My dad moved from Freedom Village in Bradenton to Heritage Park in Bradenton for rehab. During his stay, which was a total of about two weeks, the nursing and therapy staff were very attentive to my dad's needs. Angelina was amazing! She was very knowledgeable, compassionate and caring, especially the day that my dad passed away. The staff constantly kept me updated about my dad's condition. The room was simple, but very clean. Unfortunately, my dad was too weak to participate in activities. While visiting, I did notice many residents participating in various creative group activities. I always felt welcome and appreciated when visiting my dad and the staff was readily available to offer any assistance.

My wife is at Heritage Park Care & Rehabilitation Center. It is right across the street from where we live. We chose that because of its location, and we didn't have to travel far. My wife has a private room and it's nice. It doesn't quite measure up to the other place. We're having a little problem with them now because yesterday she was asleep, and we couldn't wake her up. I find a little bit of fault with the fact that they didn't check on her enough to know where she was. The nurses weren't quite checking on her like I would like them to, so we had to take her back to the emergency room because we were unable to wake her at that time, and I wanted to get her checked out. We tasted the food and it was good. Some of the staff are good and some of them aren't. It seems like some of them care and some of them don't. A couple of nurses don't seem like they care. They are only there because it's a job. I'm not too thrilled right now with the staff because they didn't check on my wife close enough to catch her being in the state that she was in. My wife's room is nice, and the upkeep is good.

Terrible! Condescending and rude staff. Little to no care or help. Neglectful. Injuries are not dealt with for hours without help. The food is not edible, uncooked rice, watery cold soup, just awful. My friend waited for 6 hours for water. Her medication is in question as they are giving her more pills than she took at home and do not seem to want to address her concerns. Her room is shared with another woman and is so small that they can not move. They are not allowed to walk without a walker or be in a wheelchair. The place often smells of sewage. I was appalled and my friend feels scared and alone.

If you care at all about your loved one do NOT let them go here! They don’t return phone calls to family members asking for info on our mom. Yes, Hippa approved people. They are rude and are always put on hold and 9 times out of 10 they hang up on you. Plus, they made mom wear an adult diaper instead of incontinence pads while is degrading. She was a fall risk - no guard rails on the bed. Horrible food. Staff is rude. A terrible place! No matter what she/we ask for they can’t/won’t provide it - even something as simple as ginger ale. A horrible place! One star review is generous.

I was at Heritage Park less than 6 hours when my bed malfunctioned and fell 2 feet straight DOWN. I was 5 days post-op from spinal fusion surgery, L1 - L5 No one answered the bell or my screams. I was told I was delusional when someone finally came in. After no one would call a doctor for me, I called 911 and left myself. 3 days later I had to have revision surgery to fix the damage done by the delusional bed fall. The food I was served wax bad. I too was told to go to the bathroom in my Depends, rather than being helped to the bathroom. And if you or your loved one are the lest bit overweight, ask them to get you a bigger bed. The bed I was on was so narrow I was unable to put my arms down at my sides. They told me it was a standard size bed, but at my second rehabilitation facility, the beds wrret bigger

My mother was in this facility for more than five weeks. She came out worse off than when she went in. The bad reviews are spot on. She was transferred from Blake. She was left in her bed for the first three days and only allowed to use the bedpan. Twice when the staff actually answered her calls, she was told to go in her Depends. She was in bed for 3 days, all day, until her first visit with PT on Day 3 after she arrived. She was not provided proper standard rehabilitative services or necessary equipment. She did not receive treatment for her other serious issues. One MD came into visit after she was there for five days. The MD spoke to her for five minutes. She did not examine her, note her previous injuries or even understand her medical conditions and history and how it was treated, i.e, diabetes, foot injury, hypertension, lymphedema, etc. She was prescribed a medication that was dangerous for her. The meals she was given were terrible unhealthy considering her diabetes. Despite almost daily communications with management about these ongoing and repeated issues, many of these issues went on for weeks, and after more than five weeks she was faulted for her deterioration and told she needed lifelong skilled nursing care. We question whether they were truly trying to rehabilitate her or keep her there.

Please do NOT send your loved one here, especially if they are older, because if they do get out of HP, they will be much worse than when they went in. My father went in after having elbow surgery (at Blake) thinking they were going to help him regain use of that arm and help him regain mobility, yet they kept him in a chair or a bed and never even let him get out of bed to walk around or to just move. They refused to answer his calls to get up to use the restroom, or simply even to walk around. The beds there are called scoop beds and you do not lay on top of them, you lay in them and if you are older, there is no way to get out without someone to help you. So instead of helping him regain his mobility, they put him in diapers/drugged him up and at that point, he didn't have enough thought process left to complain. I honestly thought this place was - I don't know called rehab - because they were supposed to rehabilitate you, yet after 1 month of them making him so much worse, we decided to sign him out. My sister called HP to let them know she was on the way to pick up our father and the woman at HP said he's not going to get any better, we've done all we can and we do have a nursing home facility in the next building that we can just transfer him to. I'm thinking that was there ultimate goal, since they did NOT help him. Now the therapists did a good job when they worked on him twice a day five days a week, but HP is the worst place ever. Don't take my word for it, check out all the other negative reviews. Again we would never send anyone there, Blake hospital sent him there and they should be fined in my opinion for doing that, right along with HP for there horrible care. In addition to all the bad care they gave my father, I am still waiting for 4 calls to be returned, 1 from the nurse and 3 for the social worker, but I do not expect them. My sister is still waiting for the dr. to return 2 calls as well. My father has been out of HP for 2 months, he is doing so much better and no longer needs a nursing home (not that we ever thought he did), thanks to my sister for taking him out and finding a home health nurse to help my dad at home. He gets out every day, goes shopping, out to lunch and is getting stronger every day. What a difference from the deathbed HP had my dad on and now, just wow! I think if we had left him in there any longer, he would have probably already died, but now he is thriving just being away from there.

