Farmington Square Medford is a low budget facility. The positive is that they have what they call cottages. The residents are grouped by entrance state and cognitive abilities, so as they need more care, they go together. The rooms are adequate. It's clean. It's not spectacular, but it's an older facility, less expensive, and it reflects that. Their amenities are basic.

Dora and get staff made the move fast and easy. I had so many questions and concerns and my phone calls and emails are always answered promptly by the staff person knowledgeable about it. Dora and get staff seem to understand that I am transitioning too. They made a difficult decision easier as I knew my friend would be safe and well cared for. My friend says everyone is nice, there is lots to do and she got to take her dog with her when she moved. The outside area is big and allows for walking the dog and has seating to visit and enjoys the beautiful trees.

Overall, we were impressed with Farmington Square Medford. We liked the staff that we met with and liked the facility. My wife was very much leaning towards going there. The staff, Theresa, she was very helpful and very knowledgeable. She walked us through everything in the facility that we asked about, answered all our questions, and I followed up with emails with her since we left. So, she was very helpful. I don't believe they had a salon. They had a nice outdoor area. It's a very nice street, a very nice community with very clean, big trees. The facility itself is really nice. They're set up in separate cottages where it goes up to 20 people in each cottage. They have four cottages; and what they call cottages is they have separate bedrooms with a big living area. So, they have their activities room where they were doing games, puzzles, and watching TV. I think they even had music stuff going on in there as well. It was nice. We're considering putting my mom in a semi-private, which would be a roommate. What we liked about this facility is there was an island, a divider

I am a friend of a resident at Farmington, and I have made it my habit to visit my friend once or twice per week. I visit unannounced, on different days, at different times. Although the exterior of this facility is well kept, the same cannot be said of the resident's or their rooms. Unfortunately, my friend's labeled personal bedding and bath towels have gone missing. Her bathroom is dirty with caramelized soap, and toothpaste, and it is not unusual to find wet, used towels on the floor and in the shower stall. The toilet is black under the inside rim, and there are usually spots of urine and fecal mater on the toilet seat. Her bed tray, clothing, and pillows are crusted with dried food. Dirty dishes are left behind. Lint, food, and stray clothing litter the sticky floor. My friend is in "hospice" and she needs assistance to drink, but straws have not been provided by Farmington, so I brought straws for her to use. The only time a straw is placed in her water glass is when I place it there. Yesterday, when I complained to staff. The aide said, "I didn't know she had straws." And then he proceeded to ask me about an opened, container of yogurt left on my friend's bed tray. He wanted it fed to her roommate. I was taken aback. Meanwhile, my friend was laying uncovered on a urine soaked bed in a dirty shirt and wet adult diaper. The day before yesterday, my friend held my hand and asked me, "why.....why.....why.......I don't understand......it doesn't make sense." I have to agree.

When my husband was diagnosed with Alzheimer's disease in 2012, I began a search for local assisted living facilities. I wanted to have a good sense of what was out there so that when time came and he would need one, I would not have to start from scratch. I visited several and was most pleased with Farmington Square. During the summer of 2014, when I realized I could no longer care for him, I did a re-visit of those same facilities and came away feeling Farmington was still the best place for him. He has been there for almost 3 years now and I am very pleased with the care he is receiving. During those years, a new executive director and assistant director were appointed and the changes have been profound. John is well cared for, much loved, and safe in the facility. The staff is also very attentive to my concerns and questions. I feel fortunate to have him in such a compassionate and well-run environment. Big thank you to everyone at Farmington Square.

Our visit at Farmington Square was informative. The staff was great and not staffed by high school girls. They had long-time employees with good hearts. There was just something I could not put my finger on. I couldn’t see my husband staying there. There were a number of apartments: studio, one-bedroom, and shared room, where there was a half wall in the middle, so two people would share the same space but on opposite side of the rooms. She said that people don’t see each other in that type of setup. It was more of an assisted place.

I made an appointment to visit Farmington Square, but I sat for 20 minutes before anybody came into the lobby, then she acted like I was taking up too much of her time. The residents were walking around in their night clothes and that turned me off. Obviously, there wasn't any thought put into making sure they were dressed and ready for the day.

For the most part everyone is very nice and organize. Except just recently, I was visiting my husband with family members in building A. I'm posting this comment for a reason hoping that for something to be done and to fixed this situation or I will remove my husband out of this facility. I did not appreciate one of the caregiver with lots of tattoos all over her arm swearing, calling my husband a fool and top of that constantly walking around my husband room talking on the phone. I think she was surprise that I walked in on her on her phone. Very unprofessional!!! My husband is very happy living here. PLEASE PUT A STOP TO THIS AND REMOVE THE PROBLEM. If not I will write to corporate.

I did not have any friends or relatives who stayed with this provider, however in a previous line of work I had a few chances to visit. Farmington Square had spacious and decent surroundings and decor, if a bit boring and hotel-like. The staff seemed to be well organized though somewhat rushed, as if there were nearly more clients than the workers could handle. The general attitude of the clients seemed to be on the dull side, as if not enough attention was being paid to keeping the atmosphere upbeat and positive. The television in the main room was of good quality, and the lunch room was layed out like a banquet hall with most residents dining at one very long table. The grounds were well kept, though there was not a large amount of outdoor area set aside for residents to enjoy. Overall, If I had the money I would prefer to stay at a smaller facility with more caring staff.

I specialize in Alzheimer's/Dementia so I have years of experience. One of the many things the Memory Care facilities are supposed to be good at is pass out medication as needed. A relative told me that the staff here are unable to get the resident to take any of the medications prescribed. This is a shame. They call this a specialty facility. Think twice before taking a relative to Farmington Square.

My experience with Farmington Square was very upsetting. More than once I found my father in saturated sheets with dirty briefs on the floor. In an attempt to get some help, I walked around trying to find a staff member and didn't find anyone for close to 10 minutes, I was then told that they were assisting other residents and argued with me when I got upset. After several other horrible events, I took my dad out of Farmington and placed him in another Memory care which had been fabulous. I would highly recommend not be sucked in to Farmington Square, they are high priced and have low quality of care.

My friend who had Parkinson was a resident at the Farmington Square Medford Oregon location. She stayed in the medium level care section of the facility. This area had a nurse and aides to assist the residents. The staff was always friendly when I visited, and the other residents were fun to get to know. They provided afternoon actives and allowed family and friends to join in. The meals that were served to the residents were both nutritious and delicious looking. I thought it was nice how they had set the dining area like a family home. The residents sat around a large dining room table and were able to converse about their day or their personal ailments. The facility was always clean in shared areas, and there was of course air-conditioned. The only downside to this facility was the lack of aides per shift. At times I would hear several call buttons go off and they may go unanswered for ten to 20 minutes. I was concerned for my friend as her symptoms worsened that there wouldn't be enough nurses on staff to attend to her as well. Her family eventually hired a private caregiver in addition to having her moved to another part of the facility which provided a higher level of care. My experience overall was pleasant with Farmington Square.
