I am a friend or relative of a current/past resident

I visited the facility today with two other family members to make an informed caring decision regarding placement of my 91-year-old mother that has a couple of open wounds and is very weak. The hospital doctor recommended a skilled nursing facility where my mother would receive PT, etc. Entering the facility, I noticed a clean lobby and the day receptionist was practicing COVID protocols. The visit appeared promising. Tonight, the night receptionist was practicing COVID protocols as people entered the facility. The floors were not clean as they had been earlier in the day. While I waited in the lobby for my mother to be transported, the receptionist made calls to people reporting that two patients and one employee had tested positive for COVID. She further reported that the employee was sent home and the patients were on the 400 Hallway. A Black male patient was seated in a wheelchair in the lobby with his head down. The receptionist moved his chair without speaking to him dragging his feet. I tried to make myself okay with the news of COVID and seeing the man being moved without a word and asked if I could go back to get my mother's room ready. As I entered the room, I noticed two beds in what was to be a private room. I was informed that the first bed was to be my mother's. The bed was unmade so the nurse ripped the sheets from the second bed and placed them on what was to be my mother's bed. The wound care nurse and an aide joined the first nurse to make the admitting team. The wound care nurse put on gloves to examine my mom, left the room with her gloves on. When she re-entered the room, my mother asked her to change gloves twice before she finally changed and said if it makes you feel better. Mother informed the team that she is a retired RN. I opened the door to the bathroom to see a trash can with trash and a closer look at the commode revealed that it had pubic hairs all over it. I went back to the front and reported the room condition to the receptionist. I also expressed my concern to the team that was examining my mother. Glass or cup water marks were on the tops of tables and inside the closet. The floors were not clean. My sister and I cleaned as much as we could while coming into the facility expecting a move-in-ready room. A male nurse entered the room a little later to explain the reason my mother had not received her night meds. At that point, my sister stated that communications is the key and she wished that someone had told us what the circumstances were. I went to speak with the wound care nurse and assistant director of nursing. Her response to my concern about leaving my mother at the facility after I expressed my concerns was, "And what now." I decided to bring my mother home because I didn't feel comfortable leaving here there not knowing if she would be cared for properly since I had expressed my concerns. My mother is home in her bed and has been given her night meds.

I am very pleased with the level of care this facility provides to my spouse. Staff are helpful and tend to my spouse's daily and special needs. I am particularly impressed with the contractors they have come into the facility to provide specialized medical services i.e., nurse practitioner, podiatrist and psychological services. Communication provided by physical therapy staff, nursing staff, business office, transportation staff is excellent. The rooms are kept orderly and clean. The physical therapy department is well staffed and they work very hard to encourage my family member to improve. I would offer the highest recommendation for this facility.

I am a friend or relative of a current/past resident

There is no communication from staff to family. During covid we can't visit, but window visits are allowed. We were shocked and disgusted on the condition of my sister. They closed the mini blinds and were hesitant to reopen them. We were there for 2 hours during lunch, they did not feed her. She is there after suffering a stroke and had been in hospital for 5 months. They have not been giving her physical therapy, or putting the boots on her feet for foot drop. She has foot drop due to their negligence. Bad attitude from staff and administrator has very little to say. Please don't put your loved one here. We had no choice as the hospital transferred her there we are working on getting her out of there.

This place is horrible and wouldn’t recommend. Bedside manners are aggressive and pretty bad. So many other better options. It’s smells and Covid cleanings are not done adequately.

I am/was a resident of this facility

Earlier this year, I was a resident at Windsor Quail Valley. I did not know what to expect upon arriving at the facility. I did not know why I had to go to a Healthcare facility after being discharged from the hospital after having major surgery. Because I was unable to walk, I needed extensive therapy. I was afraid and did not know what to expect. Upon my arrival, the medical personnel put my fear and anxiety at ease. The medical staff was very good. However, it was the Therapy Department that I enjoyed the most. The group consisted of speech, occupational and physical therapy. For about thirty-eight days, this group of angels worked with me and helped me meet my goal of walking again. I looked forward to going to therapy each day. My desire to walk again was their primary goal. My goal became their goal. The group consisted of six therapists. They function as a family. The patient knew what to expect every day. The therapy program was well designed to meet the needs of the patient. They also provided transportation for patients to keep their doctor appointments. Not only did transportation take the patient to the appointment, they stayed with the patient through out the appointment. Windsor Quail Valley has the best Therapy Department . Their goal is to meet the needs of the patient. They make going to therapy fun. I would recommend Windsor Quail Valley to other persons I know. To the therapy department, I shall always remember you and be forever grateful. On a whole, I found no major problems at Windsor.

My mother is in Windsor Quail Valley, and we are pleased. They have lots of activities. The food is very good and tastes like my mother's cooking.
