Spent 4 to 5 hours a day for 5 days. Very helpful happy personal staff. Mom and sister and I especially point out Florence Haggai Kent Tracy and Connie. All use first name greet and show legitimate personal touch.

very sad that they are so understaffed. My 83 year old mother broke her leg/hip and went there for rehab. Constantly left in her room with no response when used the buzzer for assistance to use the restroom. When someone did answer the reply was always "you are in a diaper, just use it", completely stripping her of any dignity. When buzzing to her the diaper cleaned, she was left to sit for an hour or more. Very few times taken to dinner hall. Food was always cold and a hair was found in it. Therapy people were very good, but that did not compensate for the lack of compassion or assistance with the way she was treated in this facility.

The facility is poorly managed. The rooms were never cleaned properly, we had to clean it ourselves. They did not inform us of the full extent of resident rights or that we had a choice to do his laundry. They kept losing clothes which laundry on site is included in fees apparently. We did end up getting the iron on labels. Label everything! Full name and room number! The food was horrible which would make anyone lose weight and they kept getting his meal orders wrong. Including foods that he disliked repeatedly. Despite it saying that his preferences on the ticket. Thankfully we could get him some snacks. There was no meeting to talk to our family within the week he came in. We were left in the dark and had no guidance. We know he has a right to refuse to get up out of bed, but when the nice therapy people come in all it took was some encouragement. The activities seemed lacking. It gets really boring being cooped up and watching TV all day. Wouldn’t you like to at least be outside sitting on a porch on a nice day? They never did anything outside despite there being a cute golf course. We had one of the therapists yell at him for not being ready to go. We didn’t know who his therapy director, the director of nursing, administer, or the social worker was until we started to speak on these matters that should be small! We are looking to see what our next steps will be…which are definitely not going to be to stay at this place.

The first time was a good experience following surgery of her leg. The front part of the facility is all insurance based, so they are private rooms and much better staff. Once you get to the back, it is unbelievably horrible. Pt is left in their own excrement/urine, treated badly by staff, the clannish staff will talk about the pt, or family, in front of you. If family will take them to the bathroom, the staff acts like they can't. Sit in on a meal! Staff eats the food at the food line, while pts sit waiting for food for a long time. The server behind the line should not be stuffing her face with her hands? They use the excuse that the pt doesn't remember things to excuse their negligence. And, you will never be able to talk to administration. If we could remove my family from there we would. A watchful eye is the best we can do right now. And, staff will retaliate when no one is watching?

When we placed my grandma in the facility in October of 2019 they were extremely nice and helpful. Fortunately I was able to see her 2 to 3 times a day 7 days a week no matter what up until March 10, 2020 when COVID shut everything down. From that moment on we feel the attitudes of all staff changed. Before when I had a concern i could talk directly to someone and fix immediately. After March 10, 2020 they made us feel like we were a burden on them when we would call to ask questions. There were times the only answer I would get is she’s fine, not a answer to her moving, pains etc. From March 10th on my grandmothers health deteriorated quickly and it was due to her age and health conditions but for a place like Prestonwood to act like family is a burden when they want answers is extremely frustrating. Towards the end of her life we decided to place her on hospice care but this was also a rough time since doctors would tell us one thing and staff would tell us another. We were told several times that they did not agree with our decision even though we didn’t make decisions without consulting with a physician. At one point we talked about placing a camera in her room and quite honestly the one person I had to talk to was rude and pretty much told me good luck on doing that. She informed me we would not use their WiFi and that the family of her roommate would not agree to us doing it when in fact I know they would have agreed. Disheartening to see a place that is supposed to be a top ranked facility act the way they did. And if you need to talk to a higher up in admin good luck. Always an excuse why you can’t.

Before my mother checked in the facility they asked for full month's payment and promised to get Medicaid reimpursement in a month. But they obviously did not intend to go through Medicaid and still send bill to me. After 7 months of follow up phone calls, emails and visit they still refuse to contact me about refunding my payment. Even though they advertise that they receive Medicaid patient in reality they don't. Once you check in you are liable for payment.

The facility looks nice. BUT my husband was sent to Assisted Living with a UTI and was back in the hospital within two days. Many times he was in the room soaked. When I asked the nurse what was going on I was told several times that employees had not show up. Therefore no one could assist him to the restroom. There was one person to two halls. I learned to bring my own aspirin for him as that would take over an hour. The help was very nice, just not enough of them.

We chose Prestonwood Court because when she got out of the hospital, she went to the rehab there for more than a month, and we just thought that the familiarity of being there would be better for her. We're really pleased with the activities, the therapy, and the different things that she's doing in the rehabilitation part.

I have visited Prestonwood Rehab on multiple occasions to visit a couple of friends who have chosen this facility to get their rehab. Each time I visited I was greeted warmly by the staff. My friends receiving rehab have been very please with their decision. This facility is clean and the staff seem to really focus on building the patients strength so they can return home. Should I ever need rehab services, Prestonwood Rehab is where I plan to go.

My mom was at Prestonwood this summer after hip surgery. This is a wonderful caring facility! All of the staff there were so kind to mom and I, they took great care of her and treated her with respect. The nurses and aides were very attentive to her needs and the therapists encouraged and supported her throughout her recovery. She is now home and back on her feet (albeit a little slower and more careful now) Great job Prestonwood for taking such good care of my mom!!!

