my grandpa was a resident here. he jus recently passed that’s why i’m writing this. I DO NOT RECOMMEND. when i picked him up he did not have the proper care he needed. his nails weren’t clipped he didn’t have a shower. they didn’t give him any excersise that he needed/wanted. when we dropped him back off they were rude. they were smoking while handling him. at one point one of the nurses took his phone and they lost it so no one could get ahold of him. please if you love your loved ones do not send them here. they need different employees or they need to shut it down. those people that reside there need the proper care that they don’t know how to give.

Eagle Creek "rehabilitation/ nursing home" has been one of the ABSOLUTE WORST experiences of my life and have have been through a lot of horrific things I in my 38 years on this planet. With the exception of a handful or less of staff members that have been truly amazing . But in general eagle Creek "nursing home and rehabilitation" this place is a joke and if you truly love and care about your loved one you will NOT send them here!!!!! Because chances are your loved one will degress and not progress in this so called facility. In my personal Opinion and experience here has been tramitizing and life changing in a not good way. I DO REPEAT IF YOU LOVE AND CARE ABOUT YOUR LOVED ONE YOU FIND ANOTHER PLACE TO PLACE THEM BESIDES HERE AT EAGLE CREEK!! And with that being said I would like to acknowledge the staff who has the compassion and heart for their job here and because of the all around general lacking and poorness of all their jobs here the few great people I have met here who care about their job and residents they don't get the praise they deserve so to Tiffany the amazing D.O.N. WHO IS FABULOUS, SAMARION P. THE CNA WHO ROCKS IN HER JOB AND HAS THE DEEPEST COMPASSION I HAVE SEEN OR FELT FOR ANY OF THEY OTHERS IS AMAZING, ALISHA THE NURSING UNIT MANAGER IS PROMPT AND CARING OF HER RESIDENTS I COULDN'T BE ANY HAPPIER WITH HER AMD HER CARING, AS WELL AS BRIDGET ANOTHER NURSE I HAVE FREQUENTLY IS GREAT AND AMAZING HERSELF. AND THE THERAPY DEPARTMENT ALL OF THEM ARE GREAT! Beyond that in my personal feelings the rest need to have their self's and this place shutdown!!

I have a brother at the Eagle Creek facility and after the first week we have encountered issues. First the cushion for his wheel chair came up missing. After complaining to several staff an the administration the cushion was replaced. Next, several clothing items came up missing. Although a complaint was filed and the situation investigated the issue has yet to be resolved. I feel if their investigation garnered no results the should have replaced the missing items. Finally, my brother's cell phone came up missing. A complaint has been filed, but based on my past experience with this facility I am skeptical that anything will happen that will result in my brother's phone being found or replaced. Their policies and procedures on patient theft needs to be reviewed and revised to deal with circumstances such as this more effectively. I was Deputy State Health Commissioner for the State of Indiana and as such was very familiar with the COMPREHENSIVE CARE FACILITY (NURSING HOMES) LICENSING AND CERTIFICATION PROGRAM. Therefore, I am probably somewhat more informed and attentive to issues such as this. Regardless to my past history. As a family member I don't think the facilities operational protocol is as efficient as it should/could be.

This place is horrible ..my relative was here and they gave him mercer infection...even this picture is a front because I went in this room to get another eldery patient a cup of ice ( because he waited on assistant for 2 whole hours) and when I entered this room there was dishes stacked on top of each other and nets and flies all around it...they should really do a surprise inspection on these people at least 3 times a week ....that's just how bad this place is. It brings tears to my eyes that no one knows how horrible and unclean and how cruel these employees are.

My husband was released into this facility after his discharge from the hospital. Upon arrival his room wasn't prepared, nor was his medication put into the system. He was just put in a room with another patient and left there until I inquired about his medication for the evening. Then we were told that he'd have to wait until the male nurse got done passing the other meds. Then he'd tend to him. It was 1130pm before he came back with the meds for my husband and he didn't have all of them. Said he was working on it. His television didn't work... He wasn't given a call light or urinal... He wasn't offered anything to eat or drink. And no-one even tried to help him get settled in. Nor assist him with his belongings!! The facility had a patient sitn in the hallway hollering for help in which upset my husband very much. Then staff were cold and distant!! And the food on the menu is horrible. Almost every time he pre ordered his meals he was given the incorrect food. Not to mention No remotes for the televisions. The only time a patient seems to matter is in the daytime when all staff are present. At night and evening time its like every man for himself. Literally!! This facility has earned the title GHETTO!! In soo many words.

When I got out of the hospital, I was taken to Kindred. It was terrible. I can't say one nice thing about it. It was depressing. The staff was rough-and-tumble, and the patients were rude. At meals, I said to people, "May I join you?" and more people than not said "NO!" and I had just come out of the hospital and I was in pretty rough shape. All they had were double rooms that were very dark. The decor was always dark brown and very depressing. Right across the hall from me, there was a mental patient who screamed all night long. It was absolutely terrible. When I moved in, it was about 1:00 or 2:00 in the morning. They moved me into a room where there was a woman with a broken leg who screamed all night long. The staff came in and laughed and partied with her. I couldn't get any sleep, and the next day I said I wanted to be moved to a single bed room. It was one of the worst experiences of my life.

My mother stayed there. She needed rehabilitation from an amputation. The rooms are a good size, but low light. We had to ask them to come in and clean the bathroom as the floor and walls were dirty. The a/c vents above her bed were dirty also. Everything else seemed clean, but very low light inside the building. The morning shift staff was very courteous and helpful. HOWEVER, the night staff was horrible. My mother is also diabetic and high risk. When she pushed the button for help (we timed it) it took the staff 30 mins or more to arrive. She wasn't given her medication on time and they would often fail to check her blood sugar (for insulin) and blood pressure. The food would be good some days and bad others...Didn't see many fresh greens or fruits in her meals. We tried to give them a week, but they failed several times, even after speaking with several nurses and associates. We decided to take her out. During the time my mother was in Eagle Creek Kindred Transitional Care we visited other sites (including another kindred location). All were better. The best we have seen was the Kindred care in carmel but quite the drive from the two locations. I would suggest avoiding this place unless you need the cheapest option and are perfectly healthy.

Kindred looked nice when you pulled off to it but the person I know that was there has complaints. The room wasn’t bad; it was acceptable. The dining room was OK, there was enough room, and they could go in and out pretty well.

We just visited Kindred. It seemed to be a nice facility. It looked clean. My mom's friend seemed to like it and they looked like to be taking good care of her. The staff's work ethic seemed to be good there. They seemed to be taking care of the patients. They were pretty good. The staff looked pretty efficient for their work ethic. They always seemed to have activities going on in the cafeteria where the residents could participate in if they want it. Also they were running a safety drill if there was any emergency just to practice. The room was comfortable enough for the patient. It seemed to be a nice room. The dining room seemed to be clean and nice. I would recommend Kindred. The best thing I like I think was the cleanliness of it. I like that.

The staff was horrible, when they bothered to show up for work. They were not attentive to the patients needs and often stole from them. The facility was clean but had a horrible stench. People were always screaming. We moved dad out as fast as possible.
