ProMedica Skilled Nursing And Rehabilitation (Perrysburg) was beautiful. The rooms were very good and the layout was also beautiful with big windows in them. Everything was set up for your comfort. It is very, very nice. The meals were great. The employees were excellent. It was a beautiful experience staying there. They had a lot of group therapy where seniors would gather together and they loved it. They had a great exercise room, a great physical therapy staff, and an occupational therapy staff.

ProMedica Skilled Nursing And Rehabilitation (Perrysburg) was beautiful. The rooms were very good and the layout was also beautiful with big windows in them. Everything was set up for your comfort. It is very, very nice. The meals were great. The employees were excellent. It was a beautiful experience staying there. They had a lot of group therapy where seniors would gather together and they loved it. They had a great exercise room, a great physical therapy staff, and an occupational therapy staff.

My brother was admitted to this facility after surgery to close up a wound from Fournier's gangrene (look this up as it is a very serious, horrific, and graphic wound in the perineum area). This wound was left open for almost 2 months to make sure they had gotten all of the infection and to allow healing. Because of his wound and it's location he can't sit up or go to the bathroom without assistance. He can walk for about 50 feet with assistance from someone. He was sent there in an ambulance and dropped off in a room. NO ONE came into his room for 3 hours. There was NO COMODE. They made him use a bed pan and it ripped opened the stitches from his wound. He was taken back to the hospital so his wounds could be checked. He was then sent back to this facility. His wife took a photo of the prescriptions and handed the paper prescriptions to the ambulance team. Somehow they were lost in transit from the hospital to this facility. My brother still has not gotten his meds. My brother also takes heart meds and asthma meds. He did not have these meds for 2 days. I would go on, but I crying as I write this. This facility was recommended by the social worker at the hospital because the hospital and this facility are supposed to be able to see his medical chart and history. Because of Covid his wife and family members are not allowed to visit. I would not allow anyone I love or care about to he sent here. This is a truly horrifying experience for my brother and his wife.

The Therapy Team at this facility is excellent. Their director is also excellent checking with me daily making sure I was satisfied and improving. I had the pleasure to work with Tasha, Dawn, Ryan and several others. Very professional. They worked the entire time with me giving me suggestions of things I could do between visits. This I feel helped greatly. I am staying a total of 13 days and then off home to care for myself. I feel I am more than ready to go home safely. My nursing staff has been great too. Everywhere has work to be done in these very trying times, but I felt everyone has been great.

My husband was admitted just to get stronger through rehab. His rehab was very sparse and he was given a medication without his medical power of attorneys ok, which caused him to become frail overnight. Just a few days after,his continued downfall was ignored by staff that should have done test at least, no baths of any kind in 6 days. He had heart failure and went to hospital with dehydrated, stroke, malnourished, and was put on a ventilator. Within less than two months, my husband died! Positives were, most staff was nice, but drastically understaffed at night. Rooms were kept clean and comfortable. Most, "not all," nurses were caring and friendly. Some were rough with my husband and non caring.

They care has a lot to be desired. Nearly killed my mother there. The Pulmonologist of her care in the Hospital Said they FAILED HER. She had kidney disease and was never put on a Diuretic. Not too many bright people there Including which the staff call themselves Nurse practitioners joke. So she was basically drowning in her own fluids. She also contracted some infection Which caused an infectious disease Dr to be called in because she got some contact infection. People not washing their hands. Horrible food. She would not eat unless I brought her something Hopefully you don’t need to send a family memeber there. Such poor ratings even with Medicare. I wish I knew all this ahead of time Good luck

It took them three days to meet our Mother's nutritional needs. Specifically, during the first three days of her stay, they kept sending food that she couldn't eat since she doesn't have any teeth. Also, they failed to send any boost/ensure drinks until the day we left the facility. The facility was well aware of the fact that our Mother was on a Dysphagia three diet and that the hospital dietician ordered boost/ensure drinks to supplement her diet. Also, the wait times, after pushing the Nurse's call button were horrible and unacceptable. For example, our Mother had to wait 12 minutes before a nurse's aide brought her a bed pan and then she left her on the bed pan for another 16 minutes. The Nurses at this facility would not even offer to help our Mother when she needed to use the bedpan and when they saw her on the bedpan they turned around and walked out the door as if they were above helping a patient on a bedpan. We were 100% dependent on the Nurse aides or we had to help our Mother ourselves.

I have visited this facility several times as I used to be an ambulette driver for a living (wheelchair transportation. My experience with this facility has been fairly pleasant. The nursing staff all seemed to be very friendly and they have an excellent physical therapy department. The facility has always been clean and organized. The patients that I transported never had anything negative to say about their stay there. Overall I believe this is a decent place to put your loved one if need be.

In October of 2007, I began a position as Staffing Coordinator for a company called Adecco in their RPO (Recruitment Process Outsourcing) department. My main client while working with Adecco was HCR Manorcare, one of the nation's top Skilled Nursing Facilities. My main responsibility as Staffing Coordinator was to source and recruit for Physical Therapists, Occupational Therapists, and Speech Language Pathologists for a number of HCR Manorcare's 500 facilities nationwide. In the course of my employment at Adecco, HCR Manorcare presented the opportunity for me to visit one of the facilities nearby my office in order to get a better feel for what happens in the facilities with regard to patient care. Upon arrival, I was greeted cordially (with a handshake and a smile) by the Director of Rehabilitation and several of the staff members. I was first given a tour of the patient rooms, the common room, and finally the Rehabilitation room. Here, there were several machines that patients in Physical Therapy could utilize and a kitchen set-up for Occupational Therapy patients. Speech Language patients were given an area for group as well as one-on-one therapy sessions. And the main attraction was the Nintendo Wii system. We were shown how patients in all areas of Therapy were able to utilize the gaming system to help with Physical, Occupational or Speech progression. The demonstration was quite impressive, and the progress of the patients using the game proved to great throughout my employment and contract with HCR Manorcare.
