Our mission is to provide compassionate care in a warm, friendly environment while meeting the health, social, and individual needs of each resident. We pride ourselves on the strong relationships formed between our staff, residents, resident families, and the community. When a resident chooses to make Sylvan Crossings their home, they meet with an interdisciplinary team that includes their families. Together we assess the services that are needed.
Residents are encouraged to bring their own furniture so they feel more at home. Each suite is spacious, has a 24-hour monitoring system, a full bath, individual thermostat, and an emergency call system.
Most residents appreciate routine assistance with personal care, such as bathing, medication monitoring, and dressing. Residents with walkers or wheelchairs enjoy mobility assistance. Incontinence care promotes independence and helps retain dignity. Optional services include on-site hair and nail care and assistance with transportation to offsite locations.
Menus for meals, made from scratch, are thoughtfully planned by our staff dietician. Residents have three meals in the dining room, and snacks are available all day long. A private dining room can be arranged for visiting family and friends.
We manage medications, including ordering and dispensing. Nurses provide regular consultations and assessments and regularly review medications.
Daily exercise is offered both for individuals and groups. Recreational programs such as games, crafts, and music are provided. We offer assistance with writing correspondence and reading. Private areas are available for family gatherings.

I don't know if Sylvan Crossings at Hunter Ridge had apartments or not, but they showed me a studio and it was adequate. It was a large room with a large closet area and a private bathroom, but not exactly where I'd want my mom to be. My mom doesn't need as much care as a lot of people at that place. They said they had activities, but I didn't see anything going on. It could have just been the time of day I was there. It was located in a newer developed section of town. It was all on one level, and it looked fine. It was a nice place and it was clean. I felt like there was a lot of information they didn't give me and they were just kind of trying to rush me through. I only met one person who showed me the room, and that was it.

We visited Sylvan Crossings at Hunter Ridge. I like the size of the rooms. The staff people seemed friendly, but I thought the place looked a little bit rundown. The beauty shop was very important for my mom. She was happy about that, and since she's never been in a communal living situation, she doesn't know what to expect. They showed us the menus, the activities, and the library. They were very accommodating. They gave us the sheet for the month of what was happening; it was bingo was one that stood out to me. The staff was so friendly, and I think my mother would get really good individual care there. I like the idea that she can get her nails done there, and the private room was nice. They also have a little church thing, it's nice on Sundays. The outside was gorgeous and the common areas were, but you could tell it was an older building.

Mom and I toured Sylvan Crossings at Hunter Ridge. It was fine. We got the information we needed, but Mom did not like it as much as she liked the other facility we visited. We were there on a Saturday.

My husband and I recently had the difficult task of finding an assisted living center for his younger brother. We chose Sylvan Crossings in Beaver Dam, Wisconsin. they offer security and caring comfort for all the residents and their visitors. This is primarily due to the exceptional abilities of the director and her very capable staff. Just knowing our loved one was given the daily assistance he needed and was encouraged to participate in social activities was a tremendous comfort. We visited almost daily and always found him clean, dressed and looking forward to his next yummy meal. The welcoming smiles and hugs when needed made Sylvan crossing feel like home.

The Sylvan Crossings at Hunter Ridge assisted living facility in gorgeous Beaver Dam, Wisconsin was one of the places we saw while looking in the region for our uncle. The lovely exterior looked like other resorts in the area, with beautifully landscaped grounds and tree-lined walkways.The staff was kind and caring to all of the residents that we saw -- of varying capabilities. The communal rooms included a lovely library and the ability for seniors to participate in a number of stimulating activities. The exercise center includes daily yoga and aerobics classes. The equipment is available throughout the day and they receive assistance whenever necessary. The residents may also safely navigate the lovely grounds for exercise. The food we saw looked and smelled delicious. The menus all had healthy food options for all kinds of diets. Healthy snacks are offered all day and into the evening, as well as whenever residents request them. The dining room was quite nice, with plenty of space. The living spaces were generous, with plenty of space to house seniors' cherished possessions.The many planned activities feature everything from exercise classes to book readings and arts and crafts. The residents are taken to many interesting cultural events in the region. Weather permitting, they go on nature walks to enjoy all that Beaver Dam has to offer. It was clear that all of the residents' needs are met here. The decor of the entire facility was really fresh and bright. The staff was also exceptionally attentive -- and we could see how any senior would really enjoy living in this environment they consider their home.
