After two months, things at Paradigm at First Colony Health seem to be going well for my mother. They seem to have plenty of staff who are nice, caring, and looking after everybody. They have one section that is isolated and two other sections that are not. There are plenty of activities and they seem to keep everybody busy. The facility is getting a little old but they do have a remodel coming up next year.

If I could rate zero stars I would. If anyone has a loved one here I would get them the hell out ASAP because my father is in the hospital fighting for his life. He was found unresponsive here at this facility a few days ago. This place is horrible. The staff doesn't check on the patients at all. The staff all have very bad attitudes. This place is very filthy and smells very bad of urine. I've even heard one staff member saying how she wants to slap some of the patients. I feel like most of these staff members don't care about any of these poor patients. I hope this place gets shut down

The room are terrible. The bathrooms are filthy. Our loved one had on the same clothes for four days. Their diaper was dripping it was so full of urine. No toilet paper, the tile on the floor had been removed, the seat on the toilet was completely off. The water in the bathroom was the color of tea. The nurses don't speak good English, it's very difficult to communicate with the staff. There are patients walking around with no underwear on, one lady had no top on. I wouldn't put my dog in this place.

This is the most depressing place to take your elderly. My father was there for a short rehab period, and when home in a horrible shape. Hungry, weigh loss, and in same or worse condition as he arrived. He didn't even have a diaper on, and smelled like urine. His hair was never washed. Don't take your family there. Front desk staff is rude when visitors come in, or responding the phone. The insurance provider and Medicaid should be aware of the services at this place. DON'T TAKE YOUR LOVE ONE THERE PLEASE.

My dad is currently in Paradigm at First Colony Health and Rehabilitation Center. So far the place is good. He likes it, it's not new or anything but they treat him well. One side of the place is for the memory care part, and the other part is medical which is where he is. There's just him and maybe another 10 patients, and he's always in his room watching TV.He likes his room because he's by himself. The staff is good and attentive. When he calls the staff through the call button they come quick when he needs help. He really likes the food there. When he was in another nursing home he didn't like the food. He has Medicare so they're the ones paying for his care, we're not paying out-of-pocket. They don't tell you what you're going to eat, they just send you whatever it is and everybody has the same food.

Not a good place! Please do not think your loved one will be in the best of hands. They treat you nice at the beginning because you're new. Wait a few days and you'll be treated like the rest. The staff seems like they are just there because it's a job. Most of the cna's do the job because they have to get a paycheck. Not because they love their job. It shows it how they treat those poor residents there. That place smells like urine all the time. They must not have enough employees or they are over working the ones that need more hours. They need to get trained properly on how to treat the residents and the families with their concerns.

Overall not a friendly place. They should TRAIN their staff to listen and be respectful to families of residents there. And to remind every staff member to treat everyone like they would like to be treated. If a CNA decides to work at this facility Please learn to be the 1st CNA to set an example to all the unhappy CNA'S. This can be one of your loved ones in the near future. The rooms are not very clean and the hallways smell like urine.

My mom is a current patient of this rehab facility and I would NOT recommend this facility to anyone I know. My mom was taken here after she broke her hip for rehab and we had no choice on the facility because this facility was covered by her insurance and was close to my place of residence. Since being here they lost all her clothes, her shoes, she has gotten two bed sores and now one of her surgical sights has opened. I'm there every day and no one had the curtsey to contact me about my mom's wound and it being infected. Most of the CNA's that are working there don't give a crap and you have to constantly be on their cases and some nurses won't even give you the time of day or pass you to another nurse. Hopefully my mom won't be here any longer, but if you can help it DO NOT send your loved ones to this facility!

My dad was in this facility. It is filthy they ignore concerns. My dad wounds got much worst when we transferred him to First Colony. He also developed new wounds. They only clean the bedridden patients up if they think someone my visit or not. I have a photo of a Cockroach that was on the side of my dads bed, it was a window in the room what really pissed me off was that nobody hadn't checked on him before I arrived or they just didn't care. Anyway proceed with caution.

I Wouldn't take my loved ones to this place. The staff of cna's are rude..the nurses dont care about patients and leave them dirty. I wouldnt recommended it in the world.

This place is terrible...please don't bring your family members here. My mom was here.The worst mistake EVER...she practically had to beg for her medication on a daily basis...she had a small bedsore when she got there..it took them a two weeks before a doctor to prescribe medication to treat it.. as for as the doctor...he never once discussed anything with the family members. We never even met him the whole time she was there. The staff is very rude and don't have any compassion for the patients except Roxanne... I was there once and heard a patient fall down in her room I was the one who had to run to the front to grab a nurse. When I got to the nurse station 3 workers was there all on their phone texting. When the heavy set Nigeria ( aide) acknowledge me she took her slow time getting to the patient room. And I heard the aide yell at the patient "WHAT WERE YOU TRYING TO DO" this people are only there for a pay check...This place smells really bad. And its not clean at all

My father was put in this nursing home back in April 2014. We had to make a tough and sad decision to do so due to his progression of Alzheimer's. The staff was very unorganized and there was a huge lack of communication. My father did not receive the proper care he needed and was cut off of his medications from one day to the next instead of being weaned off. I have doctors and nurses from the VA who have agreed with us that he was not properly cared for. I went to speak with DEBORAH after a whole week of getting the run around from her after my father has passed. She is supposedly the head Admin of the facility and she was BEYOND rude and heartless. She did not care about what I had to say, did not care that I had complaints on her staff. She had an excuse for everything and did not want to hear it. My father passed away too soon, because of this facility and I have the proof. I have also had other families from here tell us the horror stories they've incurred. Please do yourself and your loved one a favor and choose elsewhere to send them.

I was very impressed with First Colony. I visited it twice. It was sparkling clean, and they have good meals, which I observed. The manager was impressive, and the marketing director was very nice but would not accept my husband.

Great facility, the nursing staff and nursing assistants are caring and strive to make your loved one and you feel like you are part of a family. My mother broke her ankle 2 years ago and she stayed there for her recovery, I was so impressed that when she broke her hip the following year, I knew this was exactly were I was going to send her. Great place I would definitely recommend it,
