The staff at Farmington are incredibly caring, and skilled caregivers. The staff are very warm to each patient and attend to their individual needs. I have also found the staff extremely friendly, and many know me by name. The bedrooms are very good size, and are always kept clean. My wife is a “shopper” (removes clothes from other bedrooms). I believe closet doors should be installed, with a lock for staff only. It may be the timing of my visits, but I haven’t observed a real activities program. Given the population, that may be a difficult task. In summary, I have found Farmington Square to be a wonderful environment, where my wife is always happy, and well taken care of. I wouldn’t want her any other place.

I am so impressed with staff and management at Farmington Square Beaverton, particularly during the last few days while my mother was being transferred into transitional memory care. Mom has lived there for the last 5 years in the main building, and the care she received has always been pretty good, (with a few outstanding individuals). But during the move to her new building, I got to see the current group of folks that work there in action over a couple of days, and I was amazed at the patient, caring and professional manner in which they conduct themselves. I want to thank the Executive Director Tawnya for how much she helped us through what could have been a difficult transition; she was so present and participating during the move. She really knows and cares about her residents and her staff! And I have to thank their activities/transportation guy Rob, for being so great to my mom over the last years, and for just being the best! Sorry I can’t mention all the great staff over there; so many of you, thank you all!

My mother is a resident & has an affordable single room. It's clean & she has a window view of a tree that's in bloom with pink flowers. Besides her room & view, she's very comfortable & has positive things to say about her care. The staff at Farmington Square are to be commended. They're attentive & very, very kind. Staff, residents, and family members are on a first name basis. It's a small facility and has the feel of a family. I can highly recommend this facility over a larger one.

My friend has moved into Farmington Square Beaverton. It has a nice setup. There's space for her to walk and interact with the other residents. The food is good. The rooms are small and clean and have nice windows. The food was rather tasty. It could use salt, but we enjoyed it. The desserts were great and they have a good variety of menu. Everything looked great. We usually sit in the dining room to visit. They have a game room and they have hallways that make a big circle, so you don't get bored and you can just wander around. The sitting areas are nice. They have a program on a big TV screen. We did the singalong one day I was up there, and they had different stuff. It was fun. Everybody's nice. All the staff is great. They were in the process of plugging holes and they were gonna repaint some part of it. I've been up there three or four times. I didn't look at the list, but there's a lot of interaction, socializing, and things like that, and they have three different divisions of placement. All the places I visited were all about the same cost. She's doing well, and I was surprised she settled right in.

My mama moved into Farmington Square Beaverton. It's good. They have good and friendly people, good food, and good care. She has a nice room. It's not too big and not too small. It's just right for mama. They provide different activities. There was a whole schedule on the wall. With my mama, she has vision and some memory problems. She couldn't participate in a lot of it, but they provide activities to residents. It's not too expensive compared to something else. It's pretty high. On the market of assisted living, it's reasonable. It's close to where we live, so location is great. We considered the staff, friendliness, care, money, and everything. That is why we chose Farmington Square Beaverton.

What I like most about Farmington Square is that it's a smaller community. They also got memory care and assisted living. They're a little bit smaller compared to others. The staff, Sophie, was fantastic. She answered all our questions, and we had a nice tour throughout the facility. The place was old and that was kind of a turn-off. The rooms were definitely old and kind of boxy. It wasn't modernized. Their price for a studio seems to be higher compared to others too. It's probably because it's a smaller community, and they have more people to take care of the residents. The visit was great. The tour was great, and the person we got with was great. My mom was probably looking for more activity, though. From the outside, the place was fantastic. It felt homey.

My Father was in the Memory Care of Farmington Square at Beaverton. I came to visit at least one a week. I feel he was, well taken care of and safe. The staff is very friendly (to me and the residents) and the facility was kept clean and sanitary. There were lots of activities and consistent routines much of the time. I was not able to give my Father the level of care he needed and this facility gave him a wonderful place to live and met his needs with caring and capable people. I thank them very much!

