Do not send anyone here. Since Elevated North took over the place has gone down hill. The only CNA that was helpful was Liliana! The administration team is awful. Didn't return phone calls when issues would occur. The staff was very unprofessional and didn't follow protocol. Too many issues but with it being one of the only rehab centers with a dialysis center we didn't have much of a choice to put our loved one there.

My sister was in a nursing home at Elevate Care North Branch. Initially, the experience was OK, but then she fell just before she was supposed to leave, and that wasn't a good experience. She fell at night, and there was no nurse. Somebody did pick her up, but there was no nurse to assist or anything. She had to go into the hospital because she broke her arm. The staff members tried to be helpful. The facility appeared to be clean, because I was only allowed on the first floor. Everybody had to wear masks, and they didn't want anybody that wasn't associated with the facility to go upstairs to the patient's room. They just had physical therapy, that was it.

The facilities are clean, well lit, and friendly, with large rooms. The nursing staff are patient with difficult patients, although paging nurses can yield slow results. Deals heavily with Alzheimer and dementia patients. They claim that Wifi is officially provided, but actually isn't, due to malfunction. Single rooms are available, and all rooms are large and comfortable, with television in all of them.

This is a fairly large facility that provides short term rehab and long term care to mostly geriatric residents. There are historically usually a few non-senior adult disabled residents as well. The building sits right on the main road, and it is sometimes hard to find a parking spot in the lot. There is a small designated patio/gazebo area for residents and families to use, that fills up quickly on nice days. There is a forested area with a walking path behind the building, but it is not part of the facility property and it is not paved. The common areas area always neat and clean, and have modest and appropriate furnishings. Much of the facility has been remodeled/redecorated in the past few years, providing improved aesthetics and more up to date fixtures and finishes. Rooms are of average size and most are shared. Windows are large but light varies depending on where a given room is located. Storage in the rooms is improved in the remodeled areas, but remains a bit limited as in most facilities. The food is average, and sometimes does not appear appetizing. Usually there is enough staff to readily meet resident needs without any extensive wait times for help, but sometimes it gets busy and little things seem to get overlooked. Residents who can advocate for themselves, or have frequent family visitors, will have the best results. Staff is typically responsive and kind, though at times they get shuffled around and thus be totally unfamiliar with the residents in the area they are assigned to work on a given day.
