Forest Manor was very spacious, they had a lock down unit, and they also had in long-term care. They had a very large physical therapy area and a nice outside area. The atmosphere was not quite as homey; it’s sterile, but it still met the needs of the person we need to find a place for.

A relative of mine was at this facility for about 3 years. Due to the lack of help, the care is sub par. My aunt was put into a diaper even though she was able to care for herself. Aides do not respond when you ring the bell for assistance and there is no communication between staff and family members. Food is terrible, bed area is not sanitary and there were little or no social activities. High turnover of staff too. My aunt passed with little or no communication from management so we couldn't say our last good byes.

I WOULD NOT SEND MY WORST ENEMY TO THIS FACILITY! It is dirty and smells like urine all the time. Cleaning is not thorough and there is NO hand sanitizer anywhere!!! Floors, fixtures and bathrooms are old, dirty and outdated. Residents are left in bed, shower schedules are not followed due to lack of staffing. My family member has been there for a month and the care is terrible. Staffing is too low, and the food is terrible! There are activities but if you think that handing out donuts is something to do, well the management is mistaken. My family member is bored to tears! Church services on Sundays are cancelled due to lack of staffing? Unacceptable!!! The management is a joke! They yes you to death but nothing gets done. We are currently looking for another facility and filing a formal complaint with the state for lack of senior care.

When we first put my mother in Forest Manor, it was much better. However, over 2 1/2 years that she lived there it went downhill, especially with this new administrator. It is a very clean, bright facility, with good recreation for the patients, but they are unable to keep good staff. The turn over rate is terrible and alot of good nurses and aides have left or got fired!! The food is horrible and they do have a dietician, but I am not quite sure of her job responsibilities?! That's right....she designs a healthy looking nutritious meal plan to show Medicare and Medicaid, which most of the people do not eat!! They have NO Low Salt, Diabetic, Low Cholesterol diets....I was told "this is not a hospital" from the kitchen manager!! They can't even get a piece of toast cause it would be cold by the time they got it.....well solve the problem?!! So they would rather spend the money on diabetic meds than give a nutritious diabetic meal?!! They serve pasta almost every day and desserts with every meal, even to the diabetics!! The food is tasteless!! The kitchen is always short staffed and the mother and daughter in the kitchen on evenings.are not nice to deal with......they will give a cool hamburger on a tea plate with nothing else and when asked to reheat it....I heard them say "she's a pain in my ass"!! Plus they cater only to a few women who are with it! The daughter came out to a patient who asked for a cheesebuger and she put down a cheese hamburger pasta dish and said this is your cheeseburger, now eat it....very nasty! I cannot believe how they treat these elderly people, If that was there parent, would they treat them like that?

Forest Manor is not of nice quality. He didn’t like it at all at the beginning, but they have a new chef, and the food is better now.

What a great experience we had at F.M. I can't believe the reviews I saw on here, I experienced none of this. Great place, friendly and clean. Highly recommended.
I was impressed!

What a beautiful facility located off the beaten path. Beautiful scenery, gorgeous courtyard, you just feel so at peace because of the natural surroundings. Definitely a great place to live or rehab. Staff was great, friendly and their was plenty of activities going on. Food was typical, different choices.

This facility should be shut down. The care is horrendeous. I went here for PT and OT after having an infection and being treated in a hospital, Call lights are not answered for 30-40 mins. After asking an aide t tell the nurse that I needed pain medication I would wait 1 to 2 hours for the nurse to bring it to me.I also waited over 1 hr t be assisted to the bathroom. This occured daily both daytime and evenings. The food is awful and after asking t speak with John from dietary about my special diet needs he finally showed up to discuss it 3 days later(I asked to see him daily). There is a lack of communication between staff members which highly impacts patient care. The social worker did not come to see me for over a week after my arrival and at that time I was given my patients bill of rights. I was also left soak and wet for 3 nights in a row. I ended up calling another facilities social worker and was tranferred to that facility after spending 8 days at this hellhole. PLEASE do not place a friend or loved one here. They will not be cared for.The only reason I gave the facility one star is because their are a few staff members who do a good job and are caring especially a day CNA named Connie

Forest Manor Healthcare Center is an excellent facility. They specializes in long-term dementia and Alzheimer's. They also have a wing for rehabilitation if a person had fallen. The facility was really, really nice. The staff were very attentive, they make sure the patients are cared for. The place is clean, organized, there was enough staff available to take care of the patients. They have a wing dedicated for dementia where people are allowed to walk around. But if they went any further, alarms would go off. And then another wing for severe Alzheimer's patients that was securely locked down so they couldn't get out and wander. They just spent millions of dollars on a renovation. So it's fairly new.

my mother was a resident at Forest Manor. The building is gorgeous, but the care was so poor, we took he out quickly. Nobody cares when we made requests to correct issues. Can u imagine what goes on when family is not around? We r sooo happy we left Forest manor

I believe this assisted living provider is one of the best available today, simply due to the staff alone. They truly, honestly care for those who are staying with them. It is clean, well kept, and very smoothly run. There's not many bad things I can say about Forest Manor. Some of the ameneties seem a little 'stingy'- old beds, small TV's, but nothing out of the ordinary for a nursing home. Very friendly staff and always helpful in even the worst situations. Would definitely recommend them!

My Father Died At This Facility. The Facility is plagued with problems and not recommended by me to anyone who has a family member in need of specialized health care. Communication between family members and staff/attending doctors is terrible. My request for a hospice assessment for my Father was road blocked over a week prior to the day he passed away (you need a specific doctor's order). Leading up to this, my Father told me that he was too weak and could not tolerate physical therapy any longer; however the staff continued on pushing him and ridiculed him that he was just "giving up".
Housekeeping and nutritional services are also lacking. Odor of sewage is evident throughout from a faulty septic. Therefore, the facility in my opinion should be closed.