My wife is in Heritage Park for the second time after having a spinal fusion operation. During her first visit of approximately two weeks she did not receive the physical theropy that she needed and lost her physical strength to the point where she could not walk, stand or even sit up in bed. The staff was neglegent and when they did visit her they were rude and uncaring. I spoke to her every day and learned about how bad her treatment was. After being transferred back to Blake Hospital where her back surgery had been previously been done, she has now been returned back to Heritage Park. They have denied her access to her cell phone and tablet so I can no longer call her to see how she's doing. I've called the Director of Social Services who hasn't returned my calls. This place is worse than being in prison where you at least get medical attention if needed. Now I'm stuck because I have to move to an efficiency apartment and she has no place to move to when they decide that she should be released. They should have their license revoked.

My dad was at Heritage Park Care & Rehabilitation Center for rehab. He was getting physical therapy, but they had limited activities for him. The food was okay.

be wary of this place...they had an LPN who gave meds via a peg tube and got the meds caught in it....then they tried to clear the peg tube with a coat hanger,,only causing more trauma...pt was sent to Blake er.

My husband was in Heritage Park for over two weeks. He was admitted after a hospital stay; was weak and not very mobile. The staff/team at Heritage treated my husband with the utmost respect; he had PT and he has improved remarkably. We found the facility to be clean, spacious and well staffed.

My father had a terrible experience at this facility. The nursing staff for the short term rehab showed no sense of compassion, caring or professionalism. Because he was admitted late on a Friday evening, little attention was paid to him despite my frequent request for assistance. I was constantly told his vitals were normal as was his temperature. By Monday he suffered from complete dementia/delirium due to extreme infection that went unnoticed by the nursing staff. Again, this was in spite of my continual request for aid - I was treated as if I was annoying them and when I told the charge nurse "we want him transferred to the hospital" her only response was "who's we" and it'll have to wait because there is paperwork involved. Because his infection went untreated he has potential permanent swallowing problems that are life altering if not life threatening. The experience was a heartbreaking nightmare that I wouldn't wish on anyone.

My father was sent to this facility for short term rehab following a TAVAR procedure. He was weak so needed a bit more time before returning home. Upon arrival on a Friday evening, he could get no one to assist him to the restroom and became very upset. By the time I arrived within the hour of his arrival, he was shaking uncontrollably. I attempted to find a nurse, and was stunned by her "put out" attitude. She spoke to me in a very condescending manner and in his presence was totally dismissive of my presence. By Saturday, he had become lethargic, Sunday hallucinations. Both days I stressed to nursing staff something was very wrong. I was told vitals were fine, no fever. By Monday noon he did not know who I was. Upon advice from his cardio office, I had him 911 to the hospital. We are still not sure he will recover. He was suffering form two serious infections which Heritage Park nursing staff failed to recognize and he was running a fever (which I was told by the nursing staff he had none). The ER confirmed a fever with infection. A cannot comment on the long term care portion of this facility but the short term rehab was quite literally a nightmare for my family. My father, should he survive these infections and side effects they created, will never completely recover. HIs illness was not addressed in time despite my numerous attempts to get help from him. Sadly for our family, he arrived on a weekend and I was at the mercy of the nursing staff there thinking they were truly watching and caring for my father's health needs. I now know how wrong I was. The nursing staff had absolutely no compassion or any attitude of caring. Not once did I receive an offer of assistance - I continually had to seek out someone to assess him and each and every time was greeted with an attitude of annoyance. I work for a church and have heard numerous times since my father's event of like experiences. I would never send anyone to this facility. The nurses working there should be shamed by they manner in which they preform their jobs. I hope they never have to experience the heartache my family has due to a caregiver's total disregard.

The rooms at Heritage Park and Rehab Center were nice and comfortable. Everybody was really good. They had field trips periodically and different activities for the residents like games. They had a place to go outside where they could get fresh air. The staff was sweet, very attentive, very kind, and very knowledgeable.

This care at Heritage Part is spotty at best.. My mother is a patient there and her personal items repeatedly go missing and her call light routinely is on for 25-30 minutes, There are a couple of really good caregivers, but they are the exception.

I am a patient at Heritage Park Care and Rehabilitation Center. It’s not a bad place but it would not be my number one pick. I’m not happy with the food. They don’t change it much around and the only thing I really like is the mashed potatoes and gravy. My food has to be cut up in little tiny pieces. They call that a mechanical soft diet and it doesn’t impress me. Sometimes I go to the church service and sometimes, I go to their range of motion class but it repeats; they don’t change the program around. I have heard a call button go for 15 minutes and I think that’s way too long. You don’t know what’s going on in that room and they don’t tend to get there. The patients don’t want to get to know anybody and I’m an outgoing person so that doesn’t make me too happy.

Heritage Park is a nice rehab facility. It's smaller and it's basically designed more for people that are going to be in there for a period of time to either rehab or for hospice care. It's a great place for people needing those care, but the pricing there was way more than I can afford. We were in there when my wife had some physical problems for a rehab and Medicare paid for it, but if we have to pay for it ourselves, I wouldn't look at that place. It was just too expensive. Food was fair too. However, I thought that the staff was extremely good. They were really concerned and did an excellent job with my wife. Her improvement was important to them than anything. They would keep you in the loop and we had meetings with them regarding my wife. It was just fantastic.
