Autumn Leaves of Cityview is kept clean. The people who run it are very nice and caring. They are (like most places) having staff shortages. I see the residents, and they seem to be well cared for. I have seen a few accidents from patients (getting sick or going to the bathroom in the hallway), but they are immediately on it and clean it up. They disinfect, and they're very thorough. They have an activity room, and they do all kinds of crafts and activities. They have two television rooms and a workout room with a few items in it, but I don't see a whole lot or anybody really using that. Although my husband was gonna get some physical therapy, but he refused. They have a very good mix of men and women. My husband has memory issues, but he needs other men to tell all his stories to (even though they may not be listening). I haven't seen anything that they refused to do or that they haven't followed up. They have a cleaning lady that's there every day. She goes around and cleans each room and the bathrooms in the whole facility. Each resident has their own bathroom and some have joint rooms. My husband has a private room because he's a little too aggressive sometimes. There were trying to adjust his medication, and he seems to be better now. They have different weekend staff and regular day staff. Their kitchen facility seems to be a little tight. They have a lot of residents, and they have two different rooms that they put the residents in to eat. They have a fairly good size dining area, and they have another smaller one that adjoins it. They also have a private dining room for family members, if they want to come and have food with their family in there. I'm usually there before the evening meal, and evening meal seems to be extremely light, and I know that's probably fine for a lot of residents. However my husband's diabetic, and for him to go on a very light meal and then go all night until breakfast, it makes his sugar go really low. They are getting a handle on that. I get a report from his meter that comes directly to my phone when they test his blood sugar. I get an overall rating for the week every week. They have seemed to have gotten it on the average within guidelines for blood sugar. They have a nurse there all the time during the day and she follows up with the medication. The facility is exceptional. A lot of the activities are in the mornings because the residents are more alert at that time. There's a lot of people who walk around and disassemble the cushions off of the couch, take the pillows, and they walk around with them and hold them. The aides come and take them away and put them back on the chairs or on the sofa. They keep it looking nice, and it's always nice. It's extremely expensive. They did just go up on the cost by $200 more because of the supply cost. They are going to be providing the pull-ups, pads, diapers, soap, deodorant, and toothpaste. It's a little bit expensive compared to some of the others, but for the quality of care the people seem to be getting i's better than most of the others.

My dad moved into this facility on 1/4/22, healthy and walking. We discovered him 4 days later in horrific condition and we had to notify and alert the staff at the facility and insist they call an ambulance. He arrived at the nearby hospital with a broken femur, hip, multiple lumbar fractures, black and blue bruising all over his backside and contracted MRSA, all within 4 days. He spent 2 weeks in the hospital and then died 10 days after that in hospice on 1/28/23. This all occurred within the 4 days he lived there and no one has any answers or has offered any suggestions or any help whatsoever into what happened. It absolutely wasn’t his time and should have never ended this way. Please please do not let this happen to your loved one!

Hello, This is a great community the staff and directors are very caring. my family member has improved since we have brought her ,even gaining weight. This community is very clean and always smells great.' The rooms are nice and a great size. The activities are great, they have them busy all day. I would recommend this place.

Autumn Leaves of Cityview was a very pretty community, but the director was horrible, and they had a big turnover for help. I was not pleased with the help or the care. It was not a good situation for Alzheimer's because they need to have the same people all the time. These situations make them very agitated. They had constantly new people and new staff taking care of their needs, and that was very stressful for my mother. She didn't like it, and I was quite often upset. The facility was bigger than where she is now and had more activities, but the dining area was very small and very chaotic. I don't feel like the residents really got their needs met. There just wasn't a lot of continuity.

In all my experiences with Autumn Leaves I have never been dissapointed with any aspect of service. When I first visited the facility, I was taken back by all of the fine art, and artistic views openly expressed throughout the entire building. I had plenty of opportunities to meet with the staff and on call nurses. I was very pleased by the attitude of the employees, and was not worried at all for my family members well being. The friendly staff of nurses were very helpful i n answering my questions regarding the daily health care for the patients. Along with fully assisted living areas it seemed like all of the people I met were in a very positive mood which led me to believe that my family member was going to be in good hands. It is always a last resort, but when running out of domestic options, and looking for an assisted living facility, I would highly recommend this establishment due to its caring environment and friendly staff.
