I am a friend or relative of a current/past resident

My mom moved to Grace Assisted Living at State Street. It's clean. The halls are bright and spacious for all those people in walkers and assisted walking devices to get by. The people are very friendly both the residents and the staff. The staff have been very attentive to my mom. They've worked very hard at accommodating her for her dietary issues, and it worked well within her financial limitations. There are many different activities during the day, throughout the week, card games, and bingos, and they have a Bible study that you can or cannot attend. It's up to any person. There's exercise, there are movie nights or movie days. They have programs. People come in to perform piano, singing, and light entertainment. They've had therapy animals. There's a small hair salon in the facility. There's a gym. There's physical therapy for those who need it. They have outside gardens that individuals can work in if they choose. The dining area is quite large and roomy. So far the food has been good. She enjoyed it.

The rooms are a great size and the staff are so helpful and professional. The staff all treat you like family. The food is great and my grandma is so very happy living here!

I am/was a resident of this facility

The rooms are well maintained. Housekeepers are well trained, helpful and friendly.. The facility staff are courteous, helpful and they really care about all of the residents (it’s not “fake” it’s genuine). They treat residents with respect. The food: well, I am admittedly a “picky eater” but I won’t be critical. Considering the constraints (diets, costs, etc.) they do an admirable job and coming from me that’s a high compliment. Activities: too numerous to mention.(that’s a compliment ) Residents seem to really enjoy them

Rooms are nice. Office staff seemed untrained for handling situations and family members. Care staff had some real stand-outs but there was a "click" run by a caregiver named Debbie who ran whole show. If she didnt like you she found a way to get you out. My parent's neighbor was targeted by her and they ended up getting kicked out based on her lies. She tried the same thing with my parent and we tried to alert administrer to our concerns but they didnt believe us. Administraters are too young and untrained to work with family members and handle issues. We had family there for less than 3 years and they had 5 different RNs and 4 different administraters. The worse thing was that we were being charged for add-on services that they then did not provide. They kept adding service charges and raising the rent. The only bright spot was the activity director - she was amazing. We moved our family member out and we do not recommend them to anyone, no matter what the price is or how nice it looks.

When my Mother moved to Grace, she was so excited by the friendly welcome she received from the Administrator and the other staff. She had a great room and was looking forward to the activities and food. When the Administrator moved, the quality of interaction with staff and food deteriorated quickly. The residents were told the facility would no longer provide Paper towels or tissues, and they then limited the toilet paper. The food has become so poor it seems that most often it is reheated or overcooked to the point it cannot be chewed. The portions are very small even for elderly residents. The activities consist of bingo, and an occasional bus ride to the grocery store or appointment. The residents used to be happy and would visit with each other in the halls or linger after the meals. Doors are locked at 6:00 pm and you have to ring for a door to be opened. When the door is finally unlocked there is no welcoming back or any other conversation. My mother is moving as is several other residents, due to the lack of care, friendliness, and the current administration. When the previous Administrator was at Grace, there was always a friendly happy atmosphere as she was visiting with the residents. The current administrator seldom comes out of the office to even say hello. There is seldom any interaction with staff. My Mothers studio room started at $2300 a month in 2019, on June 1st, 2022 it will be raised to $3100.00. I am so disappointed with this facility and would not recommend it to anyone.

I was not impressed with Grace Assisted Living at State Street. I was not impressed with the staff at all. I wasn't impressed with what they offered. It didn't seem to offer as much compared to some of the others as far as activities and things. The room was nice and big. It was a big facility, probably bigger than I would have wanted. It had big long halls. There was a library and there was a piano there. I didn't meet any residents there. They had a calendar of activities. They had religious groups that came in and held different services on Sundays. The person that took us around was not very impressive at all. She didn't try to sell her place. The facility seemed nice; it was just a little older.

I am a friend or relative of a current/past resident

My 90 year old grandmother moved into this facility following a hip replacement. Several months after moving in my family was informed by the Nurse that my grandmother needed memory care because "she couldn't find her room" and stated that she had a week to move out. This was precipitated by little to no communication about the fact that my grandmother was having trouble with this and there was no collaboration or problem solving sought with my family in regards to this problem. My mother who is an occupational therapist took a week off of work to find a new living situation. Following this she was evaluated by the new facility who did not feel she required memory care and felt they could in fact do the job of helping her find her room if need be. I am an ICU nurse in the area and I found myself completely appalled at the lack of communication and care collaboration that this facility engaged in. In addition to this it is obvious that many older adults need time to adjust to new living environments especially after surgical procedures- each move precipitates a possible "mental hit" that with proper care can resolve. However, Grace took no such approach to the care of my grandmother. She did suffer another decline in her memory following moving which likely would not have happened if she had not had to be moved when she did not need to be. Now months later she is doing so much better after having time to adjust to her new facility. She has started sewing again, doing activities, and although she does need help with some things- she doesn't require memory care. Ultimately I am thankful because we are getting better care and collaboration with the new facility. I would caution against anyone considering this facility. I would not want my loved one in their care.

As soon as COVID gets under control, my wife and I are moving into Grace Assisted Living at State Street. It is an excellent community. It has a Christian-like atmosphere and at orientation they offer food to the residents. Their apartments have a door to the outside. They also play bingo and there's a bus that goes to various doctor's appointments and grocery stores. The staff is professional. The facility is very clean, relatively new, and well done.

The good: The day to day staff are friendly and keep the place very clean. It is a visually appealing place with very nice facilities. The bad: The management team is pleasant but not very responsive to tenant needs and requests. When the management needs to make changes in routines or ask tenants to refrain from doing something, they are rude and do little to explain why the rules are needed. The changes are usually reasonable, but the tenants don't always understand that. Dinner is nominally served from "5-6pm". It is not. You must be there by ~5:10 or you get nothing. If you ask for seconds, after ~5:15, you can't get any because they have already put the food away. The residents asked to have dinner at 5:30 or 6. The management refused. The residents don't much care for the food. If you don't like what is being served, alternatives are pretty much just sandwiches. During the time of covid, the staff is mandated to wear masks. They routinely wear them around their necks, not covering their faces. The management was notified of the practice, but nothing has changed even though the facility has had cases of covid.

My wife moved into Grace Assisted Living at State Street. They took an interest in her, they went out of their way to visit her in the hospital, they called me up and said they were ready to take her. None of the other ones really got back to me. The staff is very responsive, they try to help her, they're always friendly towards me, and let me know if she needs anything, and if they need me to talk to her about something. They just built it, so it's brand new, it's very nice, and they have their own bathroom and shower. It was cheaper than the others.

I visited this facility

Grace Assisted Living at State Street was a very nice building. The food looked good, but they have long halls and it would not have worked for my parents. The rooms were beautiful, had nice little kitchenettes, they were roomy, and light. The dining area was also light, pleasant, and roomy, and they had lots of choices on their menu. The layout was really stretched out because of the long, old halls, but it's very lovely and nicely decorated. They have a nice reception area, and they were friendly when we came in.

I visited a friend at Grace Assisted Living at State Street. It's beautiful and the room they were in was very nice. It's very well done and everybody seems to be very happy. It's a brand-new facility.
