My sister was in the facility for almost 3 months, she received great care during her stay, she received therapy and was able to go home. There is no perfect facility; but overall the care was great with our nurse aide teams and therapy teams and you can tell they really care about this population.

I visited my mother and she felt very cared for. The staff are welcoming and friendly. I’ve been told it’s a new team, but I just know she has received good care and it’s clean and friendly. The food tastes good, it is a little late some days; but I know the staff care and I truly appreciate them.

My 87 year old mother was sent here after back surgery for rehab. She got no rehab as far as I was concerned because she was worse off AFTER we rescued her from there! She was walking when she went in but wasn’t when she left. When visited by her assisted living nurses they found her sitting in her own urine and she had bedsores. Another sign she never left the bed. NO ONE EVER ANSWERS THE PHONE!! And I mean NEVER! We had no way of knowing how she was doing. And one last point...they sent us a completely made up bill. They had the dates wrong and tried to charge us for next month after she left a week before!!! DO NOT ALLOW YOUR LOVED ONE TO GO HERE!!

My mother was there for rehab after back surgery . she is 87 years old. She could walk a little when she got there but according to the staff she couldn't walk. We did not know how much rehab she actually got because NO ONE ever answered the phone to give us an update. I'm not joking No one answered the damn phone. I asked several times for my mothers hearing aides to be charged but in 2 1/2 weeks they didn't do it.. She wasy always in her bed when we visted her outside her window, which tells me they just parked her there. So she ended up with bedsores. Do not send your loved ones here.

My husband is staying at Forest Hills and getting Physical Therapy and Occupational Therapy. They have helped him learn to use the tool for putting socks on. They are working to help him learn skills to do the practical things; and he is learning. They are kind to him and do not talk bad to him. I have a problem with them taking care of dressings on him. I asked for a dressing on a wound to be changed several days in a row. Finally, I provided some of the materials for the dressing change. It wasn't until I demanded they change the dressing and brought the supplies to do that- that a nurse from the facility did the dressing change. My family member was also afraid that the catheter was going to fall out or be pulled out accidentally. At the same time as the dressing change, the nurse stabilized the catheter (with supplies I provided) so it felt secure. I appreciate the help from the nurse getting this done. My husband and I would have never have been able to do those things alone. I do offer one criticism; why did I have to get ugly and forcefully demand the dressing get changed? Why could they have not done it the first or second time I asked them? Really keep an eye on your family member if you put them in this facility. Make sure the care they need is being done!

We stopped by on a Saturday looking for a long term care home for my stepfather. The place was chaotic. Smelled strongly of urine. Dirty floors. Lots of tripping hazards. We saw two TVs plugged in on the counter next to the sink - an electrocution hazard waiting to happen. I pointed it out to an employee and she didn’t acknowledge the problem. When we left a man made sure to tell us he was taking his mother out of the facility because she wasn’t getting sufficient care. He was going out to get her food because she hadn’t been fed. Seemed like too few employees on duty. Beware of this place!

I like that I can order whatever food I would like for my meals. Our physical therapy staff is gentle, patient and very caring. If you get to the point where you hurt or need to stop for a moment they listen to me. If you need anything, they will immediately get it for you.

I was sent to Forrest Hills for rehab for 3 weeks with two broken ankles. The room was nice, the staff was really friendly and the rehab staff was excellent. If I had any problems or issues whenever I spoke about them the nurses, they were quickly resolved. I felt that the staff made an effort to know me personally and visit with me often. At the same time they respected my privacy when I wasn't feeling too sociable. The physical therapists really worked to get me back on my feet.

The only thing good about this place are their physical therapy workers. When i got there i couldn't walk. I would have my light on for 25 mins or more holding it before i would finally wet my bed because they never came to respond to my light being on. They were very short staffed. There were a few aides that were helpful but for the most part they didn't care and acted like you inconvenienced them if you called for help. I was so glad when i was able to walk again and get out of there.

