I am a friend or relative of a current/past resident

THE WORST PLACE!! If you place your family members here, shame on you. The whole place smells like ammonia, the floors are constantly sticky, the nurses don't give meds on time or ever, I was able to walk in and walk out with my Uncle and no one stopped me - no one asked who I was - I was shocked. My uncle had extensive neck surgery and he fell in the middle of the night, after pushing the nurse button for 10 minutes with no one coming, he called the main office and THEY called the nurses office and STILL no one came to help him. He had to call a SECOND time and finally somebody came to help him. he was on the floor for 20 minutes. He could have died in his fall and Avante and it's staff wouldn't have cared. This place should be shut down. It’s a disaster.

I am/was a resident of this facility

The care of patients, as far as I am concerned, was terrible on the most part. Upon arrival they put me in a diaper and a wheel chair. Wouldn't allow me to get up and walk even with my husband. The first night I was told by the care nurse that she didn't have time to clean me up as she was working alone. When she did come in the early am (approximately 6 am) she changed my bedding and diaper but didn't have warm water to clean me up. The food wasn't very good. For example, one breakfast was a dry biscuit....no grave, no jelly, nothing at all. Was not advised of any activities for the 6 days I was in this facility.

I visited this facility

I know what a good facility is supposed to be. My Mom was a nursing home administrator for multiple facilities. My Dad recently had a bone amputated from his foot. He visited this rehab facility and in the middle of the night he asked me to come get him. He’s a diabetic. They didn’t bother to get his Med list. He received no meds. Last straw was asking for a walker and shower chair after 30 hours of no bathing he received none. Food was pathetic. Shameful.

I visited this facility

We like Avante at Ocala because of its cleanliness, the attentiveness of the people who work there, and how they greet everybody. The staff was excellent. They knew my mom was coming for a visit, they greeted her, and everyone down the hall knew she was coming. They seemed very personable. It was as clean as a hospital, both inside and outside. It was an inviting place, it didn't look like it was an institution. The rooms were very nice.

My wife was transfered to this facility, from the start they kept her life meds from her. For almost 4 days they didn't give her meds or check her sugar or blood pressure, they kept saying her meds were on the way. She got sick and they let her stay in her own vomit all night without helping or changing her. This place is a complete joke and does not care about their residents only the money! Please don't let your loved one go here

This faculty does not care for your loved one. My mom was admitted for rehab after shoulder replacement. Because of COVID no one could visit. She was there 3 weeks and contracted COVID. 18 days later she passed away. They let her blood oxygen level drop to 70 before calling an ambulance. When I went to retrieve her things they gave me someone else’s pajamas, some of her things; mostly toiletries. She had 2 rings worth over $7000 and they have disappeared. I tried getting them from her before she went there but she wouldn’t part with them. They didn’t return her clothing or her cell phone. The director of nursing is very rude and unhelpful. I will NEVER put a loved one in this facility. Would give zero stars if I could.

Do not send your loved one here if you can avoid it. Every social worker, case manager, and management that I've attempted to contact does not care and will not go out of their way for you or your loved ones. They will tell you that you all of a sudden need to start paying for your stay and then dodge every attempt to contact them with questions. They want your money. I'm not sure if this lack of communication is form disinterest or malicious intent, but either way, it is neglectful and downright unethical. Please look very hard to find other locations if you do not have the money to pay out of pocket because they do not know what they are doing over there. If you do have the money to pay out of pocket, the nursing staff is friendly and helpful. They seem to really care and they made my loved one feel comfortable and safe while I dealt with the money. It's a shame that the other aspects of this place are so horrendous.

The only reason this facility gets a 2 is because it looks nice and some staff are very friendly. Otherwise it would be a zero. Mom was admitted late day and came in for rehab after major surgery. She has such severe reflux the was throwing up. If took forever for them to bring her her prescribed medicine and she was in misery all night. It seems that someone was off duty and they had to do all sorts of things to get this medicine. She was hot, it was noisy, bright and just all around miserable. When I told staff next day that she was going home, they advised us in very clear terms that Medicare would not pay for therapy if she left.. NOT TRUE...... We are set up for home health and Medicare covers 100%.. this was and is so infuriating and scary, until we got some answers to the contrary. Oh, and the food was horrible! Sorry Avante, but you need some drastic improvement.

