My 93 year old neighbor was here with a broken hip. No one answered the phone most of the time. They gave me a direct line to call but no one ever answered that either. Turned out it wasn't the correct number and I have no idea where it went. Severly understaffed. Call lights on for hours. They'd leave her wound uncovered for hours resulting in an infection that landed her back in the hospital. AND she got SCABIES there that went undiagnosed by every "medical professional " who saw her there! I think that was intentionally because then they'd have to admit liability. She's home now but still dealing with the repercussions of her stay there.

For being a "REHABILITATION" center there is very little therapy. My mother broke her hip and has been in the facility for 9 days and just finally received her 1st physical therapy session yesterday after I had to call and complain. Her clothes that we were required to bring for her have come up missing and now she is wearing someone elses clothing. Every time that I have visited, I have had to change the day and date on the board (and I go in in the afternoon to evening). This has just been a very expensive hotel instead of rehab.

Sunset Lake Health and Rehabilitation Center seemed to be short-staffed. They didn't seem to get the patients up and moving. They seemed to leave them in their wheelchairs, and that's their way of kind of watching them and preventing them from falling. Overall, it was clean though and the staff was very kind. The food was terrible. They served not very appetizing food. My mom didn't want to eat it most of the time and I was with her when she was eating her meals. There were activities, but she did not participate, although we tried to get her to participate. They had bingo and a cookie social kind of thing, but she has dementia, so she's not in the place to be able to really do that stuff. What they need to do is they need to train their weekend and night staff better. Mom was not well taken care of on the weekends.

Did not have the appropriate medications, kept changing medications without consulting the family or the primary doctor, will not answer the phone and have no voicemail to leave messages. Cannnot reach staff when needed - won’t talk to family or the primary doctor. Very reluctant to give medical records and will not fax all the pertinent information to the next facility that will take care of my mom. Attending doctor told my mom not to drink water after looking at her swollen feet - my mom does not drink water unless I force her - only coffee. Nursing staff is overwhelmed, drug distribution system very outdated and burdensome to staff. Staff does try to help and be present but are inhibited by the general recordkeeping methods and general understaffing. Covid went thru the facility while my mom was there and she was given paxlovid even tho her primary doc recommended against it! Stay far far away from this place. There were some excellent staff, particularly Bernie and Nurse Nancy.

To give this facility even one star is over generous. My father recently had a brief stay at this facility and we were completely appalled by the irresponsibility, disregard, and unprofessionalism of the staff as well as the unsanitary/filthy conditions of the facility environment. Placing any loved one in this facility will be one of the biggest mistakes you make. Some of the many issues we encountered: -Staffing: Even supervisors in nursing, housekeeping, and maintenance were complaining about how limited and short staffed they were. -Room: The physical condition of the room upon transfer to a private room due to inappropriate initial admission placement, was appalling and that of complete filth. There were feces on the walls and furniture, dried blood on the bedside commode, and roaches in the bathroom -Nursing: Majority were incompetent at best. Upon admission no medication’s were present. Medications needed to be provided from home for the first day(s). Throughout the 10 day stay, medication‘s were given in error (for example 2 opioids given at the same time, leaving patient completely snowed). Supervisor stated she had spoken to staff after the second incident. Narcotics were left bedside, and my father was unsupervised with taking medication’s. He also had to wait 2 to 4 hours for pain meds. Several of the nurses and staff we encountered were rude, dismissive, and were observed to disregarded patient questions and concerns. We had to file several grievances which were submitted. -Wait time:On numerous occasions, call bell wait time was 1 to 2 hours for any staff to enter the room and respond. Majority of staff would simply walk past the flashing light without any regard during this timeframe. -Food:Standard menu with no ability to choose meals. My father did receive a piece of paper with “specials” however, we attempted to order the special at least 4 different times with no success or acknowledgment. We even left notes on his tray and alerted staff that this was not what he had ordered. Nothing changed. -Maintenance- The TV in my father’s room was not functioning and the remote control and was stuck on one channel with the power on for >2 days without the ability to turn it off. Upon admission we had requested bedrails due to need for assistance with functional transfers. It took 3 days for the actual bedrails to be Installed impacting safety and independence. -Therapy: admissions had stated that my father would receive 2 hours of therapy a day. He was lucky if he received (1) 35 minute co-treatment. In the one session that I did get to observe, the PTA came into the room and observed one sit to stand transfer then proceeded to let him know that she had to do a progress report and would use the time to do that. That was his only therapy for the day totaling approximately 5 minutes. -Discharge planning : We are the ones who made all equipment recommendations to social work as therapy did not assume this role as is typical standard practice in rehab facilities. Social worker did order the requested equipment but never contacted family to see if they wanted to self pay if insurance did not cover. Stay away!

