I was so very well seen to and rehab was over the top with all therapist tended to everyone’s special needs I enjoyed the food and nursing staff is great. Very clean. So impressed and will recommend to all.

We have a dear friend in this place. When we first saw him after being sent here from the hospital, he was much worse than when he was in the hospital. The receptionists were not friendly at all and acted put out when we came to the door. Lots of folks standing around talking but not much help to the patients. We even spoke to the administrator and the liaison who gave us her "personal" number and said to contact her any time. We tried texting twice and calling twice but got NO response. We might as well have not had the number. Our friend sat for over 2 hours with his lunch in front of him unable to eat because he cannot use his hands. No one noticed until the occupational therapist came in and realized what was going on. The CNA came in about 3ish and said she didn't realize no one had fed him for that he needed to be fed. Who knows how many meals he has missed. They keep putting an IV in him because he gets dehydrated but no one is popping to give him his water -- which he cannot get by himself. I would definitely think twice about putting a loved one here. The really sad thing is, we only found one person who seemed genuinely compassionate with the patients. He is the kind gentleman who comes to get our friend and take him to physical therapy. All the rest seem more involved with talking among themselves. They don't even acknowledge you when you come in.

My mother has been at this facility since late September. Most of the staff I have talked to or been around are very helpful. My mother imo, is a handful at times and the staff handles her rudeness and outburst very well when she has these episodes. The staff that I have more contact with are great and always very helpful. The facility is very clean and smells clean. Tammy, the business officer manager is fantastic! Should I have an issue, I know I can always call her and she will check into things even if it has nothing to do with the business part of it. Carlos, one of the housekeepers appears to have a calming effect on the residents. My mother mentions him nearly every time I talk to her. She always tells me what a hard worker he is. Sylvia (not sure exactly of her position) is always very sweet and helpful and will walk messages back if you can't get a hold of one of the nurses. It is hard to get a hold of the nurses or head nurse at times but it is because they are always with the residents. I would have given a 5-star review except for communication at times isn't the best. With covid gearing up again they moved residents around and didn't notify me so that I could call my mother and get her prepared for it. This resulted in a fiasco and nearly 2 days of her calling me complaining. The assistant director did call me and helped with the issue. Other than that there have been no problems. It takes time for the staff to get to know the new residents that come in so there is an adjustment period. They have activities for the residents should they want to participate. I've witnessed them playing bingo, working on puzzles, and doing other activities. They also have ice cream socials. I highly recommend this facility.

My husband was at San Antonio Wellness and Rehab for a week. The nursing staff was inattentive. It would take sometimes 45 minutes for them to take my husband to the bathroom. All equipment in the facility was very, very old and outdated. The physical therapy was very brief. They're short-staffed. Medicine was not always given in a timely manner, and he was taking medicine that needed to be exactly given at the same time. There is one good thing that was there. The speech therapist was excellent. Most meals were served in takeout containers. They should hire additional staff to attend to the patients and hire additional kitchen staff. Then maybe they wouldn't have to serve on paper containers and stuff. Additional medication monitoring would be helpful and to refurbish all equipment, beds, and wheelchairs. All physical equipment needs to be redone. There were things that needed repair. I think they were doing some remodeling. The equipment in the patients' rooms was extremely outdated. Beds were at least 30 years old. Wheelchairs didn't really work that well. It was pretty bad. They didn't even have a recliner for the patient to sit in. There were no modern conveniences in any way. There's not a piece of equipment in there that does not need to be replaced.

My experience here at Heartland was very pleasant and happy, thank you Heartland. I would recommend Heartland to a friend if they needed it. And all of you are doing well, I don't have any complaints about how to improve the facility. Thanks for everything that all of you done to me.

I am writing on behalf of my wife, Aurora. I've asked how her stay was at Heartland, and her response was, that she was very satisfied with the way the staff treated her. She is extremely greatful for all the help she was provided with. She has stayed here at least 2 times before and she'd recommend to a friend that they should stay there if they ever need to. I was there for aggressive physical therapy and I found it. The therapist were very encouraging and free with their praise. I left ready and confident when I went home.

