I recently had a family member at Forrest Oakes for rehab and general nursing care. The room was was neat and clean. The CNA on day shift was wonderful, actually all the CNA staff were pretty attentive. The nursing staff were VERY busy but still did a wonderful job. The social worker and director of nursing 'I think' are new but both were great, knowledgeable about what was going on, and made us feel very welcome. The therapy department did an awesome job. I am seeing several bad reviews but there seems to be a lot of changes taking place in this facility for the better!

The nurse Donna is very rude to family seems to not care about the patients care. Donna complains about having to make multiple visits to patients room even though patient is not responsive and monitors are alarming. The level of care and courtesy that is normally provide to the family is non-existent.

If I could give this place negative stars, I would. My mom was placed in FO for rehab. She got no rehab, and was left alone in her bed. I drove from out of state to see her and was appalled when I found her in the fetal position alone in a room where there was blood on the wall next to her bed and on the floor. There were face masks on the floor beside her bed, along with other medical trash, and the area around the toilet was disgusting. No cleaning was being done, much less increased cleaning because of COVID. She was dehydrated because no one was helping her eat or drink. The clothes she went in with were gone, and clothes that weren't hers were in their place. Her shirt hadn't been washed, as there was blood all over the sleeve. It had been thrown in the floor of her closet. The staff were not in any hurry to help when asked, even when there were 6 people at the desk. I have photos and, if given the option, I will post them. DO NOT place the care of your loved one in this nursing home. Had we not gotten to her and insisted on her being moved out immediately, she would have died, without question.

My mother had to participate in therapy service at Forrest Oakes Healthcare during her stay her Rehab service was top-notch and very professional. The /nurses/nursing assistant was awesome with my mother they treated her has if she was their parent which made it easy for me to rest at night. The facility was a little dated but the care was Awesome and the facility was very clean, If I have to ever think about placing her long-term care or short-term she will definitely be back at Forrest Oakes. Thanks Forrest Oakes Staff.

I REALLY ENJOYED MY STAY AT FORREST OAKES HEALTHCARE. THE STAFF WAS VERY VERY NICE AND CARING MADE ME FEEL REALLY WELCOME. IF I EVER HAVE TOMGO TO REHABILATION AGAIN I WOULD CHOOSE FORREST OAKES THEY ARE REALLY ENCOURAGING STAFF OUT THERE. THEY JUST SPOILED ME AND SHOWED ME HOW MUCH THEY CARE FOR PEOPLE. THANKS AGAIN, MARY WILIIAMS

I visited my grandmother on numerous occasions. Her wedding rings were stolen. We were paying to have her clothes laundered since I lived out of state. More than once I found them stinking so I had to gather them all and bring them to a laundromat. My grandmother had dentures and I found her toothbrush stuck in a drawer dusty and dirty. Just to walk in the place you would think this is a nice place. Not so. Outward appearances can be deceiving. I am posting this so other family members are aware that all is not as it seems. Hopefully things have improved in the time since my grandmother had to be there.

I visited this place and it was excellent. The staff was very welcoming and accommodating. They were very nice. It was a wonderful place. The facility was very clean. It didn't have a smell. The lighting there was very good. It's a very bright and cheerful community. The residents seem happy, seem nice, and they seem to be at-home there.
