We have chosen Legacy Miamisburg Skilled Nursing & Rehabilitation for my sister. It seemed ideal for her Alzheimer's condition and the state of her disease. It was very bright and clean. Everyone was very friendly. We just felt like it would be a good fit for her. The admissions director was very helpful, very friendly, and very compassionate about our situation. We were also pleased to hear about their psychiatric presence.

Since my visit here at Heartland of Miamisburg, the nurses have been very helpful, and the therapists have consistently pushed me to get better.

Heartland of Miamisburg is great. The meals are great, and the physical therapy is great. The place is spotless and clean. Everything about the place is great, but they need more aides.

My Grandma transferred to Heartland of Miamisburg from another facility. The care she receives is top notch, and the nurses treat her with respect and dignity. They take the time to try to understand her and meet her needs. Therapy pushes her, but doesn't over do it- they know her limits. I would recommend this facility to anyone looking for a place for their loved one.

Don't go there ...it's bad mom died there step dad still there ...took weeks to get them to call for an appointment, he is passing blood . It used to be so severely understaffed.

Heartland of Miamisburg is clean. My friend is getting good care. Her only comment is that she didn’t like the food. Whenever we go visit her, she’s clean and well taken care of.

My Mom was brought here for rehab. Upon arrival she was never fed. Family members brought food up to her, but she wasn't given the food. The next day I talked to her on the phone. She complained about food to me as well. That evening she was given her meds and shortly after was unresponsive. She was sent to the hospital and pronounced dead that evening.

AWFUL! I feel like I failed my mom for putting her in this facility for rehab. My mom has been at this facility for less than a two months. For several days I noticed a change in my mom's behavior, confusion and lost of appetite and I spoke with the nurses about my concerns and nothing was done. I wanted my mom to go to the hospital, at the hospital they diagnosed her with a urinary tract infection, dehydration and malnourished. The doctor stated that she was so malnourished that they might have to put a feeding tube in her. Now how is that when my mom is in a nursing facility. I help out with my mom's care and I noticed her aide wiping her private area from back to front. No wonder she has a UTI! I had issues with my mom being in bed all day and not up. I visited my mom almost everyday, I never met or seen the Director of Nursing. Usually the DON is walking the halls checking on patients, introducing themselves to patients and family members and seeing if there's and problems or concerns that need to be addressed. My mom did not receive quality care. If I did not have my sent to the hospital to get checked out, her condition would have gotten worse. The facility looks nice in the inside but don't be fooled. The quality of care is horrible.

HORRIBLE!!!!! My grandmother was transported here after having a hip replacement surgery...a few days after she had 3 strokes. I cannot say enough bad things about the care of patients at this facility. This place should be shut down. Within one day of her arrival, she was fed NO FOOD, the techs were trying to make her walk to the bathroom on her hip that was just surgically replaced, they did not follow doctors orders and keep her medically necessary pillows on her body, they provided water that they left in the room all day until it was warm & rarely refilled. My mother started getting calls within that 24 hours from my grandmother with her thinking mom was trying to kill her by putting her in this facility. When she went to investigate, the nurse said, "She was having a lot of trouble walking to the bathroom." Mom says, "WHAT??!?!?!!? She is not allowed to put any weight on her hip! It was just fixed! That's why she is here-for recovery!!!" "Oh, I didn't know that." HOW COULD YOU NOT KNOW WHAT YOUR PATIENTS ARE IN THE FACILITY FOR??? The bells at the nurses station ding for hours (these are emergency call buttons from the patients)-I am not exaggerating. The station attendant only said to me, "I hate those things. I dream of them in my sleep" then went back to checking her cell phone rather than actually CHECKING on the patients. Then-here's another one for you-the recent tornado warning...Sirens going off all over town, NOBODY IN THE PLACE KNEW THE PROTOCOL to keep these patients safe! They spent almost 20 minutes waiting for corporate to give them "code gray" instructions. The patients were panicking, the staff was panicking, it was chaos. Would you trust your loved ones to this sort of care? I hope not. Maybe this is where you DO take your people to off them as Grandma assumed. I feel very bad that she spent any time there at all. This place needs investigated by authorities. I've had great grandparents in facilities, my mother-in-law retired from a nursing facility, and other relatives so I have experience with the level of care that should be provided. This place was the worst of all of them.
