Hi Mom is 90 year old and a resident of elevate care of Irving Park I'm very very pleased with the care she is receiving. Since Elevate came on board, I have witnessed change for the better in terms of the staff, their work ethic. And maintaince of the facility. I hope things continues to improve. I give high shout outs to the owners, staff, and all who are involved in caring for my mom and the other residents. Thank you from the bottom of my heart for the care and love shown to my mother.

We toured Irving Park Living and Rehab Center. When I came in, I was warmly received, and they gave me their attention as soon as I got in there. I did get a chance to tour the place. As I came in, I could see that even at the different desks they had on the different floors, those receptionists were warm and receiving. They took the time to speak and say hello. I noticed that the residents there seemed to be very comfortable and relaxed. They seemed to have a good rapport with the employees there as they walked through the halls. It's small and it's tight, but the purpose of us coming there was to find a place that was going to be able to meet my brother's needs. And right now, he needs physical therapy. I like the fact that they offer therapy, so that'll help him strengthen his extremity. And I just kind of felt like it was a warm atmosphere. It wasn't mechanical. The staff was able to answer any questions that we had, and she stayed on a professional level. I liked her professionalism. I liked her warmth, and she seemed to be very caring. She seemed to be interested in knowing what we wanted. I liked the fact that there was no odor and it was clean. The floors didn't have stains or anything on them. And the people themselves were fresh, wearing decent clothes, and they didn't have an odor.

High turn over in staff. They do not wear name tags. CNA is always on a break or at lunch. Or they are short staff. Some staff are harsh with the patient. Residents have their property taken away by staff or other residents. Most of the time a person does not understand the staff. No bed bound activities. No consist therapy. Patient was transfer to another facility and is standing with a walker after one week. The food is terrible. Alot of time the food given is cold. Same menu over and over. Must of the resident's meal slip does not match what is given to them. If possible stay away
