My mom stayed in Nazareth Living Center 2. It was fine. They did a fine job taking care of my mom, but unfortunately she had a fall while she was there, and she fell hard into something. That's what she passed away from. I'm not accusing anyone and I don't intend to. I've never said anything to them. Anybody can fall anywhere, but they're really good about keeping in touch with me. Nothing special and I think my mom was safe there. I'm sure a lot of homes, or maybe all of them, find it tough to keep staff, and I don't think Nancy was any different. I think they do a good job of what they have to work with. The rooms are clean and I have no complaints about that. My mom was fine with what she saw, but then she was 90 years old. There's nothing I can complain about. I wish somebody would have been more aware of how bad my mom's fall was. I'm not putting this on anyone. It's just one of those things that I wish they would have checked on her more often. The people that I saw there were always friendly and my mom wasn't the easiest person to deal with but I saw she did a great job of taking care of her.

My sister spent 3 years in assisted living at carondelet manor part of Nazareth living center. She absolutely disliked it. She felt isolated and ignored. The food was the only halfway acceptable thing. The financial manager charged me two hundred dollars a day for nothing after she passed. I had to clear out her stuff very quickly because of that. They wouldn’t tell me the charges until I had finished. They said “look at the contract”. No help . No grace period. The housing manager even had the nerve to show the apartment while I was waiting for the furniture movers to come. Nazareth wasted no time to move another person in. Nazareth is just a business not a home. 9/15/2024

The rooms are old with beat up furniture. Skilled nursing care was not skilled, nursing or even care. My Mom was left in the same clothes 3 days in a row even sleeping in them. She lost 28 pounds in the 4 months she was there and no one seemed to care. She fell and laid on the floor for several hours before being found. They are not equipped to handle memory care patients in the skilled nursing area. Lots of promises were made, not acted upon. Very disappointing experience; it was not like this before COVID.

Staff was helpful during normal business hours. Evenings and weekends were terrible. Staff including supervisor and director handled a call from former resident who had my mother's room regarding bed bugs in a very unprofessional manner, treated my mother like it was her fault that the previous resident possibly had bed bugs. Was unable to visit my mom for 2 1/2 days due to work schedule. When I arrived, I found her half sitting in the chair in just a shirt with altered mental status and temp of 101. Her nurse didn't even want to come to the room to see her and when she did, she acted shocked to see my mom in that condition but instead of offering to help she called a CNA to the room to get vitals on her. I requested a call for ambulance and was told that they couldn't call until the supervisor was notified. I called 911 myself, still not nurse in the room to assess the situation. Staff would not give EMS a current medication sheet. When the supervisor arrived she was more concerned with having me sign out AMA instead of being concerned for the person that the facility was supposed to be providing care for. I would not recommend this facility to anyone.

Room was great, there are various sizes. Staff was helpful and efficient. My friend appreciated regular activities and Mass on campus. I know a lot of places are struggling, but I really feel my friend is safe, loved and well cared for.

The staff was NOT helpful! Dad was in this facility for less than 24 hours and I had to remove him. He went in for Rehabilitation for an amputation. He fell and laid in the floor for 3 hours; no one checked on him and he couldn't reach his call light. They did not give him his medication. He hit his call light and it took almost 3 hours and me showing up for it was answered! The wound care nurse, the nurse practioner nor the house doctor seen him upon his arrival. The room had a hole in the wall, no TV remote and no internet.

Our experience with Nazareth Living Center was not good. We ended up taking my father out from there right away. They were so understaffed. People would wait for hours to go to the bathroom, and there were 28 residents at one time for 2 nurse's aides. At that point, there was one nurse's aide. They would drop the food off when my dad was sick and just leave it there while he was sleeping. So, he wouldn't get fed. The actual people, especially the receptionists and several people were very wonderful and kind. They just needed to hire more people. They're overworked, and I think that's almost every nursing home these days. So, the residents suffer.

