I am/was a resident of this facility

1. Very Very Clean, Bathroom are ample to move a wheelchair or walker around in, and SO CLEAN! It was spotless when i came to tour the facility prior to having hip replacement surgery and is just as clean and spotless now that I am here! Everything in the bathroom is disinfected and garbage is removed regularly. 2. Food is Ok. It is not a Michelin Star experience, but certainly satisfactory for a care facility. Personnel were more than willing to go get you something if you did not get what you ordered, or if you requested additional. So if you wanted an extra cookie and your dietary restrictions allowed it they were happy to accomodate. Met daily nutrition requirements. 3. Staff - OUTSTANDING ON EVERY LEVEL!!! Everyone came by introduced themselves and were really good about letting me know what their function was and coming by to see periodically if i "needed anything" They were really pleasant and professional and when i pushed the call button, they came within 5 minutes. They were really attentive. - taking me to the bathroom, monitoring me in the shower, They were really great about making sure that my water cup was full and had ice. If you spoke to them, they would converse gladly, talking about themselves or asking how they could help. a. CNAs - very attentive, would ensure that your request call button was answered promptly b. Physical Therapists/Occupational Therapists - all the ones that worked with me were great so far, i still have a ways to go, but they have given me useful advise to be safe as i heal. They had useful "tools" to help bathe, put on body lotion, and ways to use the reach extender to not bend over 90degrees. c. Nurses - really wonderful - made sure that i didnt stay in pain and were available to give medications as appropriate. and even in the middle of the night they came fast! d. Facility - did i say how clean and spotless it was??????? and there is a lovely sunroom off the second floor nurses station complete with puzzles, games - has a nice view and is quiet and a pleasant place to just sit and relax. So far, they have provided an awesome experience to recuperate from a hip replacement surgery. I will update as I progress through their system. Overall, so far a very positive experience trying to recover from having my hip replaced. It is the perfect midpoint between the hospital and home. One doesn't realize how much energy it takes to go to the bathroom, make food/meals, remember to take meds, All this is covered in this facility.

The rooms are spacious and clean. The dietician closely followed the prescribed diet and made sure the food was satisfactory. I can't say enough about the staff. They were so exceptional! My husband was checked on constantly and they always answered the call button quickly. The physical therapy folks focused on what he needed in order to come home, and the speech therapist was an absolute rock star. They dealt kindly with my hubby's grumpy moods and they were always very polite to me.

The care delivered at the Center was the best around. The operation, the food, activity programming, rehab and nursing are unmatched. I felt comfortable, safe, well-cared for and all my medical issues were addressed surpassing my personal satisfaction. The clinically complex product was better than even experienced at the hospital. I found it so comforting that everyone at the hospital completely endorsed this Center as the best in town.

Facility was very nice, physical therapists did a good job. Daily care staff were not personable, took poor care of my mom. Some would come in to answer her light & not say a word to her. If we would go out to desk to ask for something, they would not acknowledge us or would say “ I don’t know” ?? Meals were often late & not what my mother had chosen. Water pitcher never refilled, family would do it when there ( dehydration one of my mothers problems). Overall, I would not recommend this facility. My mother has been in other places, physical building not so good, but staff/ care much better!

My dad was at The Center at Centennial. It was a very nice facility, and they had a very, very large rehab room, which was wonderful. All of the rehab employees were wonderful. My dad really enjoyed the rehab he got there. It was very effective. The staff was very caring. Some of the CNAs were a little rough, but the nurses were very caring. They kept his sugar under control. We didn't see the physician too often, but maybe that's because he didn't need to. But overall, the facility was very nice. The rooms were a decent size and the TV worked. The bathroom was very accessible for him and there was a shower in there, a toilet, and a sink. I was very pleased with that place. I tried to get him there this time, but they didn't have any beds available. I know the food wasn't the best, but it wasn't the worst. My dad was satisfied.

