I am a friend or relative of a current/past resident

We placed my friend at Fall Creek Rehabilitation And Healthcare Center. She was doing inpatient rehab, OT, PT, and skilled nursing care, and then we just transitioned her to the long-term care side. Everyone is very attentive. If you ask for something, you don't have to wait around. They're very quick to try to help you solve a problem. They're very friendly. My friend says nothing but good things about the care that they give her and the help that they give her. The PT and OT team are phenomenal. They worked hard with her every day and were patient and kind. So, we've had no problems. They always have activities, like most facilities. Everything that they've done for the residents, everyone seems to be very happy and satisfied all the time. The rooms are always kept clean. The halls always look great. Everything's well decorated and well-lit. It is very nice. It never smells. I've only had the food once, but it was tolerable. It's cafeteria food, basically, but they have a good variety on the menu. They have a posted activity schedule in the rooms, in the halls, and on the windows. There's always someone in the activity room. So, even if there's not something going on, I've gone up there even in the evening time, and they leave the room open for people to read or do puzzles or listen to music, but they have structured activities all the time. I think she's getting great service and a nice facility for what Medicare is paying (she's a Medicare patient). The administrative staff did a phenomenal job working to transition her and get her approval. They transitioned her to traditional Medicare off of one of her Medicare HMOs. So, it is providing reimbursement and services for her while she was in the facilities and what she had with her Medicare HMO. So, they seemed to be in her best interest and we appreciated that.

My sister has already completed her rehab from Fall Creek Rehabilitation & Healthcare Center. They helped her with exercise, with taking her temperature, and with rehab. The staff were very helpful and eager to attend to any needs that may have come up. They have a home nurse and a physical therapist coming in once a week. The place was very clean and smelled very good.

I visited this facility

If this company would pay their employee a decent salary they might keep some help. They are always short-handed, no proper sheets, pillows, pillowcases, diaper' workers, food, communication, the list can go on. They do have some devoted workers. But why work them down till they leave. These people are human, and pave the road for us. They are not DEAD yet, Treat them with RESPECT. You will get old one day. God, Please take care of our Love ones in this PLACE. Still Praying for you all, Staffs and Clients.

Don't do it. I hope they get shut down. Staff is awful. They just don't care.

I am a friend or relative of a current/past resident

My wife has been to Fall Creek Rehabilitation and Healthcare Center. She went there the first time for rehab and then when the rehab period was over, she stayed there in a nursing facility. The appearance is nice, it's always clean and the layout makes sense to me It's easy to navigate and figure out where you are all the time, and I'm assuming that's the way for patients as well. Everybody is polite, nice and efficient, so it looks very good to me. There's not much they can do because of the rules that's done for the pandemic. They do try to get everybody out and have a meal together. But they gotta observe distancing, so that makes it a little bit besides the point. In my wife's case, she happened to get a roommate who has been a friend for nearly 30 years and so they're pretty happy in their own little room. The dining area looks fine to me. The food itself is institutional, which is substantially less good than what she's used to. But on the other hand, I think it pretty much refined her. It seemed, on the case, she doesn't complain and she had a good attitude. They have a beauty shop, lunch room, and there is sort of a little fireplace. It's not being used because of Covid, but they could once it's over. If there's an available interior courtyard that's open, that would be very nice.

I visited this facility

The rooms at Fall Creek Rehab & Healthcare Center were pretty nice. It's a fairly new facility, and we got on a waiting list with them. They had an activity room and a courtyard.
