The rooms were very nice and clean. Everyone was very friendly. My friend is there and she tells me all about the Bingo tournaments they have, and many more activities. The facility is very clean on the outside, no trash blowing around. She tells me there is a courtyard with a fountain that attracts lots of birds.

Silver Tree Nursing and Rehabilitation Center seemed clean and nice. The tour was more of giving me information, but the staff was very informative. I saw a common area where people were watching TV. I didn't see any activities though. They need to improve staffing, which is a consensus with most of the facilities.

Silver Tree Nursing and Rehabilitation Center was very nice. They showed us the rooms. They had an extra room for visitors for lunchtime, suppertime, and family dinners. They had some other craft areas, game rooms, and a beauty salon, too, that they came in and doctors were available along with the nursing folks. They have a single room for a nursing home, but they could put both my mom and my dad in the same room, which was nice, instead of splitting them up. They said they would make it work without any problem. They were limited to the furniture. We could provide one of those chairs that move and kind of helps you to lift up, like a lift chair, from sitting to standing. We can put those in there and maybe a small refrigerator. The staff was very good. I think it was the marketing director that took care of us at both times and they knew the facility. They showed us around a couple of rooms. We toured the whole place front to back.

The building is set on a nice property with lots of trees. Has a warm feeling when you walk in, receptionist is very sweet young girl. Rooms are spacious. My friend was having therapy done and exceeded expectations! She also loved her CNA's and food.

The management is terrible and the staff outright lies to cover things up. My relative was in a room by herself they moved a new patient in without having the results of their COVID test and the next day found it was positive. My relative let me know but the facility did not call to inform us and when we called they lied about it. My relative contracted COVID and was sent to the hospital. The hospital called to notify us not Silver Tree. My relative just died from COVID due to managements total neglect.

Worst place ever! They were neglectful in caring for my mom! Their communication is non existent. They lie right to your face.....all the time! You cant get them to fill out everyday paperwork required by insurance companies.

If you are a Veteran do not come here. There’s a lack of staff to attend to their so called quality of life motto. They will not take your word in following your doctor instructions , if you don’t have them in writing. Those familiar with the VA know that progress notes in new clinics take a few days to be written up and approved before being made available. This cause lack of basic need such as bathing and blame the veteran as refusing to bathe because the Veteran follows Dr orders. The so called Veteran representative is just a title for they don’t work for the VA or represent the Veteran,instead they work for Silver tree and will lie to defend the facility. VETERANS. BEWARE.

I had really good results with Silver Tree. I saw my dad excel like crazy when he was there in rehab. His room was a five. They had him so busy with physical therapy, but they had other activities available as well. They had veterans meetings once a week where they would get-together and just chitchat. They had bingo. They were treating him for a hip fracture, and he made a great comeback. They were very good at making sure he got that attention he needed. However, the food sat for long periods of time before it was served on trays in the dining hall, because no one would take it to the table. They may be short staffed in their dietary department, but I would rate the taste and flavor a four because they had a variety of menus for the residents. However, the staff member did not do a good job communicating among themselves about the resident or where my dad was. I would have to go to several different staff to members find answers about where he was in his rehab and how he was doing. The facilities were very nice. They were very active and everyone was busy. There were a lot of physical therapists there. It was a great facility as far as what they did for my father.

When i visited the facility and it was more than one occasion I saw residents crying asking for help and heard the CNA tell the housekeeper to ignore them that they get on their nerves and I've never seen anyone disrespect the residents like the CNA s did. I always heard them gossip about the residents and say how they wish they can shut them up. They also ignored the residents and ignored alerts when they called for help the therapy crew were good about attending to them. My biggest concern is how the CNAs would ignore the residents family when asking questions like to me for instance.than heard them talk about us (family) and say how their better than us. The worst assistant living I've ever been to. I advise people who are thinking of sending their loved ones their to think twice.

My dad was there for rehab after a stay in the hospital. They discharged him with no follow-up supportive services whatsoever. He has been home five days, and he is doing terrible. I had expressed my reservations about the home setting when there was a meeting of staff and family, and I was reassured that home health would address my concerns. There is no home health. My Dad is just vegetating away.

My mother was at Silver Tree Nursing & Rehab. It was for respite care. We were going out of town for an extended vacation, and Silver Tree has filled all of my needs. It was even more than what I expected. They did physical therapy for my mother, and they took care of all of her personal needs. My mother had socialization while she was there. Everything was fine. My mother didn't want to be in a room by herself, so she had a bedmate. As far as recreational, she played bingo, she had her nail polished, and had her hair done.

I didn't like anything about Silver Tree. I didn't like the way they talked when I came in. They were very cold personality-wise. They just treat you like a patient. I don'y know any activities that they did, and I never tried any of their food. I also didn't get to see the rooms. It was a very big letdown.

the management of this place is terrible. My father lost a limb and this facility would never acclimate him to using the prosthetic. My father spent half of his time having to back to the hospital. the workers are very friendly but overworked. I would never put my loved one in this facility.

At Silver Tree Nursing and Rehabilitation, the staff is friendly and fairly attentive. They try to give a homey atmosphere by hosting holiday events and opening them to the community. My father-in-law is a resident there and we had the opportunity to participate in the Halloween trick or treat event they do each year (complete with a little haunted house). Children in the community come through and trick or treat at the individual rooms, so the residents get to interact with them and pass out candy and admire their costumes. They have two different 'sides' of the center, the nursing home side and the rehabilitation side, and in all honesty, the rehab side looks alot fancier then the nursing home side. The food varies on the the diet of the resident (like diabetic or high blood pressure) but is somewhat better then the average hospital food. Overall I would say it is a decent place.

Silver Tree was great. My grandmother is not very social but the staff here was very helpful with her and made her laugh. They made it enjoyable and when we visited, she wasn't as mean as when she was in the hospital. A staff member came in, while we visited, and the two of them joked about taking my grandmother to the night clubs. My grandmother rarely laughs or talks to anyone in a comical way, but they knew how to do it. The staff was very helpful when it came to assistance for her. She was in there for stroke rehab, and the staff kept a positive tone, and never got upset with her. I can't say I would have been as patient. The staff was well trained in medical as well as social skills. I have eaten at the cafeteria, and the food isn't terrible, not the best but not terrible. The furniture was very modern and had a homey feel to it. My grandmother had a wonderful time there, and overall the place was great!

BEWARE!!! This place needs to be shut down before someone dies under their care. The staff never has answers to basic questions and we are investigating if they were taking unauthorized funds out of my mother's trust fund account. DO NOT RECOMMEND THIS NURSING HOME TO ANYONE!!! THE STATE CONFIRMED THIS PLACE DOES NOT PROVIDE THE CARE THEY ADVERTISE AND THEY HAVE HAD NUMEROUS COMPLAINTS.

My father went for physical rehab and left this facility in worse shape than when he went in. He was discharged to my mother's care in acute renal failure due to severe dehydration and a kidney infection. Within 18 hours of discharge, he was in intensive care and has spent a month in the hospital. He had lost almost 20 lbs. in 3 weeks and had an bedsore on his sacrum that we were not informed of. Besides not recognizing the medical problems that my father was having, they provided NO discharge teaching to my mother regarding his medications. We almost lost my dad and I hold Silvertree responsible. The medical care was extremely negligent.

I am not impressed by the level of skilled nursing or care of this facility. I think that the patient's family and friends have to be too involved in monitoring the patient. I did not feel that they are very good this business. The only way I think I would feel comfortable would be to constantly monitor the patient myself. But then this defeats the purpose of this facility. Maybe they are too new at this. But I was not impressed.
