Great care and rehab program at Meadow Park. They took very good care of my loved one. The rooms were being renovated when she was there and also the rehab gym. I was able to speak to her FaceTime during covid which was great since no visitors.

Deserves a 0. DO NOT EVER PUT YOUR LOVED ONE IN the Meadow Park Rehabilitation Center. Upon arrival I was put in a gown and they insisted I wear a diaper. I struggled terribly with them over wearing a diaper, but they absolutely insisted. I found out the reason for the diaper is that they will not take you to the bathroom. I asked for them to put ice on my knee. They said they DID NOT have ice bags. I was there for 4 days and received 15 minutes of physical therapy. No nurse or attendant came to check on me throughout the weekend. During the 4 days I was there, I asked to wash myself, but was only offered to wash myself one time, and only got a fresh gown once. On the 4th day, I had had enough and figured this was not on the up-and-up. The nurse, case worker and social worker came to say I could not leave without doctor approval, and I said “bet me”. My husband and I gathered my things and left the facility.

KEEP AFTER THE STAFF! Be sure to make regular visits so you know the patient is being treated with utmost care and courtesy by the staff. Especially if the patient can't advocate for themselves. Check for bedsores and look at their feet periodically. If you see anything that’s cause for concern, tell someone and follow up until you know it’s treated. My family and I did all of this, and I still feel like my fathers’ care was not handled as a priority. Communication and following through was a constant frustration with this place. If your loved one is at risk of falling out of bed, BE ADVISED! This place does NOT have bed-rails. Instead, they put the patient into a “scoop mattress” and lower the bed down and put floor mats down on either side. If the patient falls out of bed, they will lie there until a staff member finds them. My father had fallen or crawled out of bed a few times, and was injured once in the process. He had a huge scab on his knee that was left untreated and unbandage which eventually got raw and inflamed, causing him pain and discomfort. I’ve read several reviews of people falling out of bed in this place. You’d think by now they’d invest in a few beds with bed-rails. On 08/22/2022, my 67 y/o father was sent here after a moderate stroke that left him paralyzed on his entire left side of his body, unable to sit up or walk, feed himself, or do basic self-care without the help of staff. The stroke also left him with major cognitive impairment, which made him confused and agitated at times. His case required ‘round the clock care with PT, OT, and speech therapy 5 days a week, 75 minutes a day total. Immediately after arriving, his health started to decline rapidly, and by the end of his 13 week stay, he had lost appx 30+ lbs. He was alarmingly thin, especially for a patient who we’re told has been eating well and being fed by the assistance of nurse techs/staff. There have been several visits where my family or I would show up and he would be sitting in a soiled brief, and never any cups of water within reach for him to try and drink from. He was constantly dehydrated (as told to us by the doctors who seen him at 3 different hospitals after arriving to Meadow Park.) His call button was rarely within reach and always flung behind his bed, or hanging over his side table. He would not be able to call a nurse tech if he needed anything or there was an emergency. During our first care-plan meeting on 09/14/22, we requested my father be seen by their in house podiatrist as his feet were in so much pain, you couldn’t touch them to put socks on. We were told this would be done, however; it never happened. On our second care-plan meeting on 10/24, we brought up the podiatrist again, and the team mentioned they would make an appointment for him to be seen by the podiatrist and also by a psyche doctor to see if he was a candidate for anti-depressants. He was FINALLY seen by a podiatrist on 11/11. That’s TWO MONTHS after we originally requested this appointment be made. Unfortunately, he was never seen by a psyche doctor for anti-depressants which I think would have helped him a lot. By this time my dads feet had gotten so bad, that when he was sent to the ER on 11/27 for aspiration pneumonia and dehydration, the doctor told us he’d need to have his foot amputated as he had lost total circulation in his right foot. Mind you, the side that was NOT left paralyzed by the stroke. Sadly, he ended up going into hospice two days after admission and passing on 12/05, so the surgery never happened. If my father’s health hadn’t taken a dive due to other complications near the end, and had his foot been seen by a doctor the moment we requested it, his foot could have been saved. It saddens me that this is where my father spent the last 4 months of his life on this earth. It was heart breaking for us to see him deteriorate and wither away here, and I can’t fathom what my father went through during these long weeks.