Apparent limited staff caused my mother to be placed in bed at 5 on Fridays and left in bed until Sundays after breaking her pelvis. After developing ability to walk to the bathroom from her bed with a walker, she was sent home without home health. She was told to get home health on her own. It took two weeks to see a nurse practitioner who referred her to Home health and after two weeks, no one has called. She has deteriorated from independently living, managing weeding and watering her yard, to being unable to walk more than a few feet and requiring assistance. This has been a poor outcome with poor hosp setting and support from Sunset Lakes Rehabilitation.

My sister was just released from Sunset Lake Health And Rehabilitation Center. It was a very nice community, but I think they missed a few things. She met her goals with being in rehab. It was fine, and she's doing well. She didn't object to it, and she didn't think it was terrible, but they released her with a catheter still in, and I don't think that was right. I don't think they followed through with whatever things they should follow through with. I have actually no experience with care facilities, so I don't know if that's a common thing, but to me, it was not acceptable. The communication was good, though. The staff was nice, and they were good at getting back to me to answer all the questions that I had at the time. She was trying to sell her home, and it involved a lot more paperwork and favors than it normally would have, like asking them to take papers down, have her sign them, and pass them back to us -- that kind of thing -- and they were all very cooperative.

The facility is clean for the most part! The staff is overworked and short staffed. The administrator Steven could care less! He walks around and says he’s working on improvements but I have seen nothing thus far! The place is noisy ! Their are little to no activities here! The weekends are like ghost town ! The therapy dept. is nice, The nursing director Sue visits every pt. Everyday and cares about what we have to say! She answers questions honestly and cares about us and the pts. I called her several times on the weekend when I needed something ! The dietary person is nice but could try different food items but I am sure she is limited on what she can buy. This place needs more staff, better activities and a administrator that cares about us rather then himself ! I hope this place turns around because I just see more problems !

I am/was a resident of this facility

I was a resident after a total knee replacement. I am a retired nurse and from the first day I was absolutely disappointed to know I had made a big mistake in agreeing that I woukd need rehab after surgery. My Physician recommended the place as he "rounds" there on Monday. I was made to think this was a Rehabilitation Center, it is only licensed as a "Nursing Home" with Physical Therapy. Nothing was discussed that I would be required to have either the MD or NHP associated with the facility be responsible for my care. My Physicians were never contacted regarding my care. My blood sugar spiked after surgery, no one discussed with me the fact I had not previously been diagnosed as Diabetic. My physician ordered a Sliding scale insulin. Then the MD associated with the facility decided to discontinue that order, without discussion with my physician or surgeon or me for that matter. This MD who had no past medical or history of my health needs. As in a previous review, this facility does not have any kind of Diabetic Diet, only levels of the same foods for every patient. Lient carbohydrate was the diet ordered for me. There is no Dietician or Nutritionist on staff, no trained kitchen staff. When I did discussed with the Kitchen Manager, the need for protein and more fresh food, I finally got some fresh fruit. But it was the same at every meal. Non-compliant with my request, nothing changed with my other meals. I would get what I requested (sometimes). The food was all pre-made packaged food, canned fruit, no type of balanced meals. The PT Director was told to take my case because I was complaining about everything. She was assigned to do my therapy twice daily. Her services were the only piece of my care that I can rate excellent. The CNAs as a whole, working there, have only the basics of basics for caring for patients. Some do not speak English well enough to communicate and cannot understand enough English to comply with the smallest request. None of the CNAs know how to verify a patients diet tray to even allow salt and pepper, ice for ice tea, coffee with cream or sweetener. You were lucky if your tray was delivered to you with some warmth in the foods supposed to be warm. None of the food were ever served cold, like chicken salad, or chef salad. Below poor would be the rating for food service. In leaving I was told I would get a piece of equipment delivered to my home. The wrong order was placed. I was told I would have home health the day after discharge, of course the HH company stated get did not get a discharge dare from the facility. I would not recommend this facility for anyone, especially someone who cannot speak for themselves. If the patient has family and can help with nourishments and diet. But if there is no one around to watch out for the patient, no way should they be admitted here.

