My 90 year old mom fractured her hip, required surgery, and was transported to Granite Mesa Health Center for live-in rehabilitation. It's been absolutely amazing! The staff are attentive and kind, and mom's therapy is fantastic! She feels confident and stronger every day. She enjoys the meals and positive vibe! God bless the staff at Granite Mesa...keep doing what you're doing! Thank you, The Woods Family

My mother was a patient at Granite Mesa. I was nervous to leave her there at first. However, she settled in quickly and received excellent care. When she got there she was very weak. Therapy worked with her and helped her get her strength back so she could go home. The nurses and aides were also very attentive. It took a few days to get her medicine schedule on track but the staff listened and tried their best to follow her home routine. My mother told me the aides, “give great showers!”. Once she discharged home, someone from the facility even called to check on her to see if she needed anything. I know they had a daily activity calendar but my mother did not participate. She would watch TV or play games on her phone between therapy sessions instead. She liked the food, always said they served her too much. Overall they did a great job and if she ever needs a rehab facility again, she would go back to Granite Mesa.

The staff are all very friendly and helpful. Our Dad loves the food and we are always made to feel welcomed when we are visiting.

My mother was a resident at Granite Mesa and the nurses and staff were all very kind and helpful. She was also on hospice at the facility and again the staff could not have been more accommodating.

I used to visit my grandmother at Granite Mesa when she went to live there after getting dementia. I was always so grateful for their friendly, knowledgeable, and reliable staff who treated her like family. Her room was always immaculate and she was able to get the care and attention she needed. Dementia is an extremely difficult and debilitating condition but Granite Mesa's well-trained professionals gave me and my family peace of mind.

GM took wonderful care of my mother. She lived to be 92, and I don't think she would have made it that long if it wasn't for them and their awesome staff.

I have been to lots of facilities over the years, visiting friends and loved ones. I'm not joking when I tell you that Granite Mesa Health Center is the best! I live here and from my perspective, I know that these folks care about us, they smile, they laugh, and they are real! When I came here, I wanted to be here because I had no help at home, and so it was like a breath of fresh air to come here and have people willing to help me. Our rooms are comfortable and there are staff members who will help you get dressed, help you get into bed and out and do whatever you need to feel at home. Our food here is good and you have a menu of what you can choose from. There are activities that you can take part in, or you or you can choose to watch television, read a book, visit with other residents. The staff can help you find activities to do if you need them to. I've found it so good just to talk to others here... You cheer them up, then there will be a day someone will cheer you up when you need it. Please come and visit and you will find folks ready to help you feel right at home.

I had my uncle at Granite Mesa. Every time we visited him he was always happy and love the food. He loved breakfast the most. He got to choose from a select menu. He's room was always clean. Sometimes we had to visit outside. He loved going outside in the mornings. The only complain the had was that they didn't have sodas. He loved the CNA and the nurse on the hall. He is a very social person. He loved playing bingo. He really enjoyed therapy . They are very friendly. He felt therapy helped him go back home sooner. When he was discharged home he was so happy but at the same time sad because he had made some friends.

I didn’t want to go to Granite Mesa. I had been sick in the hospital and was too weak to go home. My daughter had to go out of town and couldn’t be there for my transfer. They sent their van driver to pick me up. He was very professional. I felt safe with the way he helped me. When I arrived at Granite Mesa I was taken to a semi private room. The rooms were pretty basic but they were clean. There was an electric bed and small TV on each side of the room. There was another resident in the room. Two nurses came in the room and started the paperwork. There were a lot of questions and they checked my vital signs and looked at my skin. I’m a night owl but my roommate was not. I was afraid I was bothering her so I told the nurses and they moved me to a different room. The next day I started therapy. All the therapists were very good. I was short of breath and scared they would get impatient but they did not. They helped me with ideas on how to do things easier. To my surprise the food was pretty good. Except the coffee it was very weak. The portions were too large for me. Every staff member was very nice. From the nurse aides, nurses, therapists and so on everyone was very helpful. I didn’t really participate in the activities but the activity director came by and invited me to several. They had Bingo, crafts, jewelry making and live music. I could hear the music when I was walking in the hallway. Overall I was was happy with my care.

My wife went to Granite Mesa from the hospital.She was weak after being hospitalized with pneumonia. On her first full day a CNA left her standing alone, her knee collapsed and she fell to the floor. Several days later it happened again. This time her leg was caught between the wall and a toilet. This time that same knee was badly hurt. They never offered xrays. The day after that the office manager came into her room when I was there. She had an "incident report" with her and expected my wife or I to sign the bottom of the page without letting either of us read it. We both refused. She called Medicare to say my wife, as a patient, was uncooperative and got an order for her to be discharged. Now the lawsuit begins.

