Very weak physical therapy unit. JUST USELESS! Better to have physical therapy done at home. Also, mother would be left soiled for hours because they did not have enough help to change her. Her spirit has been completely broken Carroll Manor. Please , please watch out for their billing practices . There’s so much more that went wrong. I absolutely do not recommend them.

My advice to caregivers and Families do not send your loved one to Carroll Manor my mom passed away in that facility from complications stage four bed sore and which it was so infected and got into her bloodstream she had a stroke there also and the staff there didn't notice until the family came up and noticed so I advise families not to send your love one there or if you have a low one there move them as soon as you can and also her room had roaches in her drawer as well

Ascension Living Carroll Manor was in a nice location. The outside grounds are very well kept, and the lobby area was very nice. When I went there, I had an issue because I had an appointment schedule like at 8 AM and no one was there to do the tour. After some back and forth with them, one of the head nurses gave me the tour. She was pleasant and nice. She was the only one I interacted with overall. Other than that, I interacted with the security at the front desk, and it was a little bit off as far as organization is concerned. Those were my only issues. The ground and everything were fine, but once I got to the section where the individual would stay, I would give that a two. It doesn't seem right to lump that outside in when the inside does not match the exterior if that makes sense. They need to upgrade a lot of their memory care clinic. A lot of the facility is outdated and that was my biggest problem with them. They did have a chapel that was towards the front of the entryway, but for someone with my mom's issues, unless she was with the family, she wouldn't be allowed to visit it. I didn't see a beauty parlor; I didn't see any of that. There's a rehab center in there, and then for short-term patients, there's something there. When I went to the memory care portion, there was an area where they prepared food, but it wasn't like a dining hall or anything. It was just a room with tables set up. My goal was to see the memory care clinic or whatever the proper term is.

This place is terribly staffed it's an old facility and the old good thing I witnessed was the pool for physical therapy. Overall this place should be shut down. It is understaffed and the social worker was indignant. Patients were sitting around in the tv room looking very unhappy and wasting away.

I have been back and forth checking on a relative. I noticed the room isnt very clean. There is a lot of dust behind the bed and small dresser. All along the sides of the walls their is dust too. How often due the cleaners dust the rooms? The floor in this room has been dirty the day the resident checked. There is also a suspected mattress on the other side of the closet. It has a sheet on it which has accumulated dust too. We visited the day room where the piano is located, and right above it their is a vent which should have been cleaned well over 2 months ago. The filter should be changed every 3 months from a building maintenance point if view. I was at this facilty on 2-16, 2-17, 2-18 and I'v seen the same crumbs in the floors. Ive heard employees argue in front residents. Radio plays to loud also. Every resident doesnt want to hear it. 2/16 I road with an employee on the elevator and listened to vulgar words complaints and threats! Im totally disgusted in some of the performance here at Caroll Manor. I pray for a speedy speedy recovery for my relative because this place needs a physical enema for the STAFF! Move you relative If you Can!

We also visited Carroll Manor. It belonged to Providence Hospital. It was a great community, but it was more of a nursing home than my mother needed. The rooms were great. They were semi-private. They were two to a room. The staff was very friendly.

In my opinion this nursing home is average. I did not notice anything that was dirty or did not see any staff members being neglectful of the patients. I however did not see any activities being conducted, did not notice any of the nurses laughing with patients, and did not feel a warm sense of home. I did have a bit of lunch to eat here, and will say that the food was very tasty (surprising for a nursing home) and I did enjoy the atmosphere while dining.

Located on the grounds of Providence Hospital, which has an excellent reputation in the community, this facility offers short-term and long-term medical, nursing and rehabilitation services. On a pleasant wooded campus, it has more of a suburban, peaceful vibe"”even though it's in the heart of the city. It's medium-sized facility, about 200 beds or so, and very well managed. It does have a hospital feel, but not in a bad way. The building is an older one, but well maintained and clean. There are also some homey touches inside, such as artwork hung in the hallways"”and the entire place is nicely decorated. The rooms are average-sized, and some are private, with bathrooms in each suite. The staff members are very skilled and welcoming"”and there seem to be an adequate number of them. They are knowledgeable about the patients and their needs"”and communicate well with outside caregivers. Carroll Manor is a popular place"”a lot of people like the easy transition to a caring facility from the hospital. So one possible drawback may be that no beds are available when needed. Medicaid is accepted.