Extremely disappointed with this facility. Not all things that shine are diamonds or gold. They are more focussed on creating "a resort setting" and not as focussed on quality of care. Was told family member was not responding to therapy after only 5 days and was told she would be d/c'd to home even though she was not independent to be home alone. When questioned regarding this unsafe discharge plan, her status magically improved and refused to reconsider her d/c plan even though they were informed patient lived alone, had no one to assist her and was unable to care for herself. They botched up getting home care services in place. The finishing touch was that although they did not offer pet therapy, they seemed to have implemented such a program ...2 scabies infestations during my relative's brief and disappointing stay. If your loved one is in need of quality therapy and care I would SKIP this place...

The facility and staff are top notch. This was the brightest, cleanest, more cheerful facility I have ever visited. All of the employees were smiling, pleasant and seemed to genuinely care about the residents. My issue with this facility was with the medical care and dietary requirements. My mother is diabetic and her blood sugars were out of control the entire time she was at this facility. She never saw the Doctor, and saw the nurse practitioner only once over a four week stay. Because family members were thousands of miles away, it was difficult to stay on top of this. There are only two meal choices at each meal and they are all carb heavy. While they maintain that they use a carb controlled diet, the meals my mom was getting were not appropriate for someone who is diabetic. When I mentioned my concern two weeks into her stay, a sliding scale was instituted, but the diet was keeping the medication from doing much good. I flew in for a quick visit with my mom (including buying lunch one day) and requested a care plan meeting. No dietary changes were made and I was informed that when my mother went back home to her usual foods and schedule, her blood sugars would normalize. I found this response to be unacceptable.

Sunset Lake in Venice was excellent, bright and cheery, very clean, and very well lit. The residents seemed to be very comfortable and happy there. The staff was outstanding, very caring, and professional. We could not have asked for a better place for my brother to spend his last days. They did a wonderful job caring for him.

My mother-in-law had ended up back in the hospital. Sunset Lake Health and Rehabilitation Center provides high level nursing care. They're taking good care of her. It's been a good experience. We've been happy with the care she receives. The building is very clean and the bathrooms are well kept. It doesn't have any smell or odor. The staff is very caring and they have expressed interest in her past. They try to engage her a little bit more in activities. They're interested in their patients and we've been happy with that.

When my husband needs skilled nursing care, Sunset Lake Health and Rehabilitation Center was the only one that I felt I really want to keep him in. It is close to our home. He has been there about a month now and he has seemed well adjusted. He has a large room there. Just before he moved in, they did a total refurbishment of their entire building inside. Everything is freshly painted. All is new floors and walls. Everything is all smooth and even, shiny and pretty. They also re-did their garden and it is beautiful. What I like best about this facility is that all of their staff, whatever their position, are always smiling. This is what I like best about this facility. There are also a lot of different activities for people to participate in. They do cook-outs and they have different types of musical concerts almost every week. They also have church services there. A lot of their residents are all smiles being there.

My mom was on rehabilitation at Sunset Lake Village because she fell and broke her hip. The rehabilitation center was separate from the assisted living facility. The place was clean and nice. They took very good care of her. The staff were nice and I always had a good experience with them.

I visited this facility

The first thing I noticed about this place was that there was a very adequate staff presence in all areas of the building. I found it very difficult to escape the staff actually. This is something that struck me as uncommon. And all of the staff seemed happy and well fed; which makes me think they are well paid. Which makes me think the place is expensive. I think of that as quite a compliment though. If a place looks like it cost a lot to keep it up, then it is probably doing something right. All of the guests here seemed comfortable and as content as they could possibly be. If I ever need to be in a nursing or rehab situation, then this is the facility I will choose. (provided that i can afford it). = )