Interaction with some of the staff was the worst I have ever seen. They took a very long time to answer when patient pushed button. When my hubby was told he was positive for Covid, but was asymptomatic, he was moved to the first floor. While there they never checked his blood pressure, even tho he is on Rx. Meds. they never did a finger stick for Diabetes, even tho he is Type 2 Diabetic. Physical therapist Kathy & Monica are great. Nurse/ Nurses Aid Latanya was the best

So my mom became a resident at this location early 2020. Due to Covid we have not been able to visit her. You would think that the staff would understand. These people do not care about your love ones. They don't return calls, they don't answer the phone. When you meet them in person it's a different story. PLEASE DO NOT TAKE YOUR LOVE ONE TO THIS PLACE. It's TERRIBLE, STAFF NOT FRIENDLY, DIRTY PLACE. My mom is not doing well there at all. I'm waiting for that call , that's how bad it is. Unfortunately I can't care for her on my own. I just pray everyday, god help her !!

If I could give this person 0 stars I would, but the system won't allow it. My family is currently there and its not good at all. They staff is none responsive. I have yet to speak with anyone at the nursing station where I have been transferred a number of times. The nutrition department does not really meet the needs of a dialysis patient even though they provide dialysis treatment. My family has to wait way too long for assistance and sometimes does not ever get what she needs. Do not use this facility.

Case Worker and additional stay . My husband was a resident there from 11/13 to 12/16/19. After reading all financial papers given my husband and I knew he would be leaving on 12/13. We let that be known to Case Workers. However, on 11/13/19 he was given papers and told he could not leave until 12/16/19. We had no idea that those additional days were not covered by Medicare and why the assessment appointment was not set up before 11/13/19 after many inquiries by my husband And I . After finally getting discharged after the additional days stay on weekend he was released 12/16/19. We received a bill for 3 days copay. From financial office. I think that was unnecessary additional stay and to be charged for it we could have taken him home that Friday. The inconvenience was not worth additional money your facilities is charging.

My husband has been at the rehab since mid July and the progress he has made and continues to make with extremely excellent physical therapy is absolutely amazing. The nurses and staff are wonderful and the food is beyond good. Also I have witnessed how the staff "works together" for the benefit of the patients. From the first smile and greeting at the reception desk to everyone on staff, there is always a welcome and a very upbeat feeling. We are beyond pleased.

Run away!!! My mother was there over 10 days and had to scream to get a shower. She was there a total of 3 weeks and they never changed her sheets. My mom was afraid to speak up for fear they would kick her out or treat her even worse. I tried calling and could never reach anyone or ever get a call back. They are under staffed and only do the very minimal to get their payment from the insurance companies.

I will give this facility a four thumbs up for cleanliness. The care you get is a one thumb up. My relative did not get a ID bracelet until 24 hours after arrival. The pharmacy did not give all the medications when due. One medication after four days of arriving was still not available. Only one cup of water per shift is given no more unless you ask.. They don't check on the patient sometimes for over 3 hours. Vitals is not taken in a timely manner, results are written on pieces of paper or recorded in their heads, So of course the medication are not given in a timely manner. I know this to be true because I have been with the patient and observed these things. Please don't boast about how you treat your patients because it is just not true..

I couldn’t see anything wrong with ManorCare. Everything seemed nice, well run, and well kept. I liked the friendliness of the staff.

They were a great help with understanding the type of care my grandmother would need. When we took a tour of the facility everyone was extremely helpful and they had great living conditions. While they provide a comprehensive and affordable plan I only had one small problem with it, if you need more assistance you need to hire a nurse to stay there over night with her. The nurse practioners where extremely competent with medical knowledge, but because of the language barrier it was extremely hard to understand a few of the nurses and that agitated my grandmother a great deal. Otherwise I'd recommend Manor Care to anyone, but that language barrier was a downfall.

My mother was at Woodbridge Valley to recuperate after an emergency hospital stay. Problems started as soon as we checked her in. After luring us to the nursing home with the promise of a private room, my mother was placed in a shared room because no other beds were available (so, we were lied to). She was in severe pain (bad enough that her doctor had her on oxycontin), but the nursing home didn't have anything stronger than tylenol available and because it was the end of the day, wouldn't get her anything stronger until the next morning. Throughout her stay, she was given her pain medication on an erratic schedule, leaving her very uncomfortable. The food was poor. Her roommate needed help with the bathroom, but no aides would come to help her and she often had accidents. The worst was that my mother got to the point she couldn't take it any more -- so much pain, no sleep, no food -- that we told the nursing home we wanted to leave. They refused to let her go! I signed her out against medical orders, and they refused to remove her PIC line so I had to take her to the emergency room to get that done. The whole time I was signing her out, the administrator was threatening not to submit the bill for my mother's stay to Medicare so that I would have to pay it all. Needless to say, we have not gone back. The one bright spot was the physical therapy department - everyone there was caring, patient, knowledgeable, and pleasant.
