There is poor communication amongst the nursing staff and aides and some will even tell you so. Physical therapy is a contract company whose medical records are separate from the facility nursing records. Because records are not shared the right hand doesn’t know what the left is doing. If your loved one or family member requires a lot of assistance or is unable to advocate for themselves then this is not the place to go. The food is fair. Speech Therapist is awesome! There are a few good nurses and aides who do their job well and deserve credit.

Would NOT recommend. My mother inlaw was there 3 wks for rehab. Only bathed 2 times, her bed sheet was changed 2 times once by me, she got covid there, would not clean her room , waiting 30 mi. To an hr to get anyone to assist, was brought to the attn of DON, social worker, and other staff. Nothing was done. Floor bed still not taken care of. The nurses and some of the aids were exveptional. But would not recommend.

My father passed away in April of 2019 at the Avenue. We had constant complaints of understaffing and insufficient care for his needs. The weekend of his death a change in his condition was brought to the attention of the staff on duty. Both days we were told that they would check on him. Early Monday morning my dad passed away. A copy of his medical chart from Sunday indicated that 5 of his vitals were overdue. A nursing home doesn't have to provide perfect care, but it should at least provide a standard level of care. My dad had a history of aspiration pneumonia which was always onset by a high fever. When a change in his condition was brought to the attention of the staff, at minimum, they should have checked his vitals or consulted the attending physician. They took no action to help him. This nursing home is understaffed and does not attend to the medical needs of its' patients in a timely matter which in our case was fatal for our dad. It may look nice on the outside, but the quality of care is minimal at best. Think twice before taking your loved one to this facility. I wish that we would have.

I visited my neighbor at the Avenue, and I was there before for occupational therapy. It was absolutely gorgeous, stunning, and beautiful, but their staff was overworked and didn't have much time to spend with their patients as they should. The dining area looked like it came from a magazine.

Dad went to Avenue of Medina and they were the best from all the nursing homes he's been to. They have good staff and provided good care. It's a nice place and the rooms were nice as well. It had a good room setup with lots of space. It's very convenient for us.