This facility is very short staffed. Management especially DON is very rude and stays locked up in her office all day. Upper management does not listen to concerns of family/staff members. is it impossible for residents get baths or the right care they need because so understaffed. Poor communication between staff. Staff requirements are to start getting them up at four o clock in the morning regardless of residents choice. Does not deserve any stars

My mother is in Granite Mesa. Everybody is so nice and so helpful because my mother is very uncooperative. I have nothing but good things to say about the wing she is on. They have been so caring and so concerned. They work with her to try and get her to eat. They are marvelous. I am sure that represents the entire facility. They have church on Sunday morning. They have a reading room, and it has puzzles and things in there.

My husband had a very good physical therapist that works with him all the time. The nurses were very nice, and Legend Healthcare Granite Mesa was a clean and very peaceful place. It’s close to the creek, easy parking, and very friendly staff. He wasn’t particularly fond of the food.

The people at Granite Mesa were much needier and much less free to do things. They had activities two or three times a day that maybe 20% of the people can attend, and the rest get individual care and attention in their rooms. It’s a nursing facility, so they had a bed, two chairs, television set, and electronics they need for life support systems. It’s clean. My brother likes the food five out of seven days a week, but they can serve better food. The staff is amazing in how they work with them physically to make them stronger. It's fantastic.

I have a family member that is having issues with billing. We've been to the billing dept multiple times, where the woman was just rude to us.. I've not received a bill in months. I've given this lady two checks, and received a call saying one check is missing. This lady always blames others for what is obviously her personal errors, She seems to always be on her cell phone, totally unorganized when we have been in her office but seems to have plenty of time to play funnies with the other ladies in the office. We have left multiple messages with no response. I have also discussed with upper Management and all I got was more excuses. If this is the way she handles our account, I can only imagine what others are subjected to.

This place is awful-does not deserve a star! Staff is rude if even around. Patients sitting alone. Office manager is really rude. Would not recommend this facility to anyone!!!!!!!!!!!!!

Do note bring your loved one to this place. Short staff, over worked to the max. No one in charge or take to take responsibility for the poor care your family member is getting. Buzzers going off all up the hall, our nurse takes five smoke breaks before lunch. Her boss locked in is his office all day. No one to be found while my mom sits in her fecies for two hours. I do not recommend this place.

Wow. This facility is wonderful. They have caring and compassionate staff. My mother passed away and staff came in to say goodbye and were so caring. You hear horror stories about nursing homes but this was not at all the case at Granite Mesa! From the line staff to the administration, they were all kind. You can always find fault with people facilities but I was extremely impressed. The rehab staff was also excellent. I would not hesitate to recommend this facility.

My mom was at this facility. They were very good to her, and I have respect for how much they truly cared about her as a person. It is clean, no nursing home smell. From the custodians to the nurses, everyone showed genuine caring for mom. Things could be improved. That's probably true of any nursing home you walk into. It seems like the new company might want to think of ways to reward the staff, encourage them, and let them know when they have done something right. I heard too many comments about how short staffed they were, and many people working double shifts and overtime in order to make sure someone was there to work. Then I saw no appreciation to those same workers for going the extra mile. Seems like the management is a bit out of touch with the real stuff going on with their employees. It pays to visit this facility. It is always busy, and the residents are happy and attend the activities. Of course, there are issues with response times to call lights. You have to understand that you are not always first in line, and the nurses may be busy with other patients when you call. The food appears to be improving, and that is important!

This place is very clean. I like this place because it is close to my church. I have lots of friend who come and get me for lots of functions. Most of us get along with each other. This place has improved through out the years and still improving. I've been here since July 16, 2009 as a resident.

I do have to say that this is one of the nicest, caring nursing homes I've been. These people do care about they're residents. Their therapy team is the greatest. They do an awesome job. This place is always clean. The people who don't like this place is because they probably don't like any place at all.

I'm a visitor and I love this nursing home because the first thing you notice as you walk in, there is no "nursing home smell" and it's clean. They always keep the resident entertained doing activities. And they're rehab team is awesome. I meet with some of the therapist and they know what they're doing. There people really care about their patients.

This is the worse place that I have ever had contact with. My mother was there for one month, she fell while she was there, the nurse was giving her the wrong insulin, no one ever knew what the other one was doing. Her leg got severely infected and they did NOTHING, did not even have her on antibiotic. We removed her from there and now they are not releasing her meds to us. Director of Nurses walked out, so for some time there was not even a director of nurses there, we were told today that there is not an administrator. Now Brenda, that answers the phone is telling us that she does not know the number for Corp office. They were suppose to be doing physical therapy and never did, and whey they did, it may have been for 5 min. I witnessed numerous times as I was there visiting my mother, several patients needing assistance and not one nurse or aide within sight. It is HORRIBLE. IT SHOULD NOT EVEN BE RATED, IT IS THAT BAD

We didn't like Granite Mesa much. The people are not as compassionate, especially, the administration people. They're running it like a business. However, the staff is fairly good. It's not a very big facility. The rooms are semi-private rooms, but because of my husband's condition, he has his own room.
