Horrible facility!! Nursing staff is lazy. The front desk person is always rude. The facility is smelly and dirty. My mother was here for a month. The cost is $12,000 a month for disgusting food, soiled beds, mold in the vents, and subpar food. I wouldn't let my dog stay here.

There is a Covid out break. The cases are rising quickly. The place needs to be quarantined but it’s not! Why? CNA’s are rude and not wearing masks. Even the regular precautions are not being followed. CNA’s are always on the phone sitting. Nurses ALWAYS ON THE PHONE chitchatting. Evening and night shifts are even horrible than the day shift. There is no information provided by the management about the out break and rise in cases. This place is a health hazard for community.

I placed my father with Alzheimer’s in Prairie Estates for 2 weeks of short term respite care while my mother was in hospital battling bladder cancer. Brought him home on a Thursday and started noticing symptoms on Sunday. He tested positive for Covid. I should of known as they called me twice, once before testing positive and once after, fishing for info about how he was doing etc. but never mentioned possible exposure. He died 3 weeks later. Now they are sending me a bill for $304 after I paid in full, in advance, for the 2 weeks he was there. I plan on sending them the bill from the funeral home. Families beware.

I have been here at Prairie Estates for a little while now. I don't have any complaints. The food is good and they treat me well. My room is clean and nice, and the staff is very nice. They serve my food in my room.

I am/was a resident of this facility

Some rooms tiny and nota all rooms ready for occcupancy upon admission Night (11-7) staff absolutely ignored patients in restricted rooms for over three nights, even after complaints to Director's office each time. Physician ordered medications which had been identified to the nursing staff were not made available to incoming residents within 24 or even some within 72 hours. No daily follow up was made by any nursing staff during the first ten "isolation" days, coming from the hosptal clear of any "covid-19). After going into general population, I was placed in the same room on return for two days, then back into "isolatio" upon hospital readmission and return to facility for another period. That is when I called my family to come and remove me from the facility. Therapies and activities and in the faciity are good only for the general population. Therapy started after the initial "isolatio" and wound was not checked daily. Call-lights on the "isolatio" hall were not answered for sometimes over thirty to fifty minutes, then the call-lights not turned off, so I did most of my own personal care without nursing assistance. The director did not meet with me or my representatives until I was leaving the facility, then wanted a recommendation. I advised her that she would not like it.

Other than the one or two nurses that are actually good at their jobs, this place is awful. I can’t tell you how many times they have left my mother in bed all day long. She will call and call and call the nurses with no answer. There have been times when my mom needed something quickly, no one responded to her call light, so my mother screamed for a nurse from her bed for 20 minutes with no response (her room is located right near the nurses’ station). Some of the nurses won’t even empty out my mother’s catheter when it needs dumped, so the bag will start to leak and leave puddles of urine on her carpet & her room permanently smells of urine now. Every time I go to visit, there are nurses sitting at the nurse station on their phones or just talk to each other while several call lights go unanswered. I’ve tried speaking with the director with NO response. I’ve left several messages for her. I keep getting told that I will get a call back - nope. I’ve also spoken to my mom’s caseworker about these issues & was told my concerns would be escalated. No one ever followed up with me as I was told they would, so nothing ever came of that. Surprise, surprise. I’m so sick of this place and how they ignore their residents. If you’re thinking of placing your loved one here, please think again.

This place is awful. Food is awful and delivered to residents late and cold. Our seniors should never be treated like this. I had to report several problems. Thankfully my husband is only here to rehab and will NOT be a permanent resident here. I am so sorry for the sweet people who work here and the residents who live here.

My mom is at Prairie Estates. She's been there for about six weeks now. She's just only taking two steps and she should have been in rehab, but it ended up being nursing home. They have a therapist that comes in but they just said, "She has to have the initiative." I think it's up to the therapist and whoever else that has to come in there, to get her motivated. She's 85 years old. They only allow two people and I'm not one of them so I can't go in there. My sister who was living within Frisco at the time when we're trying to find a permanent home, said that there are a few really great professionals there. I don't know necessarily who among the doctors, or the RNs, or nurses, but she just said that the few really good ones are on the ball and are trying and caring. But the therapist is very lax on her job, and she's doing it like for months. At six weeks of being there, she's just getting her to stand for like twenty seconds, and my mom can't go walk to the bathroom, she's totally in diapers now. They didn't read the paper work right or something, but she did have full functionality. Now they just keep her in diapers and she has a terrible rash. It's just really sad but I think they're more of a nursing home type, and I told my sister that I don't feel good about this place. It's convenient for my sister who is in Frisco. They provide physical therapy but this therapist, she's not doing her job. She is supposed to be getting her COVID vaccine but apparently it's a two-part vaccine, so I don't know how far apart that's supposed to be.