This experience for my mother has been awful. The quality of food is less than platable. She required a doctor and never saw one until one was demanded to appear by her POA and that after being there 2 weeks. Her physical and emotional condition is deteriating and we continue to be frustrated by the slow action to our requests as her family. She considers some of the staff unfeeling and unattentive. Others are trying their best but they seem to lack support by too little staff. The night nurse Moses is very caring and helpful. Others could take lessons from him. Our mother has not improved at all. We fear for her health in this place.

I was a resident for two three-week periods being rehab'd for total knee replacements. I am retired USAF Medical Service having spent a tour on the USAF Medical IG Team. Staff is very attentive and reponsive to needs. Food service master menu is varied and tasty. PT equipment is state-of-the art and well maintained. I made significant rehab progress during my stays and would recommend the facility to anyone who wants to make progress. One of my goals is to return to skiing.

Getting basic things like food according to your menu selection or without them omitting items from the order s or getting housekeeping to come in or someone responding to your bell, at this facility is a huge struggle. My friend has been here for over a week, housekeeping came after filing a written grievance. Unless you struggle with the reception or nurses or administration, housekeeping does not come to clean, even when you are staying long term. Kitchen will most of the time omit items from order and hardly answer calls, so patient must have a family or friend who can go to nurse’s station and tell them. It takes at least 15 to 30 minutes or even more for someone to come after ringing the bell. A very few CNAs and nurses have proper attitude. Others seem grumpy. Unless the patient is going to have a fulltime family person or friend to accompany don’t go here.

My sister has been at Centennial one week. She has been on high antibiotics for pneumonia. Which can cause loose stools . She has a non functioning hip /leg and is presently waiting for hip replacement surgery at the end of August. She very much likes the staff but has told me that they are very short staffed. She was told not to get up without assistance. But because of the wait time she has had to get up by herself with her walker . She said she couldn’t just sit and pee her pants anymore! They did provide her with depends . She had been put on a water pill because of a 13 # water weight gain . Her legs have swollen tremendously to the point they are tight and shiny . She has a recliner that has not worked properly since her admission . Therefore , she sits on the edge of chair ( she is very short in stature) leaving her legs at that position rather than elevated . A staff member put the chair in the recline position but it wouldn’t stay so that person placed a trash can underneath the foot part of the chair. Later she had to use the bathroom again. So she tried pushing the footrest down but of course it didn’t move . She leaned over to remove the trash can and fell out of the chair . She was unable to get herself up . That same person that placed the trash can under the chair failed to leave the call button within her reach . She somehow managed to reach the end of that cord to pull the call button toward her and then pushed it for help . As of today she has had one shower. I’m not sure of the policy regarding PT and OT for over the weekend but she received neither during that time. It has been a very frustrating time for her. I am very concerned for her safety . I have read and re-read their mission statement. The ones who own/run this place need to be made aware of these problems and resolve them .

My mom is in the facilities right now and getting horrible care. They have 2 nurses for 80 patients! It is not gourmet food! My mom gets telling them, she can't eat the heavy foods due to her hyninal herina. The took away a health drink that a nurse gave her because she needs to consult a dietician! She has yet to see one. My mom is sharp and not on heavy meds, which is good. But the facility shouldn't be accepting new patients when they can't take care of the people they have there! Terrible.

Not sure what has happened with this place. Experience 3 years ago was great. My mother came out worse than she went in and got pneumonia there and it progressed and couldn’t even walk when she got out. Rehab! If she was getting it, she could walk. Now in Penrose again for a serious situation that could kill her. No milk or sugar for oatmeal at breakfast. If she asked for it they would bring 2 hours after meal. I was horrified when I saw her after a month. Transferred to new facility where all situations were finally addressed but still may be too late. Close this death trap! If this is rehab stay away. Only want the Medicare money.

