I am/was a resident of this facility

I wouldn’t recommend this place. The staff is very rude and unresponsive. You push the call light and they respond maybe one halve hour latter.

I am a friend or relative of a current/past resident

As in any case where a loved one is going to a health care facility, there is some worry about the kinds of services that will be provided. My wife was sent there after surgery to recover and to regain lost strength. We entered the facility at 7:00 p.m. on Friday of Labor Day Weekend. We were greeted kindly by the nurse in charge. The following morning my daughter and I had the opportunity to talk at length with the aide. ALL of our questions were answered. Everything she told us about the facility and services proved to be true. They provided both physical and occupational therapy during her fourteen day stay. The staff was always cordial and willing to go out of their way to make our stay as stress free as possible.

My friend moved into O'Neill Healthcare Middleburg Heights for long-term care. Week one, he got COVID, and he called me last night from his cell phone because he's ringing his ringer, and nobody's coming to his room because he had to use the bathroom and he's not allowed to be unattended. So that's two dings on my first week. He's got a private room right now. It's OK. The lobby is nice. I heard a lot of good things about it. The room is pretty big. It's older, but for him, it's fine. It doesn't look like a bad place. They always have stuff going on. It's early for me to tell what he's going to do with it. It was a big step for him to give up his independence. The dining room is beautiful. We can visit there, to the dining room, have his birthday party, they have a beautiful, self-playing baby grand piano, big screen, and an aquarium. It's really nice, pretty, and inviting. It is overpriced.

Century Oak is a great place for loved ones in need. The staff from the Executive Director to all staff are the most caring and compassionate people in this business. If in need of a facility Century Oak must be on your list to look at. 12 on a scale of 10. This is from a family that had 2 family members at Century Oak for over 5 years.

I've been visiting this facility for over 2 years and I find the staff to be very attentive to the needs of the patients. The food isn't always the best but they try and are constantly making improvements. The new cafe menu offers a good selection if the main entree doesn't appeal to my grandma. The courtyards are calming and we enjoy sitting outside and watching the birds. The Staff is personally invested from the top down. This is a tough industry and if it wasn't for people like this, our loved ones would have no quality of life. I would recommend this facility to anyone!

I would not refer this place to any one. They are non-responsive to calls,the quality of care for my mom is minimal.her personal items are missing new clothes are missing.there are good people there. It's just sad how its under staffed 1 nurse is not enough to care for all the people there should be 2 per shift.

I would NOT RECOMMEND Century Oak to any family who is not planning on making a DAILY visit to your loved one! When you arrive daily you will need to be prepared to get to work, to take care of your loved ones needs that have not been taken care of since you last left them the day before. Expect to be treated as bothersome and difficult, Ask NO Questions in regards to your loved ones care without expecting a very defensive,disrespectful, I DONT CARE ATTITUDE response! Expect a WHAT NOW attitude from the Administrators and Social Worker, when you can not resolve any problems or care issues with the staff. EXPECT FOR THE PROBLEM NOT TO BE CORRECTED!Century Oak ADMINISTRATION defends the POOR CARE given to the patient by their staff! And lastly EXPECT that if you are TOO aggressive in your complaints and demand PROPER CARE with the threat of moving your loved one to another facility, to be told YOU ARE NO LONGER PERMITTED TO ENTER THE FACILITY, THAT IT IS PRIVATE PROPERTY AND THAT THEY WILL CONTACT THE POLICE! WOW HUH? That is what happened to my family over 16 months of MY FAMILY doing CENTURY OAKS JOB FOR THEM while they were being paid for the care of our loved one. I was not permitted to enter the facility for the last 13 days of my loved ones stay at CENTURY OAK.We have moved our loved one from CENTURY OAK and I would not recommend to ANYONE to fill that BED WITH YOUR LOVED ONE!!!!

My Mom had rehab at this facility and the therapy and nursing care were excellent. We looked at several facilities in the area and we were happy with the quick turn around to get Mom home and able to walk up her front stairs after her stroke into her home. They even followed up and visited her a week later to see how she was coping at home. i would strongly recommend this facility!