1 review just now If I could give this place no stars I would. I had to go last night (Tuesday, October 27th) and take my mother out of that horrible place. They would only let me come in two days a week and stay for only 2 hours. There are no phones in the rooms...I think this is so your loved ones can’t call you. She got a nurse to call me and my mom started crying and telling to come get her. I was there in less than 10 minutes. I told them that I was taking my mom home. They said I couldn’t without doctors orders. I told them to watch me. My mom is very easy going and tries to do what anyone asked her. So I know it must have been really bad. I was there during lunch yesterday. The food is horrible. She had a hamburger patty with a little sauce on it. The patty was cold like it just came out of the refrigerator...I know, because I tasted it. Along with that were some cook dried beans. The day before she had chicken fried chicken and mashed potatoes and macaroni..no vegetables. They ignored the call lights. When I was there, a lady down the hall kept calling for the nurse. She was yelling that she needed a nurse. She also had the call light on. No one ever came. I almost went to help her, but they were very strict about staying in the patient’s room. My mom told me that she, too, had been yelling for the nurse.....and I do mean yelling. While I was there yesterday, they took the sheets off of the bed. No one ever came back to make up the bed. My mom wanted to get in bed. I finally got someone to bring some sheets and I made up the bed. She also said she needed to do to the bathroom. We rang the call light, but no one ever came. Someone did come in and turn it off because it kept ringing at the nurses station, but no help. I had to stand out in the hall and yell for someone before we got help. If you have anyone in this place, get them out!!!! A place like this should not be in business!!!!

First impression is good. Stay for lunch or any meal & dining rooms are not cleaned properly.. Cutting back on staff & good food. Residents are not watched in diningrooms. Some choke on wrong food given to them. Today there are no aides 8/11/18 to help them. State visited this week so everyone was called in to work & now no help. Used to be well-run, but has gone down hill mighty fast in last year, especially since the staff has such a turnover. Don't have an answer to the problems. Family meetings & residents meetings do not accomplish anything. I have been going to Praire for 5yrs. & can probably say it has been a lot better in previous yrs.

I visited this facility

We tend to hear so many unflattering remarks regarding care facilities for our loved ones. We moved my 100 year-young mother from another nursing facility in south-central Texas the week after Thanksgiving 2016 to Prairie Estates, Frisco, knowing only remarks we read on the internet as well as reviewing Prairie Estates ratings with the State of Texas, and all the information we could acquire through any other source. Information from all sources regarding ALL skilled nursing facilities in the Frisco-Plano-Carrollton area is far from reassuring. We chose Prairie Estates after visiting at various time of the day or night, interviewing staff (Nursing-CNA's-Doctors-Rn's-Janitors-and various employees) to acquire as much information as we could attain. We were NOT wrong with our final decision of where to place my mother and are still thrilled with that decision. Our family can not be more pleased with the care and attention she is receiving. There are many fine employees as well as management at Prairie Estates - but...be advised we will be the first to report to Prairie Estates Management Staff should these accolades be violated. So far I am less than impressed. Actually, quite disappointed. My mom's medicines were not correctly dispensed the first 3 days. She asked ro for for a walk and was denied. They have not delivered the meals she orders. Either they send whatever else they want or forget part of it. They waited 40 minutes in the dining room to get their dinner, today. I could go on but I won't. I am spending 5-6 hours a day here to make sure my mom is okay.

What really pushed me to get my mother out of Prairie Estates is that I went in one day, and there were urine stains on her sheets. I opened her cupboard to check on her clothes, and her trousers or blue jeans that she wore were soaked in urine hanged back up. I made the nurse aware of it, and she told me that my mother didn’t like to wear diapers. I asked her about the pull-ups, the underwear style, and I was informed that I had to purchase them for her, which I don’t. Actually Medicare and Medicaid paid for them, but nobody in Prairie Estates seemed interested in taking care of anything. The staff seemed to care, but I made a couple of trips back and on both occasions I found wet clothes hanging in her closet. I bent down one day to tie her shoes, and the leg that I was kneeling on the floor with was soaked in urine when I stood up. I believe they are trying to get rid of all Medicaid patients; they only want private pay. It is a beautiful facility, but the patients are left in their rooms. They are not encouraged to socialize, and it is very quiet. Right before she left, they had a Code Blue, and they called the paramedics. The paramedics were working on the patient on the floor and instead of standing away, the nurses were just kind of standing around the doorway of the room watching. I’ve got a medical background, and I found just them standing and blocking to be really unprofessional. They should have been at the desk or taking care of other patients, instead of standing around and watching the paramedics.

