Admin: Not one came and introduced themselves during the client's 10 day stay. No care plan was ever discussed. Social Worker: Regular worker was on vacation and we had to use a temporary contact referred by the facility. She did a good job in helping us. Housekeeping: Just walking into the facility there is a smell of urine. The place is old with dirty or rusted equipment, poo on toilet seats, dirt in the bathrooms, and lack of upkeep. Medical Staff: The Nurses and CNA's are nice enough, but lack efficient care of clients. Major incompetence on the part of the nurses and doctor to not administer any laxatives when a client is on pain killers and has not moved bowels in over a week. Nurses and CNA's are also spread thin, often covering too many patients. Nurses should be instructed that when the client and/or a family representative advocates for an EMS to go to the ER, the facility should do as requested immediately. Not doing so, delays treatment and denies the client his rights. Rehab: The PT and OT staff were kind and did their job better than anyone else. Client received Speech/Memory Care/PT/OT on a regular basis. Recommend admin and nursing staff be more educated with protocol's in dealing with outpatient hospital returns. Witnessed 2 other rehab patients being transferred by their families due to lack of care and generally being unhappy with the facility.

I am a physician who's mother, whom by the way did not speak a word of english, was admitted to this facility due to Alzheimer's Dementia. In our family we generally try to take care of our own parents but my mother's condition was as such that she did need to be in a lock-down facility and be taken care of 24-7 due to her underlying condition. I visited my mother several times in the time frame that she was a resident there. I was so surprised to see the degree of love that my mother received in her stay there. The staff did not even speak her language and went out of their way to address her with the name we call her, to ask her simple questions in our language ( where we had made simple cards to translate) and simply to give her the love and fiscal affection that we would give her at home if she was living with us. everyone from the nutritional staff, nursing, techs, and administration were incredibly supportive and loving to my mother. As a matter of fact, most of the people that took care of her including the administrator of the facility came to her funeral I have nothing more than appreciation to this facility. As a physician, in many instances I see the humanity is lost in the midst of medical malpractice and documentation. I can truly state that I would recommend this facility to anyone, specially even if their loved ones don't even speak english. there are no words to describe my sincere appreciation. My mother is gone now but I feel that she made a great impression on the employees of this facility and with one last effort she made it known that good care still exists in this country.

When my grandfather became really ill our family decided that Richmond Health Care Center was the best fit for him and for ourselves. The current staff there are very welcoming and caring towards the patients and families alike. It was a smooth transition for all involved putting my grandfather here. I would highly recommend this facility to anyone in search of good nursing care and a clean, friendly environment,

The service provided to our family member was totally unacceptable. We would never allow them back into this place. All they do is drug the patient and not feed them. On more the one occasion when we visited they haven't even changed the sheets on the bed.

When my uncle became unable to care for himself, I had to resort to finding a place for him to live since I had no room at my home for him. I visited several nursing homes in the area, and determined that Autumn Hills was the best place to have my uncle live. The staff there were very helpful in filling out the forms necessary to admit him. They even helped me complete the forms which needed to be sent to medicare/medicaid. The residents there were well taken care of, medication given on time and even the meals were rather appetizing. My uncle enjoyed the community time, the time when the residents met in a meeting room to listen to music or play games such as bingo and dominoes. They had special events for holiday times, even having a Christmas party which invited all family members. For Valentines Day, they had a King and Queen crowned. At first, I hated to see my uncle placed in a nursing home because he did not really want to go there. After he had been there a couple of months, he said he was very happy there and met many new friends. I feel this was a positive experience for my uncle because he made friends and was well taken care of.

it smelled like pee really bad, but most of them do, the activities director was very sweet and it seemed as though the staff genuinely cared about the residents which was good, but the facilities were a little old and out dated. They let the residents go out on the front and back porch, and the residents all seemed to be in a very good mood. The only downside was the pee smell that made me gag a little when i walked in. They were playing bingo near the kitchen, and the kitchen smelled really good. The staff were very nice to me, if my grandmother would have let us move her out of her house this would definitely be one of the places we were considering, it's close to home, and in a very convenient location. If I could ask for anything to make this place more inviting would be a few candles lit, and maybe not have a bunch of the seniors that are almost in a vegitative state be out and able to harass people that walked in there.