They put a diaper on you even when you don't need it. The food is disgusting. No drink given with meal. Food is not fit for animals much less humans. Find a better facility for your loved ones. I would rate them less than 1 if that was an option.

My dear sister in law had a terrible accident and suffered sever injuries. After being at the hospital she was moved to this facility. At first glance it seemed nice and clean. After visiting with her several times I could see problems with the room not being very clean, maybe a shortage of housekeeping? In addition, they knew they needed to change the bandages on her wounds yet they did not, she now has an infection. Be careful of these places, they may look clean and the people may smile a lot but the care is sub standard.

I noticed packets of white sugar on the table (patient is diabetic). I questioned the nurse about this, and she said they are not strict about such matters. I said very firmly that he is diabetic, and he needs to not be getting stuff like that. She said she would say something to the kitchen. But they knew he was diabetic when he was admitted, and for a week now, they have been working against his blood sugar levels. After that, I thought I better check about the sodium restrictions he is supposed to be on. So today, on the phone, I questioned whether or not he was on a restricted sodium diet at all. When he left Penrose Hospital before being transferred to this facility, we were told by the nutritionist there and his doctors that it was essential he be on a restricted sodium diet, no more than 2000 milligrams of sodium per day, no more than 150 milligrams of sodium per serving of food. He has been for the last several days blowing up like a balloon holding water at an alarming rate. The nurse told me they can't count every gram of sodium there (!). She said he is on a "no-added-salt diet." I was furious and told her that low sodium and no-added-salt are worlds apart!! That a gravy or soup might exceed his limit of sodium without any added salt! She said I could talk with the kitchen staff tomorrow. I asked how much sodium was in the gravy that he was served with dinner yesterday, and she could not tell me. I told her that if they cannot provide him with a low-sodium diet, that he needs to not be there! I am horrified. They are making him worse. They had no right to admit him if they could not follow his dietary restrictions as laid out on the cardiac care unit at Penrose -- restrictions given for after he left the hospital. I don't know what to do. They are "helping" him to "exercise" while overloading his already compromised valve with fluids due to un-monitored and un-restricted sodium levels! This is a recipe for disaster.

I visited this facility

Everyone on second floor has been extremely caring, patient with me, polite, courteous, very professional in their job. Shouts go out to everyone: if I forgot you on this list, forgive me. Sarah, Shelby, Rachel, Ashley, Apache,Rubi, Deona, and Katt. Others included Cynthia, Matti, Janet, Samantha, Soren, Tammy, Kelly, Luz, Liz, Jessica. Physical and Occupational therapy include; Ruben, Megan, Kevin, Laura, Amy, and Ashley and Allison. Housekeeping thanks to Lenore. This is awesome!! The meals are 5 stars. Activities superb. Thanks to each and everyone for the love and care you all gave me.

I visited this facility

My father came to the facility for rehab in speech, OT AND PT, the staff was over the top and my father made great progress. Unfortunately his medical condition turned from therapy to direct in patient care. The hightest quality of dignity and respect and over the top care was given to my father and the entire family. We would recommand this facility to anybody. Can't say enough about the facility,

I had a relative stay here after a knee replacement and it was terrible! The physical therapist was great but the nursing staff was terrible! The doctor didn't communicate with the nurse about turning off the oxygen and about getting the paitent off pain meds. When talking to the nurse about reducing the nurse would not do it. Even the paitent kept telling them there was no need for the pain meds cause the pain was at. 2 they continued to give the meds. The nurse staff does not listen to the paitent and don't pay attention to what the paitent is eating or ordering. This caused a lot of discomfort for the paitent going from being constipated to throwing up and diarrhea. I would never recommend this facility to anyone.

