The level of customer service and compassion is deplorable. I have been calling this facility multiple days in order to speak with someone. I was given the number for the nurse supervisor, and when I called that number there was no answer and the voicemail is full. During this time when visitation has been restricted, it is imperative that health care providers ensure that there is good communication with patients and their families, in order to minimize the likelihood of error and to maximize the level of care.

My relative is currently at Crescent Cities for rehabilitation. The level of care is unacceptable. It's very hard to get someone to answer the phones to get updates. Four different relatives had to call 4 different phone numbers at the same time to reach anyone with no luck. On one occasion a person answered the main number and said that she didn't know how to transfer calls. It is very frustrating especially during COVID when you cannot visit your loved one. I have placed calls to the Director who hasn't return calls. It has been 3 days now. My relative's health has declined since being there. We sent them there for their health to improve. My relative called me and said that they had left her on the bedpan for over an hour. She was also given lunch while still soaking in urine. *unacceptable* We are making arrangements to get her home ASAP.

My relative is currently at Crescent Cities for reahbilitation and we are trying to get them transferred. The service and level of care is unacceptable. Nobody provides us updates so we are constantly calling to receive updates. When we are transferred to the nurses station nobody answers the phone. This is a daily occurrence. It is frustrating. We have placed calls to the Director who doesn't even have a voicemail greeting to identify who they are and no return calls have ever been made. My relative's health has declined since being there. We sent them there for their health to improve.

My relative is in Crescent Cities Center for physical therapy and occupational therapy, but she didn't like it there. The staff's service wasn't very welcoming or friendly, and this goes for the nursing staff and aides. She's in a shared room, but they also have a variety of rooms available.

Your loved ones needs will not be met by the nurses at this facility the therapists are great but the nurses are rude, inpatient and will move slower than a snail to see to your loved ones most basic needs. The rooms are a nice size especially considering you’re sharing not just with another person but cockroaches as well.

Crescent Cities Center was an OK facility. The physical therapy group was very good. The grounds looked well kept and clean. From the outside looking in, it seemed like they had activities for the seniors, and the staff seemed friendly.

My wife occupies a bed next to a woman who is forced lay in her own bowel movement for the past hour, she waits for someone to help her. For most of that time she has been pressing the nurse emergency button and even screaming out for help. What kind of tourturous place is this place. Is the staff so short handed or just don't care. I guess the answer to that question is given to visitors when we first enter the first floor lobby of this facility and are forced to inhale the stinch of urine as they struggle to make to the room of their love ones. It. is deplorable to think what these patients must endure and survive on a Dailey bases. God help them. God please help them because it seems no one here is likely to.

Crescent Cities Center was good. They have a doctor for any customer who has Kaiser Permanente. They even have an office in there to coordinate their care. It was good. They had very nice and beautiful staff there. They were kind and very astute, taking care to explain everything fully. I felt relaxed, and there was a peaceful environment. It was an old hospital, so it was very big. The therapeutic area was good, and they had a lot of good equipment.

My brother was at Crescent Cities. He didn’t get very much service there. They weren’t changing him. They weren’t looking after him so we had him moved to another facility. Just about everybody there was complaining about the staff and the care that was being provided. The facility itself was nice, but the care wasn’t.
