I have a friend that has been here a year. They have put new floors and new furniture. The floors are clean and polished. It looks good when you walk in. They put lipstick on a pig! There are holes in the walls in the residents rooms and there were no baseboards for at least 8 months. The residents are not kept clean. It looks good when you go in but the halls smell like urine and poop once you actually get back to the rooms where people live. Don't even try to call and speak to someone. They do not answer their phones. I have been on hold as long as 25 minutes. Hung up and called again. Same thing. Finally went up there. The beepers go off at the nurses station for very long times and they just ignore them.... people need help. Lately we have noticed that the activity calendar is completely filled out but no activity going on at listed times. Looks really good on paper but not really happening. Cleanliness of patients is not a priority. Even whan they know you are going to show up at a certain day at a certain time, every week to take the residents out of the facility. I have called the day before, a couple hours before and even asked when I arrived that she be clean so she could go to a Dr appt or church. I find her in bed asleep in the 7 day old clothes. I asked that her hair be washed 1 time a week. They wet it and combed it over. Stiff as a board from the hairspray that was not washed out with shampoo. Happened 2 times. I have to clean her up as best I can to take her out of the facility. That includes changing dirty diapers that have been on at least 3 days because I dated it when I put them on. All we ask is she be clean when we show up, Hair washed so I can fix it and clean clothes on. It is a battle with the nurses every single week just for those 3 things. The staff turns over so much. There is no consistency. Only 1 nurse has been there the whole 12 months. The CNA's rotate so much we rarely see the same ones. There are a few that we now recognize, but when you ask them for something they say they don't work there. Guess they are contract but have been there for a couple months. My friend has had 2 back to back UTI'S. I had to request the Urinary analysis and even that took more than 5 days to get because I requested on Sat and they finally did it on Monday. No one saw her on a regular enough basis to recognize that she was acting odd and didn't feel well. No I would not put a loved one here.

This place is horrible. Understaffed and they do not take care of their residents. My sister was here on a couple different stays and she will never go back. They didn’t take care of her and let her get severe bed sores. She sat for hours sometimes to get changed. They are not friendly and just look at you when you had a concern. Would not advise putting a loved one in their care.

The worst place ever. Do not put your love one in here, they will not thrive in this place. The staff does not care, they give excuses all day and average wait for help is over an hour. Run from this place.

My husband was admitted with a serious brain injury on hospice service. At first it seemed ok. The PT folks did all they could for him. But then he began to develop a serious infection in his ankle. They had not even noticed it until I pointed it out. They said the wound Dr or nurse would treat it once a week. It continued to worsen until I asked that the hospice nurse be shown it. It had a terrible smell & was clearly not healing. My husband is bedridden. Today he was falling out of bed head first & no one responded. He has aphasia & has a great deal of trouble speaking. They blamed their lack of response on his door being closed, but even if it were open, they could not have heard him. One lady told me this happens all the time. When I questioned why I had never been told, the answer was that if he had been truly injured, a report would have been filed. Imagine being 91 years old & basically paralyzed hanging sideways off the bed calling for help for over an hour before anyone came, and then only because he was able to push a button on his phone & alerted family members! When I asked the name of the lady that said it happened all the time, she became very belligerent & refused to tell me her name because she did not want to be reported. Her name is Toni & she works in laundry. Watch out for more & more missing items.!!!! They have had almost a complete staff change over recently. Red flag!!!!

My brother is getting good care at Keller Oaks Healthcare Center. It's very nice and clean all the time, and I'm pleased with it. The rooms are big and wide. The staff are very good at responding to your needs. They have an activity director. They have bingo. However, I don't know very much about their activities because my brother is bedridden, so I spend time with him.

I visited this facility

My mother will be going to Keller Oaks Healthcare Center. It was clean and well-managed. The people that I came in contact with were very responsive. They had good reviews, and they were current reviews (they weren't old). Every nursing home has a list of deficiencies, and theirs were minimal. Where I am located, there are four or five nursing homes around me; Keller Oaks was the best one. They offer full care. The facility was light, bright, and no odor. It was clean and well maintained. I didn't see any overgrowth, rot, or pests around. It looked like a very nice community. We got a quote for $5600 a month, so it's on the upper end. It's expensive.

Stay away everyone! Moms currently there and is miserable. She pressed the assistance for help to the bathroom and 45min later they came? And this is a physical therapy place? Staff is rude! Bathroom wasn’t even cleaned when she was admitted! And when my sister asked who can help her to the bathroom they told her that she could do it??!! She been in a car accident this place is all kinds of wrong! I will be reporting and have video as well! Please don’t bring your love ones here I beg you don’t. They’re already helpless trying to heal. But if you don’t care about your love ones and how they’re treaded well you found the perfect Hell for them!!! Please don’t take loved ones there! Please !

The nurses are helpful when my friends dad was there. He had a few complaints but when the issues were brought to their attention the issue he had was resolved. He was able to get GREAT rehab and went home. Hopefully he wont need to go back for a long time but he would go back if he ever needed to.

