My aunt moved to Countryside Meadows. They are very caring and they take care of her. They have a recreation area and they have therapy. She's sharing a room with one other person. The staff was very helpful. They took her back and forth to doctor's appointments.

My husband has moved into Countryside Meadows. It was one that would take him. Others are even closer, but they denied him because of agitation. Countryside Meadows is close to home, but no place is perfect. I got hospice, which is helping a lot. They take care of him. I love the nurses, they're very good, but the management is not so good. They changed my husband's room to prevent falls, and it made it worse, so I'm just unhappy, but I can't go anywhere else. The nursing care is good, and I have hospice, which helps out a lot. I have tried their food. The food is not very good. As for cleanliness, the cleaning crew is fabulous. I love all the cleaning ladies. They're in here every day cleaning. They're excellent. They help my husband with everything. They help with showers and bathrooms, they assist with meals, and they get him in and out of his chair. He can walk but he has a bad back. The actual nursing caregivers are good. The nurses are excellent, but the director is zero. It was expensive, too. It's $12800 a month, and I don't get $12800 worth of service. They could improve by having better directors. They control everything and make it miserable. As for the facilities, you can't do much with the memory care unit, they're the same everywhere. They all laid out the same and looked the same, there's not much you can do to improve it.

My brother has been in Countryside Meadows for three weeks now. The facility is decent, and the staff is very friendly and caring. His room is spacious. It has a refrigerator. It is a nursing home/rehabilitation center. When I went, I just stayed with him in his room. I know they have a dining area and a physical therapy room. They have lounge areas. There is one just as you walk in the door through the double doors. They have another one just before you turn off and go down to the rooms.

Most of the staff, nurses, and aides, are very rude and impatient, they also lack compassion. The recording you have to listen to when you call and you're put on hold is laughable. Anytime I had to ask for help, you could practically hear their eyes roll to the back of their heads, and when they said they would take care of it, their response was always very slow and no one would take responsibility for their lack of action. The food was gross and you never were given what they said they were serving, and it was always cold. When the kitchen closed you were lucky if you got something at all. Most of the nurses couldn't speak clear English, so it made it difficult to communicate. No activities, except basic cable, rooms were not the cleanest, and response time for an aide to arrive was not very timely. There were other issues, and I'm going to be reporting them. Also, when you leave they seem relieved that you're gone. I DO NOT RECOMMEND this facility for anyone you love and care about, they won't receive the kind of care they advertise. ππ»ππ»ππ»

I cannot recommend this facility. My wife left in worse condition than when she was admitted. My wife could not eat most of the food as it was cold when it reached her room. I found the management staff very defensive when I called with complaints- it was always, someone or something else caused the problem.

The rooms are awful my feet stuck to the floor most of the night staff couldn't speak English I went without my medication half the time my blood sugar went from 125 up to 365 they have no activities all you could do was lay in your bed and stare at the ceiling apparently I pissed someone off because I wanted my bathroom clean it smell like urine so the next night when we were supposed to be having pizza and salad I got a refrigerator cold fish and some grass that looked like they picked in their backyard my whole stay I had to worry about who was spitting in my food I would like to give anybody considering countryside Meadows should find somewhere else to stay

My brother was in a nursing home in Florida and we transferred him to Countryside Meadows in Avon. He was placed in the memory care unit and the experience has been like night and day. The folks at Countryside Meadows have been phenomenal. My brother was in the nursing home in Florida for many months and we received no feedback on his care. After my brother was at Countryside Meadows for one day, we received four different phone calls from RNs, CNAs, Therapist & a Nurse Practitioner. Very clean and organized facility. I would recommend this to anyone seeking care.

I was in a rehab at Countryside Meadows, and they have excellent rehab. I was in there after a fall with a stretcher, and I was able to walk after two and a half months. I would recommend the therapy and rehab department to anybody. It's excellent. They were strict as far their therapy, and they were all dedicated. There was no sitting around and looking at each other. The nurses were just outstanding; they know just what to do. They answer the call light, and they just donβt let you lie there. Most of the time, the food was very good and too much. I enjoyed my stay.

I also went to visit Countryside Meadows. The facility is fairly new and it was very clean. It felt very homey too. The area they had for their dementia patients was nice. They had their own eating area and their own nurses. The staff was professional and friendly. I also had a friend whose father lives here and they are very pleased with the facility.