My boyfriends grandfather lived at this facility about a year ago and I'm planning on sending my grandfather there! His grandfather was only planned to stay for 3 months and he left in two. The rehab and therapy are amazing! The communication between the staff and I were amazing and his grandfather is doing very well after leaving. The only thing I didn't like was the organization of staff. Sometimes they would only have one aid down a hallway. It seems like the employees can show up when ever or not show up because there are no consequences. Other than else, I love this place!

Brought dad here after hospital stay as it was close to our home. Don't make the same mistake we did, find a better place regardless of your drive. In our experience management does not return calls nor do they do what they say. Awful support. Some good folks on floor but most of them are happy on their phones vs taking care of patients. Got dad out of there and now travel 15 miles further to see him and thrilled to do it. Do your research before picking this place.

I visited this facility

The other one I like was Prairie Estates. They take Medicaid, and we were looking for a facility that takes Medicaid, so that was a plus. But the Medicaid does not give you a private room. Plus, the facility was one huge building. It was too big. It didn’t seem to be personalized enough because you have all these rooms in one big facility, e.g., the hallways were long, and it just didn’t seem as convenient for the people staying there. I didn’t get to see everything, but I eliminated it because of the cost. If you pay with private pay, it’s too much.

I do not recommend this place at all! My father was a resident there for quite some time and our experience was not good. The general nursing staff has little to no bedside manner and can be very rude. I actually witnessed it on more than one occasion and brought it up to the attention of the shift manager but nothing changed. Like other reviewers on this site, I saw the staff on their phones, sitting around, chewing gum, and looking totally unprofessional when they could have been doing something for the residents. The place looks clean on the surface but many times I walked in to my father's bedroom and it was in total disarray plus the bathroom was in disgusting shape. Sadly, my father passed away there and to this day, we never even received a note of condolence from them. My father was well liked and only a couple of employees were kind enough to reach out to us on a personal level to let us know how sorry they were for our loss; but other than that, nobody cares - much less management!! Don't be fooled by this place... it is a BUSINESS and they don't really care for the residents. They don't know the meaning of human touch and this starts at the top with the management! I repeat, DO NOT put your loved ones in this place.

My mother was a resident here for about a month and 2 weeks. The facility is really nice and the CNAs and nurses are pretty good. The worst thing about this facility is Administration who made all kinds of promises about what they would do for the patient but, never followed thru. Word of advise take care of your own parent or hire a visiting nurse or home health to come to your home to help out daily.

Prairie Estates was recommended to me. The rooms were nice, and the staff was very nice. They had a lot of activities. They used too many medicines as treatment, and my mom would sleep for a day and a half. They have high school students that come in on Fridays and work with the residents. They also have people come in and sing, bible study, and arts and crafts. They have plain medium size rooms with a moderate amount of light. They have two separate dining rooms. The security and safety was very good. It was really hard to find the nursing staff there. There should be more availability to direct contact to the nursing staff.

Prarie Estates is deceiving. The sales and admission staff talk up the level of care that will be provided for your love one. At admission your love one is shown support. However, once there appears to me more than a need to administer medication or bring meals, the support changes. The two most common responses to questions are " let me check" or "that is someone else's job". There is no empathy or caring for patients. Their model. seems to support a one stop shop theory. By that I mean the assumption is made that all elderly people are the same. They do not change their approach to interaction based on the patient. There are different levels of cognitive skills in an elderly patient. This does not seem to matter to Prarie Estates. They are impatient if a resident takes too long to eat or does not immediately swallow medication or may not be ready to bathe at an exact time. While it is understood that there is a schedule, it should also be understood that the elderly often needs more time. The primary reason someone is there is the care. The place is clean. There are activities for the residents. However, If the care of the patient/resident is lacking, the facility deserves a one star.

This facility is under staffed and a very low morality due to being over worked. No one seems to know anything with you inquire about care. The management doesn't seem to care or follow up on any complaints. Its difficult to call in the evening to inquire about your loved one because there is no one at the reception desk after 8pm. The calls roll over to the nurses station but since no one is ever at the nurse station the phone rings and rings. Last time I called it was over 20 min before anyone picked up. Inexcusable! The place needs a total makeover from director on down.