I visited this facility

Avante at Ocala seemed more like a warehouse facility. They offer somewhat minimal care ad do the necessities, but it seemed more like a warehouse than anything else. It's more like a rest home, so if you have medical issues, the wait time for health needs is way too long.

My wife's godfather is a patient here. The staff here are wonderful and most friendly , especially Jimmy D., he seems to really enjoy working with the patients and they seem to really enjoy him.

I am a friend or relative of a current/past resident

A relative of mine was admitted there 2 weeks ago, diagnosed with MRSA (don't know if he got that in the rehab or the hospital), was supposed to have PT, OT and Speech Therapy 5 x a week, didn't happen until I a few days ago. He had a catheter inserted and resulted in an infection, had to be rushed to the ER because it was bleeding so much afterwards. Now back in the hospital having to suffer pain in getting the foley reinserted to drain his bladder that apparently isn't working correctly. Put phone calls into Avante and spoke to the Director of Nursing and one of the PT's who claim he was getting rehab every day --- NOT TRUE. But I'm sure Medicaid will be billed for it. Not recommended.

I am a friend or relative of a resident

When my mother got out of the extensive care at the hospital, they suggested I put her in for a few weeks to get her back stronger in Avante. The staff was very kind, it was very clean, and the food was a lot better. The therapists were good, and the nurses were very helpful. CNAs were very helpful. She still was impacted though, and they didn’t pick up on it. The same doctor works there, but I insisted that she have a different doctor, so they gave her a different doctor. One CNA picked up on the fact that she had impacted bowels, and she said I think your mother needs to go to the hospital. Our insurance company had not approved a wheelchair, so they sent us home on Thanksgiving Day with no walker and no wheelchair, and I managed to get her into the house. We had Thanksgiving dinner, and she fell again. It was probably as good as you are going to get.

My mother-in-law is a patient at Avante of Ocala. On more than one occasion, she has called her daughter (my wife) to please come and help her to the bathroom as she has been waiting for 45 minutes for assistance from the staff. Just recently she was told she is being taken for her Doctor's appointment. Sge told the staff member, she does not have a Doctor's appointment and she repeatedly told the staff member she was not "Jane Doe" but "Jane Smith". They ignored her until she got to the Doctor's office and they told the transport driver this lady is not our patient. My mother-in-law has a very sound mind and is in for physical conditions, not mental ones. I know this facility, like many hospital and long term care facilities, is understaffed, but this seem more like a caring issue and not a staffing problem. Could not recommend this facility to anyone looking for long term care or rehab.

My aunt had been at Avante at Ocala, and we chose it because it was very affordable, and the staff was excellent. They have exercises, bingo, games, card games, and shopping day where people come in and bring stuff so they can buy it.

I am a friend or relative of resident

My father was a resident for a couple weeks at this facility.My sister and I checked him in and unpaced his things,signed all paperwork.When I went to see him a few days later I walked around for 17 minutes and no one asked if they could help.I finally found him in the hair salon.His glasses were not on nor his dentures in.Since signing him in his glasses had been broken and dentures lost-in a few days time.Nothing was done to get him glasses or dentures replaced.On another visit I came at lunch time a few days later.Waited for lunch for 20 minutes while employees stood around and complained about their work.As I sat to the side I noticed they put my dad at a table alone in a wheelchair even though there were tables with people.They faced him twards the window-alone.They brought him a regular lunch-ham or something-All I remember is saying"how can a man with no teeth chew this food?"I was very displeased so I went to an administrator-a man.Who sent me to a woman in the next office.She stated"we did not know your father had any relatives"..I checked him in,signed his papers."Can you sign your dads paperwork?"I already did that."your dad is in a wheel chair because he walked out the front door and almost was hit by a car,we are not a lockdown facility"..so..my dad walked past the receptionist 50 feet to the parking lot and was almost hit by a car until anyone noticed he was out the door??!!!"..She the said we should look into another facility,wow ..ya think!We took my dad out a few days later,minus teeth and glasses.Pretty buildings do not mean decent care.
