My poor mother was put into this terrible place for one month and a week, approximately. She was under hospice care. The nursing home neglected her to a level that our family was unaware was possible. They lost her teeth, did not inform of us a possible suicide attempt (deep cuts on arms), did not change diaper or attend to skin issues, which led to a large and painful bedsore. They allowed her meds to be vomited from her mouth when she was positioned on her side. The HVAC barely worked. I brought in a thermometer to check. It was almost 76 when set lower. Thankfully it was not the summer. The only time they came by is if I begged or called someone. She sadly died in this wretched place. I would not suggest this place. The only nice person was the activity director who facilitated a video conference.

This place is NOT the place to take your loved one if they are in last stage of life care. They will be forgotten about like a penny in the corner. I watched my wife deal with her mother being at this place and it was terrible. Unchanged adult diapers. Unreported marks on arms. Unwashed patient. Bedsores. Non working AC. About the only thing this place did partially right was deliver the medications on time, and even then that was sometimes sketchy. If you love someone, do NOT leave them here. Even though losing a loved one is traumatic, this passing was not made easier but only more difficult due to the neglect we saw and the seemingly oblivious staff

My aunt stayed at The Hillcrest of North Dallas (Formerly Premier Transitional Care of Dallas). The social worker was extremely great and the administrator was very good as well. There were a few good things about it, and it was mainly those two people. However, I would have my relative live at home before I would put them there. It deals with a lot more serious medical conditions, and they had skilled nursing there. She was in a shared room and the only thing separating her from her roommate was a medical curtain that you can pull. They had some activities, people play piano in the lobby, and they would play bingo. It was a very depressing environment. Most of these people didn't have any visitors, and it smelled awful. We wouldn't feed the food that my aunt got to our dogs; it was bad.

The facility is beautiful, when I first walked in I noticed there is no bad smell coming down the hall. I know that they've had bad reviews in the past but the people that there now are definitely changing and everything's going for the better.

My father was in Premier before. I took him there to recuperate. He cannot speak, he has a long hair and a beard, and they shaved his hair and beard. I was furious, because they did not listen to me when I put it in writing not to cut his hair.

If I could give The Hillcrest of North Dallas zero stars, it would be more accurate. This nursing home is very understaffed. My mother was often required to wait 45 minutes to get help after pressing her nurse call button. Many times she would run out of diapers or briefs. Staff gets wipes from other patients rooms when they run out. I was constantly worried about cross contamination as this was common. Staff are very dissatisfied and overworked as they have only a couple of aides to take care of 70 upstairs patients. Families are not happy either, they have a Family Council that meets and discusses concerns and reports them. Nothing happens. Ask for a copy of the Family Council minutes to see all of the problems experienced. There were medication errors.with my mom. I had to check on her every single day. Had to move her finally as nothing ever changes. New management is horrible, they do not care.

Preimer Transitional Care is not the same company anymore. The staff gets frustrated with patients, are argument I've with each other and family members. The less you visit, the better because they don't like seeing too much company, as that is the main time they come in the room and actually check patients is after a visitor comes. They do try hard to convince you to get medicaid, more money for them & you can loose your home in the end. When it 1st opened it was terrific. Now iit's owned by a new company, and not the same quality of care. If you want your loved ones to be in good health and return home eventually, maybe consider a different facility. If they have minimal health issues & need a place to live, get meds that were previously prescribed, and meals then this is your place. Any real care or health issues, seek elsewhere. Would give less stars.

Premier Transitional Care was highly recommended to us by physicians. We investigated the facility and we're very satisfied with what we found. It's fairly new, very clean, and very well-managed. She has been there for three months now. We have food service, physical therapy, physician care, 24/7. She has dementia and she has good days and bad days, but we are quite satisfied with where she is.
