My mother who has dementia and is legally blind has been a resident at Daleview Care Center for 4 years. I can honestly say, I'm very pleased with the facility and care my mom receives. I visit her daily and always find her clean and well taken care of. Any questions or concerns I may have are quickly resolved. The best part are the amazing people that work there!

One of the worst experiences ever!!!! One staff member aide to take care of an entire floor of rehab patients!!! Rachel the social worker claims she is HELPING you but gets nothing accomplished. Handed a nurse a bag of newly purchased clothes for him to be labeled and never saw them again. Purchased more. Same. Left him for days with NO PANTS ON. Told me they had a wheelchair ordered that was his so I ordered a walker. NOPE On his release just the walker. (The guy cant walk but a few steps). Never sent my insurance a pre authorization for equipment I needed because I live in South Carolina so today he is coming home with nothing, no aide, no nurse nothing. Had to buy a hospital bed on Craigs list. The other patients are just sitting in a room with nothing to do, or alone in the dark. One day we got there and my husband was laying down in bed trying to eat his food. The only good thing about the place is I am getting him the h... out of there!!!!!!

My Mother-in-law was in for treatment for sores on her legs and needed and IV antibiotic drip. She was given wrong meds, not enough oxygen, never saw a doctor or dietician (she was a diabetic), was denied the call button because she asked to urinate to much while on Lasix! The hall bathroom had dried feces on the floors and walls. I could go on and on but the end result she was there for one week and no she is deceased under suspicious circumstances.

Neglect, Filthy, no phones... doctors do not follow up. The Rehabilitation center on Fulton Ave in Farmingdale is what I'm referring to. If your loved one needs rehab, please do not choose Daleview...my grandma is traumatized.

Complimentary review for the PHYSICAL THERAPY STAFF, especially JERRY, LISA, and SHARON. Well planned and well coordinated activities leading up to restored strength. Also credos to Vanette and Debbie of the Nurses Aides. Both caring and empathetic. The Medical Nurse Mariana goes above and beyond her duties. Crystal the Social Worker was in constant communication with the family. TashanaLee the Nurse Coordinator was excellent as well. And lastly, Vinny the Dietitian was obliging and diversified the food menu.

My father was in Daleview from mid January 2020 until the end of February, about 7 weeks. He was there for "more intensive PT" post hospital. My mother was not offered a rehab stay since he was discharged from the hospital on 12/21/19 and I am assuming everyone was on a holiday break. He was in the hospital 5 days and was not allowed out of bed the entire time. This resulted in his inability to ambulate safely. My mother pursued placement and she was sent to Daleview. The staff was ok but the stay ended up doing more harm than good. He received PT 6 days a week for about an hour but the rest of the day he was left to sit in a wheelchair ( he was not in a wheelchair when he went in) The residents were not permitted to move out the facility without a staff member. He would have at the very least wheeled himself around the hallways. The rooms all had bathrooms but the residents are not permitted to use them, they have to be taken to the one in the hallway. Instead of making him more independent he became incontinent ( they put him in a diaper) and were not encouraged to use the bathroom.The staff especially nurses were not very friendly and they were snarky when approached with a question. His hearing aide went missing the first week and the social worker said my mom could put in a claim to be reimbursed ( this never happened) Hearing aides are extremely expensive and it is crucial to his ability to understand what is going on. They stopped putting his other one in and then they assumed he was confused when in fact he could not hear them. He is a very mild mannered man and never wants to cause trouble. My mom and brother visited every day and I was there twice ( I live in PA and work in long term care for over 25 years). I definitely would not recommend a place like this for a situation like his. It was a disappointment and things have been sliding downhill since. The only positive was that he was not stuck there during quarantine. Food was adequate ( I work in food service and it was standard. Place was clean, no smell ( a plus) Activities were not individualized and residents were left in the dining room to be supervised at all times. I understand the facility is trying to keep their "no falls" rating but if you do not let residents move around I guess they can not fall!! PT staff was friendly but it was disappointing that the extent of the therapy was 1 hour a day and then you sit supervised for the rest of the day!