Do not send your loved one here. Director of Nurses and Assistant Director of Nursing care little for their patients, they have numerous directors who never return calls. The nursing staff who actually care for the patients are overwork. Call lights ring for over a hour, sometimes 2 hours Patients not bathed or changed properly. Mom had the same outfit on for 7 days. They over medicated her when she did get her medication. Dr's do not include the family in their orders. My mother fell numerous times, took them over 24 hours to get an xray. The last fall she had, her light had been on, she tried to get out of her bed and fell on to her wheelchair and whatever else was there and still her room mate had to call the desk on her cell phone to get my mother help, she was sent to the hospital, the last fall broke her spirit, she did not go back there

My daddy was sent there for rehilbilation center after having a 5 bypass they acted like they didn't know what they were doing they didn't know what he was supposed to have they didn't have doctor's orders to give him any prescriptions that he was supposed to have cuz he was in so much pain they gave him Tylenol saying that's the only thing they could give him he he died a day after being transferred over there the facility was awful it was so cold in there all the residents were complaining and they would never do anything about it we had to bring blankets from home just to help him be warm the way the way they called us and told us just awful you don't call someone like that you should always bring them in and and tell them what happened instead of calling them and telling them that was just an awful what they did I would not recommend this facility to anybody that has a loved one when we called them to ask them questions about stuff they act like they don't care they couldn't release any information that it wasn't their fault my dad was a fall risk and it was on his wrist band and when we told the lady on the phone that you was off already she said everybody has the right to fall in the state of Oklahoma but knowing that he had a wristband saying he was just known to fall you shouldn't you know maybe just had a 5 bypass done there's no way that he's going to be able to walk I just don't believe that this if it was the right place for him to be I would not recommend this to place to anybody that has a loved one or needs care awful awful place!

Poor management and I fear for my mother. The meals are awful and the facility is dirty. Tables are gritty and sticky. The kitchen help need some guidance and better hygiene. I pray that th patients are bathed regularly as promised but I see this place deteriorating rapidly. MGM needs to visit

The Doctor is less than communicative and the nursing staff does not have a clue about what is going on. They dont get their meds on time, if ever, the are left in bathrooms along, even with sever deminsia....they are not take down for meals but left to eat alone in their room...this is a sub-par facility at best!

My mother broke her hip and came here for rehab. She is 85 years young, and quite active. This accident left her frustrated and down. Since she had never been in this type situation before, we were really worried about finding someplace that would keep her spirits up, encourage her thru the rehab, and take good care of her, without the "nursing home" atmosphere. THIS was the perfect place for her. They treat her like family. Most of the staff are younger, energetic, and very good at their jobs. She loves them all and they seem to truly care about her. Plus... the food is good! Lol. We couldnt be happier about our choice.

My Dad was acutely ill, when they transferred him from the hospital to this facility, they placed him in a room at the very end of the hall with no other patients. As a RN, I saw nurses performing skills with less than "clean" procedure. My Dad was on a feeding tube and they placed him flat on his back. When my Dad got critical, the nurse said she couldn't contact his Dr. because she was in a meeting. Finally, I told her that either she called an ambulance or I would. If my Dad had not passed away, I would have moved him. Please, try to find a better facility for your family. I would give it a minus rating if I could.

This place has been horrendous for my father. He is a diabetic and they do not get his medication on time, get his food to him ontime, etc. He's been there 3 days for "skilled nursing" and has yet to have physical therapy. He hasn't even been bathed. Please do not send your loved one here. Biggest mistake ever!

My mother stayed here on several occasions for respite care and it was the worst place ever. The staff were very disorganized. We would schedule respite care through admissions, including paying in advance, and when we arrived to check her in, the nurses wouldn't know she was coming. It would take an hour of phone calls to administrative staff to get it straightened out. Then, I would have to hunt down a nurse to admit her and I would find the nurse sitting on the ledge in the dining hall texting on her phone and never looking up. She was suppose to be helping the residents with dinner. Finally, she would admit mom and promise to do things and then not follow through. We would pick mom up and she would be in the same clothes that she had on when she came there 3 days earlier. The first time she was there, the nurse called me and told me my mom was confused. I told her my mom has ALZHEIMERS and the nurse said "I know that but I just wondered if you knew she gets confused!!?? The last time she was there, Mom was sitting at the nurses station in a wheelchair and she fell out onto the floor!! After 3 attempts at respite care, this place was a nightmare, so we quit taking her there. When I complained to the Nursing Director about the lack of care, she was very defensive and extremely unprofessional. When I was leaving, a staff member who had overheard my conversation with the director, pulled me aside and told me the director was new and didn't have a clue what she was doing. He then gave me the name of a much better facility to take my mother to!

