My stepmom was at Oak Knoll Healthcare Center for rehab. It was kind of a little dated, but it's nice. The staff is nice. It's clean. She actually was friendly with the staff, and they were all sad to see her go. They would joke around with her and everything, so she really appreciated that. She liked the food. She said it was really good. Her room was nice. It's almost like a hospital where you have a roommate sometimes, but for the bulk of it she didn't have a roommate, which is actually good. Her side of the room was nice because it had a pretty window. Overall, she was pretty comfortable there. Their place was good for rehab.

Nice facility but terrible care! Actually no care at all except for medication passing to make sure charting is done for state review or JACHO visit. The department of public health should be held accountable for turning lights out with all the complaints they receive regarding this facility. If you don’t have the staffing close this so called medical facility. So many infections from UTI’s, bed sores that they always have an excuse to why they occurred. It’s never their fault!!!!!!!! Shame on you!!!!!!!! Target someone else except the most vulnerable that cannot defend for themselves .

Rooms cluttered, Roommate has Covid so we are worried dad will acquire. I feel care is very bad. Dried feces means not since 2-3 hrs ago , it means it’s not changed since 7-8+ hours. I was told they don’t have enough staff.

I visited this facility

Our mom just stayed at this facility. After I found double soiled diapers on her as did my sister twice, I asked to speak to Director in charge of her care several times to no avail. Our mom did not use any diapers prior and doesn’t use them now that she is out of there. The nurses said they don’t know who care director is because she is new and they never met her. She was not there when we looked for her, messages left but calls not returned. We were glad to be out of there. It was nice building but awful care and no one in charge.

My mom went there to get stronger after a hospital stay. The place looks beautiful but that's where it ends. I never heard from them, I had to call to set up a meeting to discuss her care - it never actually happened because I took her out of there after a little over a week. They put her by herself in the last room at the end of the hall. No one ever came near her room when I was there. She said she went hours w/o seeing anyone. They were supposed to get her up to walk and hardly did, In a week I could see that she had lost weight - she was not eating. They would drop her tray off and pick it untouched and no on noticed. She had a choking/ swallowing episode one night while I was there and realized something was wrong. I don't know what would have happened if I hadn't been there. We got an ambulance and got her out of there. I refused to let her go back. We found a much smaller facility, not fancy at all that had the best and most caring staff. It just shows you that can can't judge a book by it's cover.

I would not send my sick dog there, Oak Knoll is completely unable to attend or care for anybody, that needs physical help. They are the most unprofessional and uncaring group of individuals that I have ever encountered. They all think that they are superior and they make no secret of it. The head nurse is completely unqualified along with the other nurses on the units. There's no compassion or respect to the residents or their families. The rooms are filthy, toilets are caked with feces and all the furniture is broken. I was promised many positive conditions when I placed my family member there 3 weeks ago and not one was met, my family member is now dead. Do not place your family, friend or sick dog into OAK KNOLL!!!

I visited this facility

Very nice place to drive up to and equally appealing on the inside. One of my Aunts was there, and she was comfortable. It was a short stay and we weren’t sure what to expect as this was our first time at a Nursing Home but it went well.

My dad went to oak knoll after having pneumonia. The facility was clean and the staff were amazing! Very kind and caring. Great facility. Wish I could give Oak knoll 10 stars!

I am a friend or relative of a current/past resident

As an RN I know it is of primary importance to move patients, and wherever possible get them up and moving. This was not done. When I questioned a member of staff about it I was told, in effect, it was too difficult for them and they were afraid of hurting themselves, as the patient was no longer able to stand. Prior to admission, she was living independently. I recommended to her family she be removed from the facility. Unfortunately in the time it took to arrange the now necessary care, her condition rapidly deteriorated. I should also mention that her inability to stand was due to a procedure done by the facility. There may be areas of the facility that are better staffed with better outcomes, but long term care is NOT one of them. If you must leave a family member here, please visit daily, and be prepared to provide care for them. GET THEM OUT OF BED!

The staff is very dismissive and cold. Ma NY act like you are bothering them when asking them to do something for residents. They often act as if they will accommodate you but you find out later it was all for show. Food is often cold. More importantly dietary needs are ignored. Unless residents can physically get themselves around they are often left in their rooms all day. For example if one needs oxygen tanks to move or such it seems to be too much trouble to arrange that . Never see the dining area in use at dinner time lunch only. Maybe that is where the hot food is. Chart s seem to be incomplete in some cases. Residents told certain amenities are free when they are not. Two of my friends, who are registered nurses of many years, had parents here and removed them elsewhere. This used to be a good place but not any longer. It may look pretty but it is not good.

I would never recommend Oak Knoll to anyone ever! The care is poor and with a few exceptions the staff are uncaring and impersonal. They always seem to be understaffed and many new people coming and going. Many employees don't work for Oak Knoll but come through an agency which explains why they are not invested in the facility or the patients. If you have a relative who can not advocate for themselves they will most likely be ignored. Concerns are only addressed when they become an emergency. They do not have a doctor on staff daily. If something comes up on the weekend it is not addressed until Monday or Tuesday. The doctor rarely sees patients, even when they are sick. The nutritionist is also usually MIA, always "in another building". Rehab is minimal, only 1 hour per day of you're lucky. The facility may look nice at first glance but first impressions are not accurate.

My dad was sent to Oak Knoll for rehab after a hospital stay. In his heart, after a short time there, he knew that he needed to stay there, and not return home. He has many different physical ailments, but has all his faculties, and can certainly speak his mind. He will express his opinion loud and clear, and is assertive enough to get the care he needs. The staff are wonderful, and communicate well with the families. They are very tolerant of my stubborn dad, and treat him well, despite his issues. I feel like I can communicate with them like family. Then enters mom. Sent here too from a hospital for rehab. She suffers from dementia, but is very cooperative and easy going, with dad by her side, of course. They have been married for 64 years, and they are living out their years together in a place as close to home as I could expect. Not ideal, of course, but a clean, comfortable, environment that smells good, where they seem happy. Sure, there are issues with staff being overwhelmed and over worked. Goes along with the industry. One needs to understand this is going to happen. But they are pleasant and accommodating any way. As long as my parents are happy, I am too. I feel no hesitation in speaking to any of the caregivers about anything I have a concern about. The administration team is great, the business office very helpful, and the reception staff is outstanding. Thank you for giving my parents a home where they feel safe and cared for.

I visited this facility

Oak Knoll is a wonderful place. My grandmother receives excellent care there. Today was an exception when we got there her oxygen level was extremely low . When I was leaving I asked the nurse Danielle if she could take my grandmother's oxygen level once again before I left and she said " No I will not take it with you here because you are making me anxious and you're making your grandmother anxious"....... I had to leave the nursing home to find out my grandmother's oxygen level. I am not only upset I am worried for my grandmother and the care she will receive tonight. Oak Knoll is a wonderful place, this issue I had to tonight was something that I assume never happens, overall this is a wonderful place!

Everything is always great there. First and foremost the facility is very clean and well maintained. The facilities and grounds are nicely treated and create a pleasant viewing experience. Once past the looks the staff is always smiling and courteous. They are all very warm hearted and will spend the extra time to talk things over with you.
