The rooms were small but nice.The main problem was the late night shift answering the phone.We had a camera in moms room that was watched 24/7.Just like any other nursing home you have to stay on top of things.Really the only thing I had a problem with was keeping mom changed.All nursing homes are short staffed and it makes it hard on the ones that are there.The Director of Nursing was extremely nice along with Latisha the Admission lady.She jumped through hoops to help us.To me everyone I came in contact with were so sweet.Just stay in contact with your loved one and never go the same time when you visit.Tommie Lemons

Founders plaza is the very best rehab everyone is awsome here. If i could giveA 100 STARS I would everyone is so very caring and they care about the people who stay here. It.is a wonderful place for anybody whos needs it. Rooms are always clean

My mother is in Founders Plaza Nursing and Rehabilitation for rehab. They're doing a great job with her. I have no complaints at all. They have been very considerate of the special needs that she has. She could still have some social interactions with severe dementia. I told them that at the assisted living where she had been, her joy was to sit out in the lobby and wave at people or walk around on her walker and wave at people. That's no longer possible. Now they take her out to the nurse's station and let her wave at people and be out there and still socializing. She's been so happy every time I've gone there. She's sitting at the nurse's station getting to at least see people and do things with them. They would do things like feeling different textures, putting her hands on a swimming noodle or little simple toys that has different types of closures on it (the chain lock or one that you turn). That's there for her to play with, and there are some balls on the little table. There are about three or four that they will let come and sit there that are becoming her buddies, even though she can't talk to them. They're very conscious of her social needs and her physical needs. They contact me every week for an update on her physical and dietary needs. They update me on all of her needs that are being provided. Their staff is very caring and considerate. I've been there when they brought her food in. I tasted it. It's very, very good. I've had no complaints about the food at all. I don't really remember any problems. They told me that even though she can't participate in any of the activities, they take her in any way for social interaction. They also play bingo.

This is a terrible place - they do not care about patients. Response time for help is terrible, they don't call for medical help when needed and treat the old people like dirt! DO NOT USE THIS PLACE!!!!!!! I tried to talk to the person in charge and was told that I cannot talk to her and also I asked for her name which they said they were not allowed to proved!

BEWARE! My mom was placed there early 11/2020, worst care ever and she died there on 11/28/2021. During a parade my mom was “out” of it on the sidewalk in a wheelchair but we were told she was sleeping, after an hour of her “sleeping” they finally realized she was unconscious. Come to find out a similar incident happened a few days earlier but they failed to tell us. The early morning my mom passed away, I believe she had been dead for awhile as when I arrived she was ice cold, no way her body could react like that in such a short period of time. I asked to view the video of that early morning as I wanted to see the interaction with the staff and my mom, they would not let me see the video or tell me what went on that led up to her death. Don’t stay there, move on, I’m just telling you I would not put my worst enemy there.

My mother has been in Founders Plaza Nursing for two weeks now. It is well kept up on the outside. Everything is clean on the inside and the hallways are clean. They have a Monday-through-Friday staff, and then they have a Saturday-and-Sunday weekend staff. Monday through Friday, they are pretty good. I have to call and talk to the director about some of the weekend staff, though, because they're the ones who are lacking. They don't answer the phones, and they don't have good communication there. I would rate them a "three" because I can't get my head around the weekend nursing and staff. When I call on the weekends, no one would answer the phone. When I go in my mother's room, the nurse's light is on, and she said it has been on and that she needs to be cleaned (this is on the weekend shift), and no one had come. She said it was three o'clock when she turned the light on, and I'm there at 5:30. When I go to the nurse's station, I get ignored sometimes. They offer activities, but my mom can't participate right now; she has a bedsore and they have to keep her off of it, so she can't sit for a long period of time. But they do offer activities, because I saw the activity board. I think they keep them busy. They have activities on the board pretty much every day, and they have Sunday services. Someone will come in and do Sunday services, and I like that, but my mother can't get up right now. It is reasonable. I think the communication as far as staff is lacking. The communication between the staff could be better. I think that someone should answer the phone at all times because we do have loved ones there. The communication is very poor there. The aides need to be better trained on what to do for the residents and to spend a little more time with them. One time my mother's face wasn't washed and they didn't take her dentures out, and my mother was upset about that.