Forest Hills Care was wonderful. They did a great job with my dad. They also helped my mother get over a double knee replacement years ago and were really good with her. The rehab was just awesome; we love them. The cleanliness was excellent. I loved the staff. I really liked it.

Foresthills is the best . They took care of my grandpa. He was able to come home and remain active . The staff was caring and very attentive.

My Mother stayed at forest hills after a total hip arthroplasty and received excellent care. The facility is very clean and well kept up. They have the latest technology in PT equipment and really helped her get back to independent living. Any problem that arose was handled quickly. The nursing staff is wonderful, kind, courteous and helpful. The food they serve is actually good. If a family member ever needs rehab in the future, I will highly recommend this facility.

My mom was here and we were so happy with the care that was provided. The staff went above and beyond. They took the extra time to explain everything. PT and OT were awesome. And my mom got better and came home.

I was in Forest Hills following a long illness. I wasn't strong enough for the hospital inpatient rehab so a case worker talked me into going here. I am not being over dramatic when I say I thought I would die here. I'm a younger person and was appalled by my surroundings. The food smelled and tasted like feet. My family brought in every meal for me while I was there. The physical and occupational therapy is the only bright spot in this awful place but they only can come 45 minutes a day. That is not what I was lead to believe from the case worker or representative from this facility before going. I was there 8 days and received 1 bath. The nurses are short staffed, stressed and rude. I was eventually sent back to the hospital and placed in ICU due to infection I received at this facility. I wouldn't send my worst enemy here. It opened my eyes to whole new world and one I hope to never return to.

When we take our family to a Rehab facility we expect quality care. We did not receive it at Forest Hills of Broken Arrow. In fact, we received the worst kind of care! My mom had to take several trips to the ER during the time she was there due to their neglect of watching her causing her to fall. Also, one time she had an injury and they could not tell us what happened and had to file state papers saying she had an unknown injury. I can tell you what happened. She had a horrible fall causing the whole right side of her face, her chin and her wrist to be black and blue. She looked like someone hit her in the face. Also, there is a big problem with medication being dispensed. They are short handed especially on the weekends and the wrong medication or incomplete medication is given. One time after my mom returned there from the hospital, it was over 24 hours before she got one of her seizure medications delivered. The law requires them to have medication prescriptions filled and recorded.We will not return to Forest Hills again!

My husband has been at Forest Hills Care and Rehabilitation Center a little under two weeks. The level of care is not really quite great enough for my husband's needs, but he is getting the care that is normal and good for a skilled nursing facility. He only has 45 minutes two times a day to be in a wheelchair where he can be out of the room. The place is secure enough, but they should have more nurses.

This was my first time having to find a skilled nursing facility for a loved one. My father suffers from copd congestive heart failure and kidney disease. He was only in this facility four days. He was needing medical attention I asked for two days to get him to the hospital and they said everything was fine. He was weeping fluid from his legs and stomach. They left him in the wheel chair for 6 hours with no oxygen when he is on 3 liters 24/7. He was on insulin yet they gave him 10 packs of sugar for one cup of tea! He has now been in the hospital four days with ACUTE renal failure. I ask anyone that reads this please if you love the person that has to be placed in a nursing facility please dont let it be this one. This place doesn't deserve a rating what so ever! Not enough staffing, what they do have there is only there for a paycheck not to help these people getbetter. I forgot to mention he had a total of 3 hours of sleep from monday thru thursday and the rest of the time he was left in a wheel chair. The food is disgusting and not enough for people who need strength...The only reason I rated it a one is because it wouldn't let me post without it