Prairie Estates has single rooms, one person per room. It’s just a typical room with a shared bathroom, very spacious, and roomy. The facility is very nice, clean, and very much like home. The people were very friendly and caring. Dad was happy with the food while he was there; they had a choice of where they want to eat, and they had a good variety. They have bingo and little private activities. I would definitely recommend this as it’s a very nice facility.

I visited this facility

I went to visit an aunt who was at Prairie Estates for rehab. The facility was clean and well kept. Her room was very nice. She did have some issues with the caregivers, and she had to report some people that were not good at what they were doing. The staff treated the patients roughly and were not cognizant or caring and gentle with them. They didn't listen to what the patients had to say as far as handling them physically. The rehab was fine. She's home now.

My dad stayed three weeks. We couldn't get him out of there fast enough! The attendants spend most of their time on their cell phones talking or playing games, the nurses favorite response to everything is, "I'll let someone know" - we never found out who "someone" was because they never appeared. Dad cannot get out of bed unattended, the nurse call button was ineffective - typical response time was an hour or more, if at all. If my family and I had not kept on the staff he would have been ignored. He is constantly left in soiled clothes to the point is is chaffed and raw. He is on a feeding tube which they "forget" to hook up to the point he often complained of being hungry - something we never heard in the four years he has been on the tube prior to coming to Prairie Estates. It looks nice from the outside, but don't be fooled. The care is HORRIFIC! They couldn't care less about their residents. I'm sure the owner is most concerned for the profit, because he certainly doesn't appear to be concerned with the level of care provided! STAY AWAY!

My mom is 86 and came to the facility with a broken right arm. She is diabetic. We had problems from the staff from the start. It takes 20-25 minutes for her call light to be answered. From 2-27-14 until 3-3-14 we couldn't get her meals straightened out. The months menu wasn't provided upfront. The diet really wasn't a diabetic friendly. Often her meals arrived late. One day I ordered crab cakes for her and we got pasta salad (it may have had some crab in it). At lunch one day she was served a cheeseburger on a bun, a roll, French fries, a dessert, & salad, inappropriate. My mother was on oxygen and the techs often left it off. Her arm was to be elevated & this was a constant problem, which resulted in her hand and fingers swelling. Knowing a permanent cast could not be set until the swelling had gone down. My mother started to vomit while reclined. Her nurse was in the room. He wasn't helping and I told my sister to get her up so she wouldn't choke while I held her up from behind. The nurse went to get a garbage can. The same nurse brought her 7 pills without any drink to take it with and left her alone. My mother is a choking risk. Her bed was uncomfortable so the physical therapist suggested they order and we purchase a gel pad for her bed. We did. After all of these problems we decided to move her to Stonemere a very nice facility. The therapist came around and collected from me the fee for the gel pad before we transferred her out. Later my sister was told that the administrator had told an employee to destroy the check & that the order would be unfilled since she was no longer a resident. I could go on but won't. I can not recommend this facility. This nursing home staff for the most part are uncaring, unprofessional, & understaffed.

My mother was brought to this facility after a 4 day hospital stay from a UTI infection. I was told she needed P/T to strenghten her legs again. The P/T dept was great with her. But as many times as I told the staff she is deaf and has Alzheimer's and does not understand she has to ring the call bell when she has to use the bathroom, or even realize she has to use the bathroom, she was always sitting in a wet diaper when I went to visit on a daily basis. In the 2 months she was there, she ended up with 2 UTI infections from not being changed often enough. Her room always smelled like urine, because she was soaking wet and the wet clothes they removed from her were just thrown in the bottom of her closet and not put into a plastic bag. Her discharge was changed to the following week and P/T did not pick up again while she was still there because "they already discharged her last week". Even though she was still there another week, she was supposed to be receiving "restoritive therapy", but not one person gave her any therapy for a week. which means she just sat in a wheelchair or laid in bed for 7 days with no therapy at all. I was told "I'm sorry, i guess we dropped the ball on this". Her strength in her legs were worse when she came home. I took her out of there on Friday and she fell on Sunday, and is now bedridden. I don't feel that was good quality care. Then to find out that some of the information I received from the social worker was not true a far as Medicare covering transfortation home for her and her qualifity for a bariatric bed which she had in the facility. I paid for transportation home to get her out of there. I called Medicare myself and was told they would have covered it and also they would also cover a bariatric bed for her. They are very short staffed as far as the aides. It is a beautiful facility, but the care is horrible and I do not recomend the place at all.