My mother was an alert, friendly, physically healthy senior when she went there for physical therapy. While she was there, "someone" brought covid into the facility and infected almost 40 residents. I was unable to see my mother for more than 3 weeks, being told all along that her vitals were good and that she was without symptoms and could be moved back to a regular ward. However, when I finally got to see her, she had lost almost 20 pounds, was incoherent, and didn't recognize me. I unequivocally to not recommend this facility!

Karla, director, was nice upon first meetings. After Mom moved in, she became unfriendly and rude. Nancy, one of the nurses, is power hungry. When Mom passed, Nancy took it upon herself to remove personal items from the room. Items were returned (from her office) once she was faced with legal rights. Food is horrible. I thought Mom was just being picky because the food in the cafeteria looked decent but the food they delivered to her room was horrible. Billing department; there was a change just before Mom's passing but they never gave me an actual bill the entire time she was there. I would get a sticky note with a dollar amount for me to pay. I did request bills and told them I would not pay without one. Mom went into the facility in decent health. She had been living by herself. After moving her, the doctor on change changed her medications. The doctor would never met with me. The hospital would adjust her medications back but the nursing home would not follow hospital orders staying they don't have the staff. I have seen controlled medications left in the open. She passed within 5 months of moving. She spent about 1/3 of that time in the hospital due to poor attention to medication. I never saw the activities room Karla likes to show off in use.

My Mom says the food is terrible. She will not eat, she is left in a dirty diaper all the time, she has fallen out of her wheel chair several times. She will not bathe there because it is too cold. The lady who does her hair is way too expensive and not very good. I am guessing all these places bite if your not private pay, sad.

My mom had been at nursing home in East Texas and was not doing well. That particular home was not equipped to handle her dementia. After coming to Founders mom gained weight and really liked the place. Everyone is very nice there. Some of the nursing staff became mom's heroes. They took very good care of her. If I had a concern, they were on it. Very pleased. Though mom has passed, I still go there to visit. Very nice place.

This facility lacks a caring administrative staff. My mother was here for several years. On many occasions, the administrator was extremely rude to my family. I do not know how she has kept her job this long. The nursing staff & therapist were very good to my mother. However, the overall "feel" of the facility is very cold & uncaring. I wish she had lived out her final days somewhere else.

I visited this facility

The one that we looked at was Founders Plaza. First of all, when I walked in, I liked the place. I really liked the way it was very welcoming and clean. I could tell that I would probably use this place because (of) the way that I felt when I walked in and welcomed by the staff. The rooms were in good size for me. I just felt good about the place. They took me around looking at the rooms, and they took me to a little cafeteria place where they go and eat. They took us to the exercise room. I was ready to just go there, but it did not happen. It was like a very good place. It gave me a very warm feeling.

My mother-in-law is at Founders Plaza. It is beautiful. It is very nice and clean. Everybody is very helpful. The nursing staff is very nice. I've gone a couple of times and had lunch with my mother-in-law outside on the pavilion, which is very nice, and it is a very nice dining room. My mother-in-law is in a shared room. It is very nice, and she likes her roommate. It is not too big, but it is not small where everything is cluttered. My mother-in-law has bingo and arts & crafts. They have an exercise room. Founders Plaza is nice and open. When you walk in, you got a friendly atmosphere. I like it.

We choose Founders Plaza because of its convenience and the close proximity to family. It is close to my cousin's house, my brother lives down the street and I live down the road, so it is just easy for us to go visit and take care of my mother. The staff truly and genuinely cares for her and that means a lot to me. I am not crazy about the food but her room is nice and adequate. The patients there seem content and they are always busy. Every time I go there, there's something going on. They have bingo, church, social days, crafts and though she is not able to participate in most of it because she is on rehab, I encourage her to try. Mom seems happy as anyone can be. Her whole life is changed.