I highly recommend that if you love and truly care about your loved ones, that you should NOT place them into this Healthcare Facility. The staff is rude and does not like to answer or return calls when checking on your loved ones. My grandmother died in this facility, and there was no compassion, communication, or empathy regarding the situation. They lied and informed my family that the 911 services declined their call for help for my grandmother when by law 911 ambulance cannot refuse services. The nursing and administration staff would lie consistently, informing us that our grandmother is fine since we were unable to visit due to COVID restrictions. My grandmother received bedsore in this facility due to the staff not ensuring that she was getting turned over and mobile. They informed us that this was just a small scab on 11/16/2020 and my grandmother died on 11/17/2020. This facility confirmed that they were aware of this bedsore on November 12, 2020, but informed us on 11/16/2020 and admitted to never giving my grandmother any treatment for the sore. Julie (the wound care nurse) stated, that my grandmother was scheduled to receive treatment on 11/18/2020. Unfortunately, it was too late because my grandmother had passed away on 11/17/2020. Please DO NOT place your loved ones in this facility. They WILL NEGLECT & ABUSE your loved ones and will not communicate with you about their health or well being. This place definitely needs to be shut down, and all employees working at this facility SHOULD NOT be liable to work in the health care field ever again.

My mother came to Keller Oaks to recover from brain surgery and an emergency colon surgery. She developed a HORRENDOUS bed sore that took months to heal. Personal items mysteriously disappeared. Would NEVER in a million years send a loved one here again.

At first appearances this facility looks very inviting. But looks can be misleading. My brother, a 100% disabled vet in a wheelchair, was re-located to this facility from another facility due to the fact he need a course of rehab after a serious UT infection. Prior to the UT he was able to transfer by himself from bed to wheelchair and wheelchair to commode. While at Keller Oaks the staff decided that he was not able to transfer and they moved him in/out of bed used bed pans instead of letting him use the restroom. He stayed in house for approximately one month. When we brought him home I had to re-train him to be self sufficient again, and it worked. If he had been there any longer he would of been too weak. The staff just did not do what was best for him to allow him to be able to be self sufficient, which is what he wants to be.

My grandmother was put here because she needed 24 hour assistance with mobility. She was fully aware of what was going on, but she was in a wheelchair and wasn't mobile on her own. She needed assistance to get to the restroom. Prior to moving my grandmother to Keller Oaks they assured my family that they could give her quality care and assist her as needed. By putting her in Keller Oaks was the worst decision we ever made. The care was horrible to say the lest. Once we saw the "true colors" of the facility my Aunt requested my grandmother to be brought back home. She was at Keller Oaks 7 days and during that time we had many sleepless nights because of the subpar care this facility provided. From getting her meds wrong to not responding when she needed to use the restroom. They even had the audacity to recommed that she use a bedpan because they were too incompetent to transfer her to the toilet. If you are considering this place for your loveone PLEASE KEEP LOOKING.

I visited this facility

The front entry is nice, however we noticed residents without call lights or lights that weren't working or answered?, Meals served that had been sitting for an hour, few staff and staff that spoke rude to residents. I would not be able to place my parents in a facility that told them to "shut up" or left them sitting in poop. I plan on reporting this facility to the Texas Department of Aging and Disabilities. After spending my professional life as a DON in a long term care facility I would assume these concerns are the tip of a very big problem and unsafe facility for their residents.

This faculty is sub-par. Yes some of the staff are nice people but the level of care over all is terrible. Not answering the call button at night, leaving someone on the toilet (who can't get back up on their own) for over 30 minutes, and not correctly giving a diabetic meds is unacceptable. This is not a safe place.

Reconsider your options prior to placing your loved one at Keller Oaks Healthcare. Leadership is non existent and uncommunicative with staff and family once committed. The RN's are terrific but assistants are scarce or absent. This was the very worst decision I ever made for my father.

I highly recommend Keller Oaks. It is probably one of the new ones in our area. It is immaculate. All the staff is friendly. The look happy working there in the facility and the word caring is most definitely utilize in that facility. Everyone from the office administration to the medical staff just seems to love what they are doing. They actually relates to the residents one-on-one. They call them by name. They take their hand and they stir them in different direction. They know the personalities of all their residents. They actually take personal interest in each resident there. The secured unit that my husband is in only consists of twenty residents right now with their own nurse station, their own aid and their own activities. They are always together as a group. They take them out on field trips and they have a special courtyard. Probably 99% of the residents there have Alzheimer's and some even need more care.

My mother had a stroke. She went to an intensive care and then to a rehab facility called Keller Oaks Healthcare Center. I chose this facility because after I reviewed and looked at other facilities, this was the one that I felt had the best capability to handle the variety of needs my mother has from speech therapy, memory therapy, to physical therapy. I felt this facility had all the best people with a caring heart. The environment is clean. The administration is very helpful and everyone seems very informative. All the rooms are double occupancy, but they are roomy enough to accommodate three visitors at the same time. They have plenty of light and ventilation. The nurse's stations are all close to anybody's bed. My mother is very picky, but she has not complained about the food. They are of very adequate quantity and quality. They are concerned about the menu and the issues the patients might have. It's really a very nice facility.

PLEASE read first. Here is what to expect from this facility. 1. The food is bad. Dont let anyone tell you different eat it for yourself and see. 2. Your belongings are going to come up missing. From cloths to food and personal items. Get a camera in your room so you can maybe get your stuff back. 3. Dont expect your loved one to get much activity here. 4. You will have to watch for your loved ones health because they dont notice things like colds and coughs and such. 5. Keep track of what med they are giving your loved one they will not call you or tell you when they add new meds. You would be suprised what they will give and for what. Just keep track. 6. Dont expect a warm reception when you visit. The nurses will just look at you and not smile or say hello or anything. So dont expect it. If one does you are lucky. 7. Baths are few and far between. Just saying. Beware and Becareful.