When my brother and I were looking for a memory care facility that could provide a quality of life for our fathers’ journey through Alzheimer’s. We were universally in agreement that Farmington would provide our father the best chance of having a positive experience for the last bit of his journey. From transitional care to full memory care, we saw from start to finish, the caring and commitment of the Farmington team for our father’s well-being was evident and on display every single time we would stop by and spend time. The entire team loved him, and he loved them in return. It was exactly the kind of experience anyone could hope for when a parent is taking that final journey. He was treated with dignity and respect; there was never disdain or reluctance to provide him any level of care needed. And the sympathy and kindness shown as he passed meant the world to myself and the rest of our family. I wouldn’t hesitate to recommend Farmington for anyone with a family member starting or midstream in a difficult journey.

The search to find placement for my mother, who could no longer live alone or care for herself daily, was very time consuming and overwhelming. Being a nurse, I am aware of standards of practice, how a well maintained facility operates, staff to resident ratio and the impact of compassionate, professional care. From the moment I stepped into Farmington Square the very first time, it was evident to me what a positive and nurturing environment I was in. The admissions team went above and beyond my expectations with helping me throughout the entire transition process. All were and continue to be genuinely caring and concerned when it comes to any of my mother's care needs/request. Prior to her move, I witnessed a gradual decline for the past three years with my mother. In just one week living in Farmington Square, my mother thrived and I personally witnessed a much happier human being whose life was just extended. Words cannot express how I will always be grateful to all the nurturing and professional staff here. Observing the drastic positive change in my mother, no doubt I made the best decision with Farmington Square. My mother will now live a happier, enriched and full filled life here!

My uncle has been at Farmington Square several weeks now. He is now in hospice care. I could not ask for better care for him. The Farmington staff is working seemlessly with the hospice nurse. My uncle tells me frequently how lucky he is to be in such good care. I agree completely.

Farmington Square was dark and dreary, and it didn't seem to be well-kept. It seemed old and not updated at all. The executive director was extremely nice, and she provided excellent information. She was really the best thing about the place in terms of her friendliness and professionalism. They do have continuum of care, but I would not consider placing my husband there. The price was on the lower end, which I didn't realize at that time.

Our mom was a resident at Farmington for a little over two years. We visited regularly and always found the staff to be helpful, respectful of our mom and of the other residents. Mom, because of advancing dementia, went from walking and talking to being non-ambulatory and unable to talk. The staff met her needs with every change. The care provided Mom was always loving, appropriate and timely. Mom was allowed to stay in her room when she wanted to but they did try to encourage her to take her meals in the dining room and to join activities. The food was prepared on site and while it was not home cooking it was more than adequate. The staff would warm up anything we brought from home for mom to eat and stored extra's to be provided to mom like popsicles, ice cream and chocolate milk. There was never a time in the two years we worked with Farmington that we had any issues with how they treated Mom. They were extremely responsive to any questions or concerns we might have from Perla, who was our building supervisor, to the front office, to the med aids or the men and women who provided care in the building. We feel very fortunate to have had our Mom in this facility.

When my mom needed to move from independent living to assisted living (with a path to memory care), my wife and I spent a LOT of time looking at local options. We visited many facilities and got a backstage view into the entire spectrum — from pretty horrible and depressing to pretty wonderful and uplifting, and everything in between, each with a different set of care options. After much deliberation, we picked Farmington Square because the staff is so incredibly compassionate, caring and skilled. It was the right decision and, even though mom’s health took a turn for the worst almost immediately and her stay there was, sadly, relatively short, we couldn’t be happier with the support and kindness we received. The facility is nice enough, the care option ranging from assisted to full memory care and the costs are right in line with other local providers, but the people who work there — at every level, from the nursing staff to the kitchen staff — are truly wonderful. They went out of their way to welcome mom (and us) into the community and provide incredible care as she worsened. We were also struck by the long-tenured careers of some of the key staff, some of whom have been there for close to 20 years. They made a sad end more bearable and we give them the highest recommendation.

Farmington Square – Beaverton is so far, so good. It's clean and organized. They do Bingo and light exercises. The staff was very nice, and the dining room was nice and clean. The food didn't look very good, but the people were nice.

I liked Farmington Square; it was welcoming, and the staff looked professional. The people that greeted me were all there for a long time. The rooms were clean and bare, and the grounds were clean and accessible.