Great staff but that is about it. The food has gone down hill since he was moved there a year ago. Every day is either ground meat, pasta or I don't know what. Staff turn-over is huge, as they are expected to handle way too much. It is depressing and I would advise seeking a brighter nursing home for your loved one.

One of my parents was a resident there. I would not refer this nursing home to anyone if you want quality services for your loved one. The STNA's were PHENOMENAL, the administrative staff however were non-responsive to calls, rude, and acted as though when concerns were brought up we were over-reacting (that's how they made us feel). There was a multitude of incidents with my parent and this nursing home where quality care was absolutely bare minimal and again NOT from the STNA's but from the administrative staff, and RN's. If you have the opportunity to pursue a different nursing home then PLEASE do......

I visited this facility

Century Oak was beautiful. It had shiny floors and shiny tables. When you came in the door, there is a piano, and the people were right there to greet you. It had a homey type of atmosphere. Again, security was there. It was very nice. I'm disabled myself, and I was kind of tired so I went just outside for a while, then one of the staff members asked if they could help me and I was really impressed with that. They couldn't have been nicer. I would say that the rooms were big enough. Their dining facility was really nice. It was handicap accessible because the doors open up by themselves. I saw they were singing around the piano, and they seemed very happy. There was also a patio outside, and if they were able, they could go outside. It was really pretty. I would definitely recommend them because if I had to rehabilitate myself -- which I may have in the future -- if I could go there, that's where I would go.

My mother was at this nursing facility twice, there were occasions where she could have had better care, but with this census of residents, it was not the aides or nurses fault. I was there everyday and I had some of the food, it was good but too much. The activities were awesome, the staff was great. We moved my Mom to another home which we thought would be better but it was worse. She passed away on May 19th, 2014. We didn't receive any information on what happened and never a call to say how sorry they were. If I would have known,I would have had her stay at Century Oak, maybe my Mom would still be alive. I miss her so much, I lost my best friend.

My Grandmother at the age of 85 years old had became very ill from having breast cancer. She could no longer take care of herself and the time had came to be put into a Nursing Home. The Nursing Home my family and I had chosen was named Centry Oak. The Nursing Home was located in Middleburg Heights, Ohio. This Nursing Home was an older facility. My Grandmother recieved a warm welcome from the Nursing staff, and resident's when she had first moved into it. This facility did have an outdoor smoking area for resident's and visitor's. The facilty was kept very clean, but it did show some age because it was an older Nursing Home. There was an actual whole hallway under construction. Shortly after my Grandmother had moved into it there were some layoff's in the Nuesing Department. Because of this layoff it did leave the Nurses under staffed. Even though the resident's were very nice, the Nurses were not at time's. The nurses did'nt want to answer resident call light's and left that to the Nurses Aid's. The Nurses Aid's were very nice to my Grandmother and me. At anytime that a resident had used their call light the Nuses Aid's had answered it quickly. My Grandmother did like her stay at Centry Oak, but as a visitor I did'nt. I did file complaint's at times and the Director of Nursing did respond in a very timely fashion which was nice and important to me and my family.

My good friend enjoyed her stay at Century Oak care center. The staff was always polite and courteous to visitors, and they seemed to genuinely care about the patients. I have toured other facilities where staff was running around and seemed to have too many patients in their care to keep up with them all. The staff here was relaxed and seemed to give special attention to each patient. The facility seemed very clean every time we arrived for a visit, and there were never any strong smells present in the hallways. The dining room was also clean, and the layout of the facility was very nice and cozy feeling. We never had an issue finding parking, as there were always plenty of parking spaces available. I would not hesitate to recommend this facility to a family member who was in need of extended care because of the friendly staff and the cleanliness of the facility. The fact that this facility is located so close to Southwest General Hospital is also a great feature, as any patient whose health condition declines can be transported to the hospital quickly.

The biggest mistake we made was admitting my father into Century Oaks. While the administration came across as friendly and willing to work with us the nurses and staff were generally of poor quality. My father was ridiculed for having accidents and forced to lay in his mess for hours. This was not a single incident, but a series of reoccuring issues over his 2+ month stay. Some of the staff mocked him, shouted at him and generally belittled him. The food was also terrible and the place in general was depressing. Since he has left he is doing so much better at another local nursing home. If you enroll a loved one here, don't say you weren't warned.