I had my mother in law at the Prestonwood facility. From the moment she was admitted we were very impressed. The care was exceptional! The staff was so kind, the food was excellent my mother in law loved the menu with so many choices. The facility was clean, and the therapy team you couldn't ask for better. Thank you Prestonwood, we have referred many people too you..... and they have had the same experience!

Prestonwood Rehabilitation is in a very nice neighborhood and the rooms are beautiful. Everything is new. The facility looked pleasant. The people were happy, neat, and very upscale. The programs they have are excellent. They’re right in there doing speech therapy and physical therapy. My mom said the rehab center is fabulous. It looked like the patients were doing really well. People leave before their time is up because they push them so hard. The nursing staff in the rehab part is excellent. Over in the skilled nursing, not so much. I think there’s a huge discrepancy between the skilled nursing and the rehab center. My mom complains about the food all the time, but I’m beginning to think it’s just something that elderly people do. They complain about everything. My son was there and he had some of her soup and he said it was fabulous. He’s a gourmet chef himself. She says the people are very nice ,and they treat her well but she’s miserable there. She’s just unhappy no matter where she is with the condition she’s in. They have a calendar up there; there is something for them to do everyday and there are multiple choices.

We put mom there to rehab after surgery. We placed a camera in the room, which by the way I suggest you alway do this for the safety of your loved one. Three days into her stay we are reviewing the video. We watch as someone comes into the room with a clipboard in her hand. Mom is not in the room. This person looks behind her, then opens up the closet where mom's belongings are stored. She is seen removing something the is soon revealed to be mom's wallet. She begins rifling through the wallet, dropping change in her search for cash and cards. We had of course removed any valuables from the purse before putting it into the room. This person takes the purse and puts in back in the closet as she is constantly checking behind her. We then see this same woman come into the room about 20 minutes later and conducts a survey with mom, as if nothing has happened. Turns out this horrible person is a DIRECTOR level person at Prestonwood Rehab. The facility is notified of what took place. They seemed horrified, and told us that they would address the issue and our identity would be confidential. The next day we came to the room and there was a sign on the door reading "THIS ROOM IS UNDER VIDEO SURVEILLANCE" . So much for the confidentiality. Then began a process by Prestonwood to remove Mom from their facility. Within a week, all of a sudden they have done all they can for mom. She was no where near ready to go. We moved her out to a REAL Rehab facility, and she was there for another week, before being able to return home. This facility may be beautiful, but it's staff is not competent, and we know they will steal then get rid of the victim as fast as possible. Stay away from this place for Rehab. Go to a Real Rehab facility such as Baylor etc.

Provider Response

We are so grateful that this customer brought the issue to our attention. It helped us protect our other residents by alerting us to the specific employee's actions. The information allowed us to deal with the employee appropriately and we appreciate it.

We also reported the incident to our State Regulators (DADS). They came to the facility and did their own investigation. They determined that the incident did occur and that the facility was not at fault, but took appropriate measures when the incident was brought to the facility's attention. Our regulators require that a sign be posted outside the resident' s room when we become aware of surveillance. We complied.

Thank you again for helping us ensure the safety and well-being of our residents and their belongings.

Overall, we have been very pleased with Prestonwood Rehab and Nursing Center. When you go in, they do have a plaque that says 2013 Best Nursing Home and that really adds up a plus to the people being there. One of their occupational therapist is amazing. She is very cheerful and sweet. They have a private room, but my grandma has a roommate and that has actually been really good for her. My grandma said she likes the food. They have a lot of food options.

I was a patient at Prestonwood Rehab for the past five weeks and I wanted to thank the staff for taking such good care of me. Dealing with your nurses was a special treat. They were all professional, but also very pleasant and easy to get along with. Thank you to all, very sincerely. It is not easy to be away from home but your staff made it more pleasant. During my mother's 1 week + stay, her nurse Lydia always kept us informed, gave the best care and treated my mom and family with utmost care and respect. The world would be a much better place if there were more people like Lydia. My husband and I, along with family spent from very early morning til as late as 2am there so we witnessed their efforts first hand.

I visited this facility

I visited my mother for two weeks in July, 2012 while she was housed at Prestonwood. Initially i was impressed with the cleanliness of the facility, then with the people I came into contact with. My initial meeting with the administrators and department head for physical therapy, Donna, I learned the objectives for my mother's rehab during her stay. I then became acquainted with the two primary physical and occupational therapists, Celeste and Karen who both instrumental in motivating mother to work diligently toward recovery from her hip socket fractures. I also became acquainted with the attendants who responded to mother's needs during her stay. I will likely leave someone out, but recall Doreen, Sierra, Geoffrey and others were both pleasantly helpful and attentive. The only shortcoming I observed was at lunch time, the residents would be brought to the dining hall shortly before 12 noon. during my visits there, the residents often sat for 30 to 40 minutes before being served. perhaps there is a logical reason for the long delay - special diets for some, whatever. It seems this is one area that could be fine tuned. Mother was discharged on Sept. 5th, 2012, a bit earlier than expected. I credit her therapists and those who attended to her for encouraging her to work hard, gain strength and be more self sufficient. (Couldn't get the rating stars to remain - you all get a 4 star rating).
