My mother-in-law stayed at Huebner Creek Health. Honestly, I couldn't go in to see her because of the COVID thing, so we do the phone, but she was able to talk to me. She liked it most of the time and then sometimes she didn't. She's 85 and she's used to being taken care of at home and waited on, so it's a little bit different when you go into a rehab. I had to call though, they didn't call me. Here's the one thing I want to say about all the medical field right now. They suck at communication. Communication is the worst it's ever been in the medical field except for hospices. I gotta tell you when you have home health people, they're awesome, but you get your loved one out of the home and into some kind of a facility and I think these people become brain dead thinking they shouldn't call the family with issues. They have to learn to communicate. We are living in a challenging time and when you don't communicate with a family it could become intense. It's communication, that's the one thing I'm gonna say. They need to learn to be the one who initiates the communication. The staff though was patient as I asked questions and I'm a very intense person. I want to know why if you tell me something, you gotta back it up with your why, and they were very good at doing that. With food, she liked it sometimes, and she didn't like it sometimes. Her taste buds were kind of messed up from the COVID, so I'm gonna say they did everything they could do to make her happy. The short spot that I saw when I entered was very, very clean. It was very pretty.

I'm not happy with Huebner Creek Place Health and Rehabilitation Center, but we're working on trying to find my Dad somewhere else to go. We just have to wait for some technical things. They don't respond fast enough. The food is not appropriate for dad to eat as a diabetic. Going through the hallways, we always smell urine, which is not a good sign. I feel like they're understaffed. So, the people are not getting proper care. The meals are horrible. It's not worth it. He's not getting what he's paying for it all.

This facility was one of the worst I have ever experienced. The horror you hear each day regarding these types of facilities is coming to life for my family member who stayed for rehabilitation. The staff did not even provide the minimum of constant care required. Nor did they seem to care. It took continuous advocating from myself and fellow family members to get a basic level of care, and even when it was provided, there was zero consistency with it. Not even senior management, such as assistant nursing directors, knew who to perform essential nursing functions correctly. The staff finally got tired of being held accountable to where they started not performing actions correctly and made my loved ones fear for their own life. Thank god I was able to get them out of there quickly. However, I can only imagine the poor people who don't have those same advocates and are suffering. WHATEVER YOU DO, PLEASE DO NOT SEND YOUR LOVED ONES TO THIS FACILITY! Choose and different facility don't let them fool you like they did my family.

I was a resident at this facility (nice clean place) for 2 months. I want to thank the staff/employees for working long hours and doing the best they could to accomadate me during these tough times the coronavirus pandemic. THANK YOU!!! I couldn't have family or friends visit me so these people became my friends. They all did a good job but these people STAND OUT THEY WALKED THE EXTRA MILE. # 1 PATTI, NATHAN, JOANNA, TAGGON, CAROLINA, JONATHAN, BEN, TINA, CRYSTAL, ROCIO, MIKAYLA, CLAUDIA, MARIA, AND LAST BUT NOT LEAST DAVID FROM THE FRONT OFC HE'S SO NICE, KIND AND RESPECTFUL!!! When I visit Huebner Creek Health and Rehabilitation, I see smiles and staff eager to help you. There are a LOT of staff here who have been with the facility a long time. You will see the same faces and there are a lot of smiles. The food is GREAT and you have different options so you can make choices. The facility is clean. The Nurse Practitioners are awesome here too. They actually talk to you. The therapy department is BIG and the team there has tenure as well. They know what to do to help people.

I am a private care sitter, companionship for my client Jerry. Staff was friendly, caring. Nurses were very caring & attentive to Jerry. The therapy department is very active has a lot of staff. Jerry enjoyed his time during therapy on a daily basis. Room was ok, interaction on a regular, daily basis

I have visited this community on multiple occasions and I have always been impressed with the management team and staff. They are very welcoming, always available to family members and the social service department does an incredible job in coordinating the needs of their patients.