Nazareth Living Center was very nice. It was clean and the people were all very friendly. We saw a lot of the buildings and this was the one where there's a washer and dryer in your apartment. You get a certain number of points a month that would give you 30 dinners a month or you could choose to use them for lunch, then they have a complimentary breakfast. They have a buy-in system, but it's reasonable, and I was very happy with that place. They have snack areas around where you could pick up drinks, bagels, or muffins. It's available throughout the day and you just help yourself kind of thing. They had a very large number of opportunities for you to be active. They have a big sit-down room where they do exercises and an area where there were exercise machines if you wanted to do that. They had plenty of extra things, which is important to me, like community areas, not just the lunchroom, and at the end of each hall, there was some area where you could get together with people with several tables and chairs, and they had a screened-in area that was kind of like a deck type. The outside was beautiful and it was just really a pleasant atmosphere.

My father and sister have both been to Nazareth. I cannot thank them enough for their compassion and care. Tiannia in admissions went over and above in helping us deal with the financial end and Cathy the social worker went over and above in her care and attention to empathetic detail. My family members enjoy being part of a strong, Christian Community. Grateful.

My loved one is dead, not because of COVID which he did contract but “overcame.” He was neglected and mistreated. The doctor’s orders for an infected toe were not consistently followed and he lost his toes. They allowed him to become severely dehydrated and did not make sure he even touched his food before taking it away. He was only a resident for two weeks before I was no longer allowed to see him. From what he told me no one checked on him except to give him his medicine. There were times when he wasn’t given any water to drink. When I inquired about him on Easter Sunday night because he hadn’t had anything to eat the nurse told me she had too many senile patients who were acting out and since he never complained she didn’t have time for him. I could go on but my heart breaks at how he was treated. The place isn’t cheap. It used to be good but not now.

My father recently died. He spent his last 7 months at Nazareth. Even though it was difficult to visit him every day as he declined, I always felt a sense of joy when there because of the staff: aides, nurses, doctors, spiritual crew, maintenance, laundry, dietary, gift shop staff, front desk, activity/music therapists, physical therapists, housekeeping, management. Three special angles (they know who they are) are especially dear to me because they helped me to at least see & talk to my dad just before he died. Covid-19 made that chore difficult, but they took the time to be there for Dad & me. I can't forget to mention the social workers. They always made me feel as if I was the only client at that moment. Both of them were professional & efficient, yet compassionate; sometimes taking time to deliver cookies or read cards to Dad after the pandemic lock down was enacted March 12, 2020. Oh, and the Sisters of St. Joseph too. They were always around - each one of them, a blessing. I will miss seeing all of you every day and pray for you as you continue to work long hours in the extremely difficult conditions these day.

Nazareth Living Center is wonderful. They're so caring and they do terrific rehab exercises. The food is tremendous and the workers are great. They have mass every day and the nuns are so kind to you. It's great.

My mother is in Nazareth Living Center. This place has been absolutely wonderful to my mother. They are very caring. Their therapy group talks to the assisted living place and finds out what it is my mother needs to do. They are working very hard with her to make sure she is able to do the things she needs to be able to do. The food is fabulous. Most of the people are wonderful. When she went in there, she was very sick from breaking a hip and getting pneumonia and couldn't do much of everything. They worked with her, and she has done such a turnaround. That is absolutely amazing for a 91-year old woman. It has a homey atmosphere.

Nazareth Living Center had residents in wheelchairs and sweat clothes sitting in the halls, slumped over and asleep. The place didn’t smell very good. The rooms were like hospital rooms, not individual rooms. It had a very nice entryway and everything looked nice, but once you go in, the place was not updated. It had a nursing home feel to it and had a very Catholic environment with a lot of nuns.

Nazareth Living Center was one of the finest that we found. For what my sister and brother-in-law need, it is probably the best. It is certainly an excellent program because it has the whole continuum of care, but my sister and brother-in-law won’t need continuum care on-site because they are very close to the dialysis center, which is separate from it. The place was very comfortable and very safe. It is all on one level and there was a second level for those who do not have any problems with steps. It was wheelchair accessible, including the bathrooms, and they had other services, like breakfast and dinner every day. They had a theater, a chapel, and everything else. It certainly fit all the needs of people who have a variety of requirements.

