Tracey, the assistant administrator, has gone above & beyond in implementing my mother’s move back to Sunset after recovering from serious illness in a nursing facility. Mom has been so warmly welcomed back by everyone from the care staff to the cleaning lady. She said she feels like she’s home again. Thank you.

Family member moved into a room that had been empty for several weeks yet had NOT been cleaned. Had to be replaced but that took several weeks.Dust and dirt everywhere. Carpet was replaced as it was to filthy to be cleaned. The family that transported the resident spent hours cleaning that day. Most meals were Swanson heat & serve crap. Only several months with an activities person. about 20 months W/O much for activities. The owner was there occasionally and had the personality of a slug. They might change their tune after being fined by Wisconsin DHS THOUSANDS of dollars. But I doubt it.

CAUTION: dangerous neglect; avoid at all costs. Our relative resided in this facility during 2021, 22, and part of 23. During that time we witnessed poor handling of resident needs, apathy toward patients, lack of staff supervision, neglect of alarms, and medically dangerous activities (ex: an aide attempting to perform what is deemed a surgical/medical procedure and must only be done by appropriate medical staff), and severe neglect of patient care that could have been deadly. Our most recent episode with staff neglect resulted in stage 4 pressure wounds (involving deep tissue, tendon, bone). The wounds were discovered after we were finally able to move our loved one to a different facility. There is no way in the world pressure wounds could go unnoticed, especially long enough to develop to stage 4 level, when staff are properly bathing, dressing, and toileting a resident. These wounds required emergency hospitalization, intensive treatment, painful surgical wound debridement and packing, and a lengthy stay in a skilled nursing facility. We are four months into treatment at a skilled nursing facility and the doctors currently anticipate it could take as long as a year for our family member to recover sufficiently to return to an assisted living environment. This could have been fatal if unnoticed for even a few more days. As it is, it has been incredibly painful, cost a great deal in medical claims, and resulted in losing the newly acquired room in a much better, safer facility. You can find confirmation of the problem on the DHS website as DHS is conducting a formal investigation. Sunset Ridge is a small facility (20-ish residents) but kept a very small crew on shift at all times. It was common to see only two aides on-site during weekends. The patient “emergency” alarm was constantly sounding, ignored by staff with the excuse that the same resident/s would pull their alarm for staff attention. How did they know when someone actually needed help? The facility claimed to employ a ‘chef’ to prepare meals, but we only saw a kitchen worker a few times and the staff mainly provided heat-and-serve frozen items from Walmart’s grocery department. They rarely had an activities director on staff to help residents remain engaged, and when they did have one it was a former aide who moved into the activities role and was not replaced in her aide role – leaving additional staffing shortfall that facility owners seemed to have no interest in filling. Staff regularly spend their time outside smoking and chatting while patient alarms go off and patients call out for help.

My first impression of Sunset Ridge was they have a great room, and I thought it was beautiful. It smells pleasant, no bad odor. I kind of like that they have the main meal at lunch, and then they have a lighter dinner. The dining area is just plain, but it is nice; nothing extravagant, but I like that outside the dining room they have a big veranda, which is like a wooden deck where the residents can sit when it is warmer, and it is right off of the dining room. The dining room was very clean and neat; it is nice. One thing I do notice, and I am going to talk to the administrator next week, is that it doesn’t seem to have a lot of caregivers. The rooms are large, and it is close to my home.