Relative resident for a year was neglected, transferred four times, his last private room was near all their soiled linens bins, garbage bin , etc. left opened ,very health hazard. When inspection comes, all bins moved , after a day or two back same place. The resident, left for about a week under the assurance by the management and care takers, he will be feed, given shower, change clothes and kept clean n comfortable.The resident has no underlying disease or illness, he just had no mobility due to unsuccessful hip replacement.He all of a sudden had fever, chills, looked frail and pain in his whole body, attending doctor prescribed antibiotics and fluids, 3-4 days still high fever, taken to ER for infection, admitted for about a week, back to Daleview, after about a week or less, still with temperature , chills,etc. , back to ER...Dr.said he got bad pneumonia after x-rays, tests.......all the symptoms of Covid-19 , he was admitted, hospitalized , given ventilator, high oxygen, the respiratory dr.said he is very sick, the attending physician said he was serious, the pulmonary dr., the infectious dr. the palliative dr., the speech pathologist dr,the cardiologist , about 5-6 different doctors with their respective specialties all said the same explanation similar to Covid-19, from the Emergency Room to isolation in ICU , PCU, SDU , finally , his family was informed their treatment was ineffective, multi-antibiotics didn’t work, there is no more they could do , the palliative dr. said , there is no way they could allow him to be out of the hospital and the family must make a decision be in hospice or comfort care room in same hospital because of respiratory failure. He was only for a day in the comfort care room he passed away without the ventilator and oxygen.

Take Daleview this place off your list if you care about your loved one. Incompetent staff and bad care for loved ones. I would not put my pet in this place let alone a loved one.

Do not consider this nursing home or rehab for any family members. The nursing team is understaffed and inexperienced. They are constantly rotating nursing and aides in and out. If you ask any medical questions for your loved ones they do not know the answers. You will never see an on and off shift meeting to get caught up on the patients current status. Lacking of notes and supervisors don’t know. Patients sit for hours half dressed in the hallway ignored or in their room soiled. They do not shower or groom patients appropriately. Due to lack of attention, training and cleanliness my loved one has had multiple trips to the hospital and sores. The Health Department needs to start making unscheduled visits and look in to the deterioration of the residents. Also look into some of the doctors associated with the facility if they are observing this behavior and quality of the patients health and are not addressing.

My friend is being treated inhumanly. There is no dignity, compassion,kindness shown. She has been left lying in urine all night and told by the nursing assistant she "pees too much" and she cant clean her because there is only 1 towel per patient. This cruel person even told her the day staff would clean her and put a clean gown over the dirty for her to eat her breakfast. She is able to go in the bathroom, but cannot get help at night. The assistant even put her wheelchair in the hall so she couldnt try to get up. This treatment was not one night. It has been night after night. My friend asked to see the nurse and was repeatedly told " shes busy". At this point she is afraid of retaliation so she wont tell nurse when she is in room for meds. My friend is there for temporary rehab!! Physical therapy to bridge surgery and going home. Agter 2 weeks she has given up. This person is not demented, her brain works fine. Talking with admin,social service and advocates is a waste of time. Please do not put your loved ones in this inhumane place!!

My dad is at Daleview now for rehab for about a week so far , and the difference I see in him is incredible. I cant say enough about the hard working kind staff , who have all been so attentive to him, I am at such ease knowing he is being rendered such great care. Without hesitation I would strongly recommend this rehab to all.

I had a hard time talking with people at Daleview and can’t get much of the information I need. They are checking her speech and her swallowing, so I am happy about that, but I am getting a lot of conflicting reports that she has pneumonia or bronchitis or that she has nothing and her lungs are clear, so I don’t like that. There are a few things I like about them and a lot of things I don’t like about them. They were a little more diligent in taking care of her the second time around. The first time, they did not have a speech therapist. She was there for 5 days and they didn’t have anybody the first time, but they had somebody the second time. So I like the fact that they were diligent with that. They noticed she had a bed sore where she had it in the hospital but no one found it in the hospital, but they noticed it at Daleview. They are very busy at the nursing station, so whenever you ask questions, they seem too busy to answer you. They need more one-on-one attention for the patients. They are so busy that they are not taking care of the patients enough; the patients call to get their diapers changed and are left to sit in their dirty diapers for an hour. They don’t talk to the healthcare practice about what they are doing, and if you try to call, they seem too busy to talk to you.