I'd like to express how disappointed I am in how overworked and understaffed they are here at Centennial. My friend had a CNA by the name of Ron, who seemed to be working one entire wing of the 3rd floor by himself most days she was there. I stood in the hallway watching while a lady by the name of Melissa stood watching as he went from one call light to another on both sides of the hallway. There were no other CNAs in the area for twenty five minutes as he ran from room to room. I did see a nurse go in to help him with patients but no one else was around. Where are the staff? Ron was very professional, not saying anything bad about anyone, even when I asked if he was the only CNA on the floor. He is a gem in this place! He loves his work and never once has anything negative to say about his co workers or patients, unlike the others we dealt with. My friend loves the food and the ones who come up from the kitchen each day to serve, including Sam and Rachel! The room is wonderful, and the place is great, just extremely understaffed. I hope that changes when we are back next month.

I visited this facility

The Center at Centennial was a beautiful facility, but I was not impressed by the individual patient care. They seemed to be lacking in staff to do what needed to be done. The caregivers were friendly, and when asked for help they would provide it. The building and the rooms were beautiful. I wasn’t happy with the food. The building was new, spacious, and bright. It had beautiful surroundings.

The physical and occupational staff are fantastic, however the nursing staff is horrible. Meals are always served 30-60 minutes late and are not warm by the time they arrive. Meals often come without a beverage or condiments (no place on menu for condiment selection). Call lights are not answered promptly. Scheduled for 2 showers a week, but only get one. Early in stay there was no attempt to get patient dressed, just left her in a hospital gown (clothes were available to wear).

My mother is on her third stay at Center at Centennial and it's always first on her request list. They take excellent care of their patients and the PT department is outstanding. The food is stellar and she looks forward to seeing what they will bring each meal. I highly recommend this place. She can even visit with her dog in the lobby. There is always something to do.

I am a past client of this provider

My mom was discharged from the hospital after a leg amputation. She went into Center at Centennial for rehab. It's a fully equipped rehabilitation hospital and the care that my mother got was top-notch. We couldn't have asked for a better care. The physical and occupational therapists who worked with her prosthetics during the rehab were great. It's a clean facility and they were accommodating. You can even bring in your pets into the lobby area for them to visit. Mom happened to be there during Thanksgiving. We didn't want her to feel bad so they allowed us to bring in all of our food and friends and family in on a banquet table and spent our Thanksgiving there. So that was great. They were also very good about informing us of her health and everything that was going on. We were there regularly and nobody had a complaint or a concern. I think she would have liked to stay longer. I would highly recommend this place.

My mother was a patient at this Rehabilitation facility for five weeks. She suffered a fractured C6 vertebrae in her neck and had bleeding on the brain due to a fall she sustained. The nurses and doctors gave her the best of care. Everyone that she (we) came in contact with were all professional and knoweledgable and caring when dealing with my mother. She never ever felt ignored or forgotten. Someone was always checking on her and making sure that she was comfortable, water bottle filled and her pillows fluffed and that she was comfy. We couldn't believe how well she recovered and how well she did through the guideance of her physical therapist and everyone working with her to get her where she could be home to begin her home therapy and begin to return to her normal routine. I would recommend the Center at Centennial to anyone who needs additional time to heal, to rehabilitate, and to get that one on one attention that is so vital to one's recovery time. The Center at Centennial provided holiday cheer, fun activities, games and the like to help keep their paiients active, involved and thinking; making their stay seem shorter than it actually was. It was like walking into a Five-Star Hotel. They made my mother feel important and how they treated her made her feel like a human and not just another problem for them. The nursing staff is bar none! I would recommend the Center at Centennial to anyone who needed additional time to heal and that person was quite not ready to go home and be be themselves. My mother attributes her achievements in getting around and doing the things she does to the therapists there at the Center. They truly kept her motivated and encouraged and yet were gentle in their manners towards her. It was if they couldn't do enough to accomodate her needs. And, as a bonus, the Center has a view from almost any room you were in! Beautiful Colorado right out your window!

AVOID!! Do NOT JUDGE A BOOK BY IT'S COVER!! GORGEOUS FACILITY. Healthcare is awful!! No ethics and patients are neglected.
