We brought my dad here a few months ago for rehab, and I have only good things to say about this facility and the people that work here. Most of the staff was fantastic and kept me informed of everything that was happening with my dad at the facility. I also have to commend the physical therapy department, the dietician who worked so hard to personalize meals for my dad, the nurses, and everyone who helped to provide such wonderful care for my father. COVID protocols are in place to protect the residents and the staff, and they truly care about the well being of their residents. I would recommend Warren Barr South Loop to anyone looking or needing short term or long term care for their loved one.

Lobby is beautiful and deceiving. The rooms are dark and uninviting. The care to details is alarming. A nurse was attempting to give my mother another patients medications. Had a sibling not been present this could have been disastrous. Food was not placed within reach of a bed ridden patient. Delivered and picked up without being touched. The roommate had to tell us why mom wasn't eating. She left the facility with a bed sore and horrible rash, all within the first 10 days. The last straw was maintenance finding her on the floor, having fallen out the bed and being rushed to a hospital ER. We went for Rehab and got a total nightmare!! The only good thing I can say is the physical therapy team are very professional and some of the nurses and CNA's were caring. But many were Not employees but temporaries assigned from outside agencies. Please, take your loved one elsewhere!!

My husband came home smelling like urine staff was rude and told my husband he didn’t belong there. The social worker was very rude. Would not send my enemy there.

My grandmother was at Warren Barr South Loop (St Agnes Hc and Rehab Ctr). They tried to keep her not fall. They did all the minimum, but not additional attention to her. They had the place completely remodeled, so they had a very nice lobby now on each floor. It looked nice, but the individual rooms looked really dark and smelly. They only did the renovations on the lobby on each floor, but the residents' rooms were still the same. The assistants were pretty good, but the staff's attention to the residents definitely needs to be improved. They also should have more activities for the residents.

Right now, my mom is in Warren Barr South Loop. She was there to begin with for rehabilitation. The level of care is excellent. The therapist has been wonderful in working with her, so it has been helping a lot. That is why we decided to keep her there. They just work very well with her. The only thing I have an issue with is their food. We have to constantly check the food and what my mom is eating, but other than that, everything else has been fantastic. Every day, they have something going on where they can go play Bingo, or do arts and crafts. Every day, there is an activity. Like I said, the care has been phenomenal.

I'm familiar with St. Agnes. The rooms were clean and decent. The dining room was clean.