My father was recently at Daleview Care Center. The care was excellent. The ultimate goal was to make him stronger and have him discharged to home, which was achieved. Although not the intense medical care and treatment as a hospital for a rehab facility it did what was needed. The staff were concerned and friendly. The food was good but not the same as at home. I would highly recommend this facility.

My father in law needed rehab to prepare for chemotherapy for lung cancer. They treated him like anyone else with a sports injury. He wet himself numours times because they took their time taking him to the bathroom. They wouldn't let him go to the bathroom until he completed his therapy which led to him wetting himself again. They gave him medicine to limit his urination...he was here for a week before he started sluring his speech and talking nonsense, we thought he was having a stroke but they assured us it was just the medication. That day he started having trouble breathing but assured us he would be fine. My mother in law demands they take him to the hospital, they told us they would just take him back but after demanding he finally was taken to the hospital. He died two days later...I don't know why they where so incompetent but maybe they weren't prepared for someone with his illness.

My mother is in right now for rehab at the Daleview in Farmingdale. Her sister was there before. The facility was clean and friendly when we first visited. She hasn't been there long -- only about a week -- but the staff has been good. I go there every day, and most of the time there seems to be a recreational therapist on hand. They appear to be there all the time, so they seem to be attentive to her. The food seems good, and there is always food for them to eat. There are always snacks going around in the afternoon so that's good. The shared room she's in is good and clean.

My 88 father is currently in the Dale View nursing home. He is there for rehab from a hospital stay for a severe leg infection. The home is not large and all patients seem to get the required attention from the staff. My Dad is hard of hearing and legally blind he is also very demanding and set in his ways. He has not had many complaints about his stay so far. He has complained that they bring him breakfast while hes still in bed without his teeth. We have explained that they are on a schedule and do not serve meals based on your demands. He accually understood their position. He has also complained that one of the aids yells at him.Every Monday the families are given an opportunity to discuss thier family members condition with senor staff members.Over all I believe he is getting very good care at this nursing home.

My uncle stayed at Daleview Nursing Home in Farmingdale NY. I visited him pretty often so I spent a lot of time here. First off it is two separate buildings so some patients are in one building and some in the others. The landscaping and buildings have been renovated and it is beautiful. Inside it is an average looking nursing home. My mother and grandmother both worked here in the past 40 years so I am pretty familiar with the facility. Daleview is a basic nursing home with a not so great staff. A lot of the times they rather spend their time chatting or gossiping about other members of the staff but I guess that is how it works in these types of facilities. The amenities are pretty basic and average. They have some activities for the patients but not that much. My uncle got bed sores quite often because of the nonchalant staff members. The operators are always pleasant. Overall this is an average Nursing Home with nothing special. There are a lot better nursing homes than this one but if you are a local and want to be close to your relative then I guess it will suit your needs.

During a summer month in 2010 the heat caused the air conditioning to fail on the first day of my mom's 21 days of rehab. At first we couldn't get answers from any of the nursing staff as to the estimated time for repairs; "that's not my job," is the paraphrase of the answers. Of what was about 60 rooms on my 83 year-old mom's rehab side there were just 6 portable air conditioners - and those weren't working half of the time. And of those 6 there was just one (instead of the capacity of 2) people in each of those rooms. Finally we got both the real timeline and a working portable air conditioner from the director of admissions. A caregiver should be able to get both straight answers and the right kind of help from the immediate staff. Empowering them with both abilities makes the time pleasant for everyone. Hopefully, things have changed now at this facility.
