My wife has been in SEM Haven for rehab. Everyone was very friendly and cooperative. The place was very clean and did not have any smell like urine smell or anything like that. The rehab people were extremely good as far as giving my wife therapy.

My mother moved into the memory care at SEM Haven in December of 2023. The rooms and common areas are clean and spacious. The staff are super friendly and very helpful. Food appears to be decent but my Mom actually says its really good! The memory care is set up with private rooms that are large enough for residents to have their own sitting area. There is also a common living/dining/kitchen area, screened porch and a fenced yard + 2 resident cats which is nice. The set up encourages the residents to interact with each other vs just being in their rooms alone although they are able to do that too. SEM Haven memory care was highly recommended to me for my mother and I highly recommend as well!

We chose SEM Haven for my mother because it is recommended by other people locally. The staff is good, and they treat my mother very well. They're very skilled in what they're doing. The food is good. They have physical therapy. They have things to keep the residents active. The place is very clean and well taken care of. They have a gym, they have people come in, they have concerts, and they are having a family night come up where everyone can meet the staff.

My mother received horrible care throughout her time at SEM Haven. She was often ignored and neglected. At the end, she began the dying process on a weekend and was unable to get meds to relieve her pain. Neither she, her room, nor her walker and wheelchair were sufficiently cleaned. SEM Haven was where she wanted to go if ever necessary. We could not have been more disappointed. Don’t send your loved ones to SEM Haven.

My mom moved into SEM Haven. It's close to where we live, she's in memory care, and there are only six women in that area. So it's pretty secluded. It's locked in. They would have to let you in and out. The six women all eat together at the same table, all of their meals. So they kind of know each other. There's a camaraderie there. It's the same nurses every time during the week, so I like it because you kind of know who they are, they know who you are, and you can solve any questions quickly with them. It's clean. The staff is friendly, they seem to know what's going on. There are some communication issues, sometimes they can't get the person that you wanna talk to. When it comes to their facility, like the layout, the structure of the building, and the cleanliness of it.

I'm staying at SEM Haven. It's excellent care. The building is well kept and the grounds are well kept. There are nurses and aides and they all know their job. They offer meals, and the food is great. There's television, and there's rehab and things like that. My ex and my daughter come out and my other daughter, too, and we play card games and puzzles and we do walking. My room got a restroom, closets, curtains, toilet and tub, and things like that.

I was in the rehabilitated hall for care after I was in the hospital due to heart and other problems . the care and rehab I received was great. All the staff is curious . I loved the aides who took great care of me and my needs.I got to chose the food I wanted the kitchen staff is wonderful.I enjoyed my stay.

My mother-in-law, my mother and my sister have all lived and passed on from SEM Haven Nursing facility. I cannot say enough good about it. They have activities, entertainment and the facility is very clean. The staff is very caring and kind. Of course you can always find someone who is not nice but for the most part people there really care. SEM even has outing like going out to eat and even going the the Baseball Hall of Fame. They even have a yearly trip to River Downs. The Gathering Room also houses the cafe where company can eat with the residents at a very reasonable cost. They have a Chaplin who does Bible Studies and sing alongs, etc.

I was there visiting a friend of the family. To start off the whole building smells. I highly doubt they regularly clean. Especially the bathrooms. I've been there numerous times and have over heard the stna staff talking very rude to the residents. For the cost of this facility it is definitely not worth it. The food is a joke. The medical care is unacceptable. When you enter the building the lady that works at the front desk is rude and unprofessional. To make things worse they are now starting to put ankle bracelets on the residents. Which is so inhumane! The nurse must put in a code to deactivate the ankle bracelet for the resident to leave or enter the building. If no code is entered and the resident tries to enter or leave the building the doors will forcibly shut on the resident. Quite quick if I may add! This is a serious safety concern. If the doors shut on the resident it can seriously harm them or knock them over. We were not informed of this by any of the staff. The door shut on our family friend and on us. We ALL could have gotten hurt! The lady at the front desk started screaming at us because we were not informed about the ankle bracelets or the codes. It is a staffs responsibility to inform visitors about these type of things. Don't just assume everyone knows about these insane new devices. Communication is key! The woman at the front desk acts like she hates her job and takes it out on the visitors and the residents. She needs to find employment elsewhere. Don't send your loved ones here to live if you care about them. I would rate the staff and facility zero star's if it was an option.

Very family oriented with lots of personal attention. Staff tries to make everything as much like a home as possible. My grandfather was here for physical rehabilitation as well as nursing care and I couldn't have been happier with his care! And we loved seeing the resident cat!

My aunt just moved in to SEM Haven, and it seemed like a very nice place. It has everything that we wanted to have as far as qualification. The staff seems to be very efficient, knowledgeable, and very caring. The size is appropriate for my aunt's needs. I love that they have animals that live in the wing where my aunt is because she is an animal lover. It just seems like a really good place. All the residents that I have met have been really nice, warm, and wonderful people very much like my aunt. It just seems like a real nice community.
