My brother was admitted to Northgate Plaza for rehab on June 3 , 2019, after his defibrillator procedure. Robert in Admissions contacted me to start the process. He has helped us the entire time by seeing that my brother is comfortable. Even as busy as he is, he goes above and beyond his role as Director of Admissions. That is the mark of an excellent employee. The rooms are comfortable and my brother's experience with the nurses and staff has been wonderful. He loves them all. They are attentive and caring. My brother receives rehab/therapy twice a day and loves that it is helping him. I can't say enough about how pleased I am with the care my brother is receiving and how it gives me peace of mind while I am out-of-state. We are in the process of moving back to Irving, but in the meantime, I feel confident about the integrity and care giving of the staff of Northgate Plaza.

NORTH GATE PLAZA: The room was not bad and the restroom was a nice size No complaints with room... Robert was awesome beautiful personality very helpful. He would get back with me at each call. Finance department is just the worse. The nurses station is awful they will not acknowledge your present, right in front of you.. My checked my sister in there two days after the nursing facilities for close because of the COVID-19 . I was only able to see and conversate with her though her bedroom window. I was good with that arrangement, one assistant would allow her to video chat with me on Wednesdays.. She was not able to communicate verbally.. One Sunday she got out the back door and was gone for hours before a passerby fund her on the manhole on Northgate. She had a fever of a 104 when the ambulance did get to her and she had COVID. So the passerby my have got it from her.. The facility did not know how long she was gone for when ask. It had to be for hours ,she had blisters all over her arms and legs. THEN THEY PUT HER OUT!!!! PLEASE PLEASE do not put your family member in there. I had a real hard time try to take care of y sister and a harder time seeing her in the hospital near death... I have not received her rent refund after three months!!!

My family member was a patient from July to September 2020 during Covid-19 times. I think this place enjoys being closed to visitors, so they can hide behind their laziness on so many levels. For the patient, very frustrating things such as no one comes when you buzz, tv and phones don't work, they won't help you to the restroom, they tell you to wet the bed, they didn't bring all of the meals...I could go on and on. As far as a loved one on the outside trying to oversee the care- they don't answer the phone, don't answer voicemails, no one seems to know what is going on, phones don't work, and so many other things. Do yourself a favor and don't give your family this heartache. Choose another place!

My loved one entered the facility needing skilled nursing care and close monitoring. Administration was very good at acquiring the needed supplies and equipment, however the staff was not equipped to work the limitations the patient had. Patient hygiene was lacking as well as attention to critical care required. Sadly my loved one went swiftly downhill in the time spent here and did not survive. If you are thinking of this facility visit first and keep your eyes open. I found that staff would disappear and call lights were ignored.

They never answer the phone at night for them to be 24hrs smh... This place is not the same like it was 6yrs ago its so sad I recommend DO NOT send your loved ones here... The nurses and staff don't seem to to care they just see $$$$$ tv's dont work patients stay yelling for help they seem to ignore them it's very sad the patients rooms stay filthy the bathroom as well the food is horrible and dry. Its like a penitentiary for the elderly please if you can send your loved one else where if not and if this the ONLY PLACE they can go to I advise you to PLEASE GO CHECK ON THEM ON A DAILY BASIS and they'll say their doing good in there but their really not... This place has really gone down the hill it really needs to be SHUT DOWN!!!! I've literally seen patients ask for help and they ignore them like its not their problem.. Its very sad if I could own the place everybody in there would be fired

I wouldn't refer that place to a homeless dog.the rooms had little bugs biting me.therapy staff was rude. Adminstrater was very rude. I pray that I never have to go back. Staff was very lazy.

Do not bring your loved ones to this place unless you want them to die within 3 days of admission. The place is filthy, you can smell the carpet mold as soon as you enter. The admitting nurse was rude and did not even get my father his medicine until the next day even though he had his own supply. CNAs sleep at the nurses station during the night shift. Take your family members elsewhere.

Both my mother and grandmother have been to this facility in the past two years. After a very lengthy process of researching facilities we decided on this facility and will continue to go back if health issues lead us to this path. The rehab team is second to none and had my grandmother up and walking after being unable to walk safely for over 6 months. The staff is friendly and the food is very good.My loved ones were greeted by several staff members everyday and they were both taken care of by a very attentive and professional nursing staff and physicians. I would highly recommend this facility to anyone!!!

My 83-year-old mother was transferred to this facility on Friday 9/23/2016. To begin with they told us her bed was dangerous and were going to replace it. They also told us they would put a portable potty by her bed for easy access at night, they also said they were going to get a raised toilet seat for the toilet in the bathroom. These are all necessary for her safety as she can not even walk without assistance. None of these things has happened. She is not being provided hygiene products such as toothpaste. She has not been bathed since she has been in there. Which she came directly from I.C.U. and they only wipe you down with a wash cloth in I.C.U. She has not been thoroughly bathed in over a week. She has an infection in her arm that has not been addressed. They are not providing her medication (antibiotics) that she is required to take. Which is extremely dangerous for her because she is recovery from "cdiff" which is an extremely contagious bacterial infection that can re-infect if not properly treated with antibiotics. She is swollen up because of water retention and this is also not being addressed. They leave her at night for 30 - 1 hour in a soiled diaper. Then when they do come take care of her they act like she is a huge bother. I tried calling up to the facility to speak with a manager or someone in control about these concerns only to be told there was no one in charge up there until tomorrow. What kind of facility does not have a manager on duty at all times? It is absolutely necessary for someone to be available to address these sort of concerns. These conditions are severe neglect and are unacceptable!t, The staff is unbelievably rude. Not only to the patients but the family as well.

My mom was in Northgate Plaza. It was a nice place. They were very nice and very caring. The rooms were very nice, spacious, and clean. The food was alright. She liked it a lot. We all liked it. She had physical therapy, and they did different things with her.

I would not recommend this rehab to anyone ! My parents were there recently and it was a nightmare ! They lied to me every time I asked a question about their labs or pain medicine ! They took over an hour to answer the call lights !! Both my parents were very unhappy there and will not be going back!!

My husband is now in Northgate Plaza. This facility is in our insurance. I'm living with my son now and we're in Irving, that's how I've come across them. The facility seems adequate. The staff seems okay. My husband enjoys the food there.
