his leg due to a broken femur bone. After arriving on Friday evening around 7:00 pm we got him settled in and everything seemed set for the night. Returned Saturday morning to find my dad very agitated due to him needing to go to the bathroom and had been pushing the nurse call button for over an hour. We got a nurse to help him get to the bathroom and was told they didn't know why he had been ignored. Early afternoon my dad said his leg was throbbing with pain so we asked the nurse when was the last administer dose of pain meds and was told he had not received any since being admitted. They were prescribed on an "as needed" basis, so he would need to ask for them. Of course we were not informed of this until he was already in great pain. So now we know so we tell dad he can have 2 pills every 4 hours if he needs them but he will have to ask. That night a nurse comes by and he tells her he needs pain meds in 15 min. and get chewed on by the nurse because he is "watching the clock". My mom goes up Sunday morning to find my dad standing in the bathroom trying to get to his bed. She asked him whats he doing and he said he waited 45 minutes for a nurse to come help him and no response. My mom try to get a nurse to help and the 5 or 6 nurses sitting at the nurses station just ignored her. Finally got one to help her get him back to bed and my dad tells her about the confrontation with the nurse the night before so my mom is now upset. She goes to the nurses station and starts to voice her concerns with nurse and she won't even look away from a computer screen to acknowledge her. She calls me and she is very upset. I go up and ask for the supervisor and tell her what is going on and she says "oh no a patient should not have to wait more than 20 minutes for a nurse" to which I said " thats unacceptable" no patient show have to wait more than 5 minutes to be acknowledged. We didn't get any PT all weekend. Was told when we signed up he would get 3 hours a day (not all at once) 7 days a week of PT. What we got was 5 days of 1 hour of PT, 1 hr of Occupational therapy and 1 hr of speech therapy. All back to back, along with 10 or 15 other patients at the same time. We found him asleep in his wheelchair in PT on day. After 20 days we had enough and pulled him out. When we told them we were taking him home they said give us till the end of the week and he will be ready. So we did. He was worked with those last few days and walking about 100 yards down to PT and back to his room. We had home healthcare come in after we get home and find meds that had been ordered but never given. The new therapist said the Dr. orders were no weight bearing on the leg for 6 weeks due to the broken bone. We thought he was suppose to be walking because thats what the therapist at Cornerstone were doing. My dad kept telling them that the pain was just to intense when he would put weight on it. Well now we know why. needless to say I would not recommend this place at all. Some of the staff were friendly and you can tell they have the right caring heart to help patience, but most of the others we delt with were rude and acted like we were putting them out when we would ask for help. I hate to think of the patients that don't have family helping them to stay on top of these staff members, they maybe really suffering.

We used Cornerstone Gardens after knee replacement surgery. We used them for their rehab unit because they advertised 2 therapy sessions each day. Because of the nature of knee replacement surgery, physical therapy is extremely important. We were totally surprised when an Occupational Therapist came to show how to brush teeth and get dressed...and then were told the 2 therapy sessions were actually one session of Occupational Therapy and one of Physical Therapy. The other reason for the low rating was that after multiple requests for help bathing, someone did come AFTER 6 DAYS. There seemed to be real confusion among the staff and we sensed morale was very low. One young lady was pulled from another area and told to do something she had no training for; she wisely declined, but was terminated. She and another woman left their jobs.

I visited this facility

The people at Cornerstone Gardens were very nice and very knowledgeable. The rooms were OK. They weren't huge, but they weren't small. They were limited on what you can add to the room. The facility was very clean and well run. It has extremely nice surroundings.

I am/was a resident of this facility

Cornerstone Gardens was very clean and had a pleasant atmosphere when you walk in. People seemed friendly. My mother had back surgery, and she was there for rehab. There was some confusion with her medicine. They got a list of medicines from the hospital, and there were some things on there that she hadn't been taking. Had I not looked at the list, they would have given that to her. I realized it was not their fault, but she was not supposed to be taking pain pills anymore, and her pain pills were supposed to be as needed or when she asks for them. But the medication lady or whoever came around offered them to her said well if you don't take it now, you can't get it again until X-time when we distribute medication again. I realize they had a schedule when they give medication, but if mom didn't want it right then, she should be able to ask for it and get it when she wants it -- that's as needed. So, there was some confusion with the medication, and on top of that, we paid for a private room for her. We had to pay upfront for 30 days, and she did not use all that time yet, so they were supposed to refund the amount within 30 days, but it has been over 60 days since she has been discharged and I haven't received it yet. But of all the choices in Temple, that is the place we would put her again because it is probably the nicest one in Temple.