It has been six months and a long recovery for my father. Thankfully we got him out of this terrible place. He was trying to heal from a lower limb amputation and was not taken care of properly at this facility. His bandages were not changed daily. He was not to have physical activity, yet they made him walk to therapy & exercise daily which made the healing worse. The staff missed scheduled doctor appointments and did not communicate with his physicians. The hospitalist on staff, Dr. Medrano served no purpose as he did not look at his wounds or check his vitals while visiting him or check that he was being cared for. We received bills from Sound Physicians for $615.00 per day for that service. We did have some luck with the some nurses, but hit or miss. At times we would call (we all live out of town) to make certain his dressing had been changed and to check on our father. The nurse on duty (if they would answer) would assure us they would do so, but the following day we would show up with the bandages from a day or two old. We had to start taping them & dating just to make certain they had changed them. The first meals they had for my diabetic father who checked in at 3pm (enough advance time) was a box of honey nut cheerios a chocolate chip cookie and 2% milk. DIABETIC. They did offer him a cold tuna fish sandwich at 8:30 pm. The photos on the website are misleading. They do a good job of keeping the place clean, but the interior is deteriorating. Ceilings stained, dining room dated. They also have snack machines that patients can freely access and use their credit cards to make self purchases of candy, cookies, sodas, etc. Found this out after I received the credit card bill. I did not see any activities for the patients although they have a monthly calendar. My father's biggest activity was watching the birds through his window. I think they do offer amenities such as salon services & laundry services for additional fees. I would not recommend Heartland San Antonio for care of someone you love.

I have been a patient at Heartland on Heartland Drive in San Antonio on three separate occasions. Each time I have made superb progress in recovery because of my excellent care by very capable and professional employees. Furthermore I am always treated with care and compassion and even good cheer by those working in a variety of capacities of the home. Now I wish to point out one very important thing. Why is it that I keep returning to Heartland whenever I have need of a skilled nursing facility? The reason is simple and straightforward. It is not the food or any of the amenities. It is the people — the same people I see visit after visit. This alone speaks well for the environment and management of Heartland. Please know that whenever it has been time for me to leave a hospital and go to a skilled nursing facility, I am presented with a list of possibilities by the assigned case manager. I never ever look at the list. I always choose Heartland without hesitation. Therefore it is my heartfelt recommendation that a replacement of staffing not be a part of the change of ownership of Heartland. Very disappointed my Mom has been here only 5 days and they have not been able to get here meds to her I had to bring some in from home. To top it off my mom failed out the bed this morning and she called me after no one answered the nurses bell that she pushed, so I tried calling the nurse desk and no one answered so I called 911 after my mom laid On the floor for over 20 mins. NOT ACCEPTABLE. This happened with the overnight crew. I have tried to contact the Director of Nursing no answer. I will be in there office First thing Monday morning!!!!!

This is the third time my wife has been in Heartland of San Antonio. She likes everybody and the therapist, and they really like her. We're happy with them. She was in the hospital, and Peter, the manager, came to visit her over the weekend. We're trying to get her on Medicaid, and Nancy of the business office has really been a big help getting the forms filled out and helping me trying to get all of it together. We had chicken a la king, Mexican food, and a Sloppy Joe which was good. They have menus, and they have a choice of 2 different meals each. They have sing-along and bingo.

My mom is in a nursing home, Heartland Health Care Center. They’re taking care of my mom. It is not much of a community. It’s a nursing home; it’s like a hospital. It doesn’t have much there for her. The staff members are all very friendly and very nice caring folks. The dining room looks like a hospital dining room. There is not a lot of atmosphere there. Heartland is like a hospital. They don’t seem to have a lot for my mom to do there. It’s a bunch of rooms with a hallway. Like I said, it’s like a hospital. Activities are very limited. They have bingo and some other activities like that. I think they have some pets that come in. It is just very limited.

My father had a stroke, and he is at Heartland Health Care Center for rehab. Their staff treats you like family. It is just wonderful. You come in, and you feel that you are not just a patient. They make you feel welcome. The staff is very helpful and informative, and they all know their job. The rooms are really lovely. The food is good; I've had it myself. They have two large dining rooms and one small one, and they are all really good. They are very well maintained. They do exercises whether you are sitting in a chair or you can stand up. They have bingo and music several times a week. They also have church services. They go on outings for lunches, so it is really nice. They even go on some shopping trips too. I think one thing they can improve is the communication between the administration because they just don?t run around to give you information.

I was in Heartland Health Care Center for 9 days, and they had really good physical therapy. I was treated well except during nighttime because they seemed to forget me. I enjoyed my stay there. They had a lot of activities. It was nice. The physical therapy department was awesome. They helped me get back on my feet, and everyone was really friendly.

This was the worst one ever. My mother did not get the care she needed. The night staff never knew anything, they threw pizza parties at the nurses desk, you could never find anyone to assist with her needs for her condition. I asked about her therapy schedule and no one ever knew. Administrators and DON's were never around or too busy to talk to the family. I took her out AMA and was able to get better care through her Home Health Agency. I do not recommend