My mom is in Farmington Square. I find the staff to be fantastic. It is a very nice facility, and they all have been very caring. The room is adequate and clean. The food seems to be adequate also. Unfortunately, my mother is in hospice care. They are very nice in letting her in sit in some social situations, but she is not able to partake. They do a very good job in a very tough situation.

They showed me a couple of places at Farmington Square, but that's just about they all they had. They were nice, courteous, and very helpful. It's close to where I lived.

My 82 year father was at Farmington Square Beaverton for almost 3 years and we never had a problem with the staff there. They were nothing but awesome. After my father had a stroke, he was diagnosed with Lewy Body dementia. He was having very aggressive behavior and was denied by every memory care facility that we had tried to get him into. On the day before he was to be released from the rehab facility he was in, after his stroke, we contacted Farmington Square. They came out to evaluate him and said they would take him in. From that point on, my father continued to improve. They were instrumental in helping even out his meds and make sure he was as comfortable as possible. He eventually began to call Farmington Square home. We all were very happy with the care he received there. They were kind and caring to all of the residents, not just my father. When my father was in the last stages of his life, to no fault of Farmington Square, all of the staff, even the administration, were very kind and understanding. Making sure that my father, as well as the rest of my family, were comfortable. They even invited my family to join in their staff Thanksgiving dinner. I would recommend Farmington Square Beaverton to anyone. Thanks to Teddi, Rachael, Jen, Carlos, Nancy, Steven, Alex and to the rest of the staff at Farmington Square Beaverton. I wish I could give you guys more than five stars!

My husband is in Farmington Square for almost eight months now. This is a memory care facility. The top three reasons why we chose this facility is that it was close to where I live, he was able to have his own room and it was a fairly active place. There are activities for residents to do other than just watch TV or sit around all day. They have exercise, music, board games, etc. The staff there really pays attention to their residents. They provide opportunities for the residents to be engaged and get busy during the day. This is important in my opinion because it is a memory care facility. They are also very welcoming to have families involved at all times. You can visit any time you want and even have a meal there. If you want, you can also stay overnight. It is a very human approach from my point of view. The one aspect they could do a lot better is the food.

I would not recommend anyone place an elderly relative in this facility. I have overheard staff members being verbally abusive to residents. Med passers cannot seem to get medications right, and when concern about this is expressed, they shield themselves by saying they realized the mistake before they gave the medicine, and took it out of the cup. Horrible staff, horrible management. We removed our mother from this facility after only a few months of her staying there, and are currently caregiving for her at our home. After everything I witnessed in those few short months at Farmington Square, I could never trust another facility with the safety of my precious mother.

I am very pleased that my grandfather ended up in Farmington Square Beaverton assisted living center. Short his own home I don't think any place would have made the transition to a living center any easier. As my grandfather began to struggle with memory loss and dementia brought on by Alzheimer's disease, Farmington Square provided memory treatments, and visits with specialists. He remains lucid more now than before he checked in.Besides this, the staff at Farmington Square are fantastic. You often hear horror stories of elder abuse in nursing homes. It has never crossed my mind that any staff member at Farmington Square would ever treat my grandfather with anything but dignity and respect. The grounds are almost always immaculate inside and out. A very minor complaint would be that the shrubs are not trimmed as often as they could be. Activities are plentiful, and residents are encouraged to socialize and stay active. My grandfather has made friends, and has been made to feel comfortable. I would highly recommend Farmington Square to anyone, but particularly anyone who has a relative with Alzheimer's Disease.

I have been visiting care centers in my area which have an Alzheimers unit. My husband is not yet reached the stage where he needs this service; however, I am being proactive in doing my research so when the time comes I will have a plan. I am looking for these requirements: 1. a safe and caring environment for my husband; 2. a facility with good medical care, including cleanliness; 3. caregivers who are respectful of the residents, treating them with compassion and dignity 4. which, if my husband outlives the maximum of his Long Term care insurance, will accept Medicaid; 5. preferably within a reasonable commuting distance from our home so that I can make frequent visits to my husband I was given a tour of this facility; spoken a number of times with the marketing director; and met some of the staff, including the Patient Care Manager. At this point, Farmington Square appears to meet all of my criteria. I shall continue to visit this facility to monitor that it continues to meet my requirements. However, if today it was necessary for me to move my husband into an Alzheimers care unit, I would choose this facility.