It was my first experience to have to find a place for skilled nursing care for a loved one. My relationship with Forest Hills began when I chose it because the re-hab was highly reccomended and it was close to me as I would be coming on a daily basis for laundry etc...She remained at Forrest Hills from early Oct of 2012 until early January 2013 when she passed away at St. Johns. While our adopted Grandmother of over 20yrs was in their skilled nursing wing, rehab was great. However when she would push the button for help it would be forever before someone came. We had to move her on to full time nursing care due to the stroke repercussions. My biggest complaint is not enough caring care for these residents. The bathrooms are NOT made for a wheelchairs! Their bathes are in another room and she complained all the time how cold it was when she was taken for a shower. I observed multiple times when I was there after they took her to the toilet and she cleaned herself, they put her back in the chair and into her bed without washing her hands. She had one hand that worked....so toilet cleaning and eating your food with unclean hands can be disasterous! I would always wash her hands and I brought it to the attention of the desk nurse, but it continued! When she had her food brought to her room....they just sat it in front of her with no help. She was legally blind and paralyzed on one side! I was in the process of securing her another room in another facility when she suddenly got pneumonia on a Thursday...went to the hospital that night and was gone by Sunday night! I saw her on Monday and she was fine, my husband saw her Wed night and she was fine except they had hit her arm going into the bathroom and he stayed until they were able to calm her pain. I have to rate this facility to leave my review, I hope it will help you as you look for care that your loved one needs. I asked for 3wks to talk to the Dr before she passed and she never contacted us. The rooms are a nice size, toilet area is so not for handicap residents and there are too few people to care for patients! If the residents go to the dining hall for their meals, all the help goes there to get everyone fed and Ive witnessed NO ONE on the floor for those who remain in their rooms! I thought the things they offered for social time would be right up her alley, but she never wanted to go. It made me sad because this just wasnt her. I found out later that where her stroke happened, it most likely cut off her social desires! That might be a great class for those caring for these residents as they become frustrated trying to engage them when it isnt their fault at all.....it is just the residual of what a stroke can leave.

My 90 yr old father is in for rehab & IVs. This place is fairly clean but if you press the help button be prepared to wait forever. Most of the time you never see anyone in the wing. You better be able to take care of yourself in here or you'll never get a drink or anything. The only time you see a nurse is for meds. They have trouble getting food that he can eat without teeth. Unbelievable! I feel like I have to be there or he is completely ignored.

I am not impressed with Forest Hills Care and Rehabilitation Center and that's one of the reasons I started looking for another facility. They left all of her clothing in the laundry and I have to replace it. They do not communicate well with me at all and I had threatened them that I would call the health department to get anything done. As far as the aides and nurses, they are outstanding. But once you get out of her hallway it is not a pleasant experience although she does say that the food is really, really good.

From what I can understand there seemed to be quite a bit of stuff that they need to improve on. They had a lot of hospital workers there that were very nonchalant and very unprofessional. However, the last time I visited Forest Hills, it seemed like a nice enough place.

The room my mother-in-law was in was more geared to Alzheimer's and dementia. They did take care of her and she stayed in Forest Hills for almost three months. The only thing that we dislike about them was there wasn't down-home cooking like meatloaf and pot roast or vegetable and fruit. They would have a sandwich that they had to make, but everything else was pre-prepped and all they had to do was heat it up. So to me, that was kind of like a TV dinner.

Nice facility with poor staffing. Would not take anyone else here. Mother was allowed to fall twice last resulted in compression break of vertebrae.

My father was a resident. He was there for physical therapy following a stroke. He had little bowel control. He developed a severe case of what can best be described as diaper rash that became open wounds. The wounds became infected because he sat in his own filth most of the day until a family member came to visit and had him cleaned up. After just a few weeks at the place the infection had spread. He was found unconscious and his organs had begun to shut down. He was admitted to a hospital, place in intensive care and died a few days later.