My m-i-l has been a resident for two weeks and the staff member at the desk of that wing still doesn't recognize her name if asked her room number. I don't expect you to have the number memorized but don't scare visitors into thinking the staff doesn't know names of who they have. The facility smells like urine all thru it and it takes 30+ minutes for a staff/nurse member to show up if you need them. She plans to leave or transfer. She said the PT staff is fabulous, though.

My mother was a patient at Prairie Estates and I could not have been MORE PLEASED with the care she received. In September, my mother fell while getting out of the bathtub and broke her hip. She was in the hospital for 4 days and then transferred to Prairie Estates for Rehab Therapy. I cannot say enough good things about the amazing staff that helped my mother recover and return home. The nurses really seemed to care and enjoy each of the patients they take care of. Each went out of their way to make sure Mom was "having a good day" and that she was comfortable. I found the therapy staff to be knowledgable and caring as they helped my mother recover from her fall. They even went to her house to evaluate what specific challenges she would have in re-entering back into her home and made that a part of her therapy. I would truly recomend Prairie Estates to anyone. My mother has been back home for over a month now and to think that we would all spend Christmas back around her table enjoying each other is truly a blessing. I truly Thank God the social worker at the hospital sent me to tour such a great place.

Stay away from this nursing home! Not only is the care bad but the business is not well run either. We had our mother-in-law at this place for a few months prior to brain tumor surgery. She had 3 diabetic coma's due to poorly administered insulin. Two times she was transported to the emergency room with the nursing home claiming she was having a seizure when in fact it was a insulin shock - too much insulin in her system for the amount of food they were giving her. Then shortly after the brain surgery, they had her evaluated by a doctor who claimed she was mentally incompetent and we had to move her to a locked down facility. They gave us less than 2 weeks to find a new place. Turns out they had her so doped up, she had no idea where she was, who she was or anything. Now that she is in a new place, they are suing my husband and I for her debts! She is a Medicaid patient and has no funds that are not controlled by the government. My stay at Prairie Estates

Food is terrible. Most employees are nice and caring. Beds are extremely uncomfortable. This place is like a German run camp. Our way or the highway. If you are in your 80's or have Alzheimers it might be ok for you, but this is mostly a nursing home............If you are under 60 you might want to reconsider............. It looks nice but looks are deceiving........ I hate it here and will never ever come back here.

My loved one stayed here

The staff is poorly trained and indifferent to patient needs. My mother's legs were swollen with water to the point her legs were numb. The doctor didn't want to treat this condition. I took me calling the primary care physician to get this treated. Medication distribution is inconsistent. As an example the morning medication was not delivered until 1:45pm. For someone on medication 4 times a day this means missed dosages. Complaints to floor nurses and to administration result in no change. The facility looks nice because it's only a couple of years old, but the quality of care is below par. There is not adequate staff available to meet the residents needs.

I stayed here

This place looks nice but the care is not good. We never have enough aides and the directors do not care about our problems. Sometimes we don't even get our medication for several days. The activities are pretty good but there's not enough choices. It's always the same things over an over. They fire all the good staff an keep the bad ones.

My loved one stayed here

our mother was there three months, she fell four times finally breaking a hip. after her hospital stay we found another place. Staff was rude, very slow to answer call and did not keep us informed of her condition. it looks beautiful but the staff is simply not trained, tryed management -that did no good. they were always out, busy or in a meeting.

My loved one stayed here

My dad was here for about 4 months. Faculty was slow to respond and poorly trained. Turnover of staff was frequent, even in administrative staff. Facility was new and very luxurious. Rooms were a bit small, but well appointed. Staff shortage was a real problem.

My loved one stayed here

My mom has been here for 3 months recovering from a stroke. I can not say enough good things about the people who are taking care of her. From the rebab department to the nurses to the attendants my family has been very pleased. No place would be as good as home but this place has been the next best thing. We feel very blessed to have found such a wonderful place for mom.

My loved one used this service

Very nice, clean appearance. The nursing staff was oftentimes rude, neglectful, and sometimes would not even respond to her questions. Although complaints to management were made, no improvement. One to three hour wait for nurse to respond to call light. Therapy staff was excellent.

My loved one stayed here

My mother stayed in this facility for three weeks. The nursing staff, and, or PA did not catch the fact she was dehydrated. We informed her nurse twice she appeared dehydrated, and was not taking in fuids. We also were not informed she had diarrhea. We called an ambulance our selves for her and took her to the emergency room. She had C-diff, and was severly dehydrated. She died within a day and a half.