My mother had a small stay(2 weeks) in Nazareth in December 2016 for a rehabilitation for a broken rib.. I can not express enough that she will never go there again, other then a couple of nice aids and PT, the staff was awful, and the care for my mother was awful. I only missed one day out of 14 to go see my mom and when I came in the next day her room smelled like urin, it was in disarray and she was a mess, she said when ever she would hit her call button no one would come and if they did come she would tell them what she would need and they would never come back, I personally heard other patient's hitting their drinking glasses with their silverware to get the attention of the staff. My first complaint was to the CEO and he passed my info on to a manager who came up with every excuse to let me know why things were happening, short staff mainly, but a 82 year old women with a broken rib doesn't care if your facility is short staff, she was in pain and she needed help and nurses acted like she was a nuisance. My next attempt, after a couple of days and things not changing, I talked with a social worker (recommend by her PCP) who was less then helpful.. I would express what was going on and she some how would turn it around on my mother, and raised her voice to me. By this time I have given up on the staff taking care of my mother the way she should've been and when I came in I would make sure she had her dinner, fresh water, I would help her use the restroom and empty her bedside commode, put her into bed and made sure she had everything she needed because I knew when I left no one was going to come in to see if shes was ok for awhile. 20,23,25,30 minute call button waits, which they told us was way to long but it never stopped, they put my mother in Depends so they didn't have to come in as often, she has never were Depends up until this point. After her 14 day stay she was discharged and I took her home where she then went through a 5 day detox from the medication they were giving her in their facility. My mother is 82 and weighs 125 lbs. They had her on 2 different pain killers twice a day, a pain patch and 2 muscle relaxers a day. She couldn't function in their facility(fell twice) and then they sent her home to deal with their aftermath. Thank goodness she had family to get her through those awful 5 days, I strongly suggest if you do put a family member in this facility you watch them like a hawk and make sure you know what is going on with them at all times. I will NEVER place another member of my family in the care of this terrible facility, Trust me when I say looks are deceiving when it comes to Nazareth living center.

I’m going into the Nazareth Living Center. They have one part that has already been finished, and that’s what I toured. The part I’m going to move into is under construction right now. It was pleasant and well-lit, and the halls were wide with lots of good lighting. The personnel just could not have been more helpful, and there are a number of nuns that live there that I ran into the hallways, and they just could not have been more delightful. The facilities are first class. They have an exercise room, a library, a computer room, daily masses, a nice dining facility, and a garage. You have an indoor place to park. It has everything that I think I would want. One of things I particularly like is the full-service kitchen, and it also has a washer and dryer in the unit.

I thought the staff was top notch at Nazareth. They have done a lot of great improvements over the years, as this is an old retirement home for Catholic sisters that later incorporated the general public. The grounds are beautiful and well maintained. I thought that there were a lot of activities as well. Parking was not very good and parts of the buildings are still very old.

My grandmother was admitted to Nazareth Living Center about four years ago. She could not function because of her Alzheimer disease. She never knew where she was, but this facility treated her like a human being. Not like some nursing homes that tend to treat their residents like animals. They loved on her, treated her like she was their grandmother. Also the staff is very friendly to the family of the resident. They know that is hard to see your loved one get old and not be able to function. Being kind a warm to my family is what has set this facility apart from the others. No one wants to put their loved one in a nursing home unless they must. Being able to come to this facility and just sit with my grandmother and spend time with her, for about two years while she slowly died meant the world to me. I would recommend this facility to anyone and everyone who has a loved one who needs extra care.

"My husband's grandmother lives in this facility, and we've been very impressed with the care she's been receiving. The staff is friendly, her daily needs are met in a timely and high-quality manner.It was a very difficult decision for the family to move her into a nursing home. Nazareth made that transition easier. We feel confident that we've chosen the right home for her and would recommend this facility to other families."

I recently toured this facility because I was visiting my friend who works there. I had visited previously while I was in elementary school because I participated in various contests for children that were held by the facility. The staff are extremely friendly and courteous to their visitors as well as to members of the facility. I enjoy speaking with older people, for I find their life stories to be very intriguing. If I had to choose an assisted living facility for myself or a family member, I would Nazareth. The building is fairly new, and I was impressed by the layout and style of it. The dining area is very clean and hosted a variety of meal choices. Each week, the center holds a variety of activities, including children from local grade schools coming to play bingo, aerobic and yoga classes, or even ice cream socials. I would highly recommend this establishment for anyone who is looking for an assisted living facility.
