I am a friend or relative of a current/past resident

This facility is poorly staffed; they do not take proper care of those patients who are there for permanent care. Those staff members who are SUPPOSE to be in charge, are pathetic. The administrator is not in the know when it comes to the patients. He needs to come out of his office and do a personal inspection of those who tend to the patients. My sister almost died due to the poor service/care that she received.

I would NOT recommend this facility at all ... RUN AWAY FAST. A family member of mine stayed at this facility for rehab. The room that we were showed on the tour was totally different from the room that we received when she arrived. When we first arrived, there was a very, very old, out-dated bed in the room that had to be cranked up at the foot of the bed in order for you the raise the head of the bed up or down. There were "wet paint" signs posted up on the walls and paint splatter was all over the floor and baseboards of the room, some paint was also in the bathroom. Upon our arrived the staff stated that they were only informed about 1 hour prior so they had to go and find a handicap toilet seat, a wheelchair and they knew nothing about my family members condition, we were told that they had to go and read the file. There is a language barrier with the deep night staff which really needs to be addressed. Family member had several accidents due to staff taking too long to help her get to the bathroom. Upon pushing the button for assistance, the wait time was from 12 - 17 minutes or longer, we timed them. The socks that the patients wear are recycled, worn out and some of the elastic was gone. The sheets on the beds have holes in them, some of the pillows were torn or ripped and had faded pillow cases on them. There are 2 non working water fountains at the facility, one was stopped up with water and had mold in it and the other fountain had ants or some type of small black bugs crawling all over it (have pictures). Also, each day, the food was cold by the time family member received it.

I am a friend or relative of resident

Lennwood appears to be a nice place until you have a loved one that you send there. I have heard the staff yell shut up to patients that were crying out for help. I heard this as I was leaving one day. My loved one had only been there a few days and this made me worry about them more than feel comfortable with leaving them there. There were many times they were not properly cleaned and smell of BM and urine. The staff acts as if they can't smell it and just ignores it. I have watched them take my loved one to the rest room and just pull up their pants without cleaning them until I said something. Lennwood looks good but the staff is horrible and it makes you afraid to leave your loved ones there. If you take your love one there...Beware!!!!!!

The Lennwood is pretty good. I had no problems with them; anytime on medication situation or if he falls or anything, they called me straight away. The rooms were kind of small, but other than that it was OK.

I am/was a resident of this facility

I chose Lennwood Nursing & Rehab because it is within walking distance from where I live and it is fairly new and clean. It has very comfortable rooms. They have a pleasant and shaded patio with plants. The staff is friendly.