I have a relative who currently and Unfortunately has to endure the lack of sincerity and care this facility provides. The CNA’s are lazy and could careless if your relative is sitting in their own urine. The facility is under staffed majority of the patients there are left alone and don’t get checked on because of the short staff. You walk down the hall and it wreaks of soiled linens. This facility has gotten from decent to worse in the last 6 months. If you can afford take your relative elsewhere. I sure wish we could

Huebner Creek Health and Rehabilitation Center is horrible. They have the small hospital beds from the 1940s with mattresses that are paper thin and you can feel all the bars in the frame. It's a horrible bed. The TV is 12 inches so my father can barely see it. They have a lot of residents in there with mental disabilities that make you feel like you're in a special state hospital. They're understaffed, but those people have been very nice. He has not complained about the food. If it was bad, he would certainly let me know. He's been very angry over there and he calls it a prison. He's gotten himself up and walking in the hall because there's nothing going on. He was transferred there from the hospital.

My mom was at Huebner Creek Rehab and while there were helpful caring nurses, the social workers were almost non responsive to my questions and concerns. Rarely did they call back. The billing department is less than considerate when it concerns the billing cycle they use. They conveniently bill way ahead, whether the patient is still there or not. We received a bill for $6500 MORE the what was owed simply because they tacked on another month's worth of care/stay even though my mom was long gone by that time.

Huebner Creek was a very nice facility. My friend was placed there after University Hospital for a couple of weeks, and it was paid for by her insurance. The facility was very clean, and everything about it was very good. The staff was very adequate. It was expensive, like $5,500 a month. They're private pay. The grounds were very nice and neat.

This place is a fraud. My mother was sent there from her hospital for rehabilitation after having shoulder replacement surgery. During the three days she was there she was forced to urniate and defecate on herself nine times because the staff was too lazy to responds to her requests for help getting out of the bed to go to the restroom. The food served to her was not fit for a stray dog to eat. She received NO rehabilitation. Fortunately I discovered this mistreatment only after three days because she was scheduled to be there for two weeks. After reporting this facility to the hospital where she had her surgery arrangements were immediately made to get my mother removed from this neglectful facility. Calling this place a Health and Rehabilitation Center is a complete joke!! I am attempting to file a complaint with the Department of Aging and Disability in the hopes of preventing this from happeining to someone else.

2 yrs ago this was a fantastic facility, but it has gone way downhill in the last 6-9 months. I don't know if the management has changed hands, but the turnover of employees is extremely high, there are less RN's than there used to be, and it sometimes takes hours to get someone to respond to a call button. The food used to be decent, now it's so bad it makes Michelle Obama's school food look like Ruth's Chris Steakhouse fare. Prepackaged frozen garbage like tater tots and steak fingers that are inedible. I see no sign of nutrition in the meals and it's gotten to where 2 meals a day are now provided by family for our loved one. We are paying a small fortune for this place and it is atrocious!

I have visited one facility named Huebner Creek Health and Rehabilitation Center. I was impressed because the facility is well run. It was friendly. I didn't find anything wrong with it except that I'm not sure about the price. We only went into the common area and into the administration office. It was clean, and it had a good odor. It was up-to-date, and it was relatively nice looking. The best part of the community is that it is friendly and clean.

The home previously had been rated poorly and valid complaints placed. I did not know this before placing my Mom there late summer of 2014. As of late September I believe they deserve a better rating. Indications of a complete staff change from CNA, LVN, RN, ADON, DON and other support areas had been implemented in the Spring of this year. It was a pleasant surprise to see a really well run facility instead of what had happened in the past. In seeing other places this is a really good place if you can not accomade or are equiped to take care of a loved one on the Northwest side of town. They need to finish their plan for me to give them 5 stars. If you do place someone make sure and inspect what you expect. Try to work with them as a team. Adequate staffing that cares will make this a really great place.

My sister was in Huebner Creek for rehab. I was very dissatisfied with this facility. We were misled to thinking they could furnish us with ten liters of oxygen, but they did not have it. She fell and they never let me know. I had to take her to the emergency room to have her assessed there. This place is no good as far as I am concerned. If you’re really sick, that’s not the place to go. Her room was ok, the cleanliness was ok, except that the trash cans were tiny and there were days that it wasn’t cleaned. The rehab was very aggressive. There were orders from the doctor, but they don’t have a copy of the orders. The Medicare director was also abrupt and said they need to expect ten percent more of my sister’s work, otherwise she will be dismissed. My sister was very upset about this. She started getting anxiety attacks and I had to take her again to the emergency room and within a day and a half she passed away.
