I would give it five stars, but it is an old building. If they can make the 2nd and 3rd floors look like the specialty hospital on the 5th floor, this would be 5-star all the way. With that being said, the staff does a miraculous job keeping things clean and tidy. The administrative, nursing, and therapy staffs are second-to-none. My mother has been there three weeks and, sadly, will have to be there for much longer. She could not be in better hands. This is a very stressful time for me and my family, and the folks at Horizon have made it much less painful. I must point out a few outstanding individuals who have helped us with the transition. Favi and Sarah, two nurses responsible for the bulk of my mother's care, are professional and approachable. I always get quick responses to my concerns and questions. They are the best! Grace, the social services director, has been more helpful and informative than I could ever ask. In all, everyone at Horizon is friendly, helpful, and patient care oriented. I am very pleased to consider it my mother's temporary home.

Let me start by saying that most of the staff that care for the patients are kind and responsive. My dad was on the fourth floor and he was very fond of the staff he came in contact with. I did not have the same experience. On of the aids on the floor spent all of he time at the nurses station texting. If you dared interrupt her, she scowled and reluctantly got someone else to help you. There was never a team meeting advising us of his condition , prognosis, progress, nothing. At the end of his stay, I received a call late morning telling me my dad would be discharged the next day. No prior notice, no time line, just come and gmet him he HAS to be out by tomorrow! I found a place for him after a chaotic day of searching and visits with the help of a friend in state social services, as I did not take the social workers recommendation for placement. I did not want dad in another substandard place. I received a call from a very smug SW asking if I wanted to order a walker, wheelchair or any other equipment for my dad. Unbelievable. In addition, he was scheduled for transfer at 10:00, because he HAD to be out of there ASAP. They finally put him in a van at 3:00 pm and tha,t because I called to remind them. The horror story ends with dad being dropped off with no medical records, no instruction for the new wound on his foot, no information on doctor appointments, no paperwork. Beware! Like I said, the actual hands on staff did a good job, the receptionist was good, but the administrative staff is unprofessional and incompetent. Go elsewhere if you have a choice.

MY FATHER HAS BEEN HERE AT THIS CENTER FOR OVER A YEAR 3FLOOR.HE HAS NEVER HAD TO BE SENT TO AHOSPITAL ,LIKE THE ONE HE WAS IN LAST TIME, HERE AT HORIZON THE STAFF IS VERY CARING AND TREAT PATIENTS LIKE FAMILY VERY LOVING,I HAVE ARRIVE THERE WITHOUT NOTICE AND HAVE FOUND MY DAD VERY CLEAN, I CAN CALL AND THEY WILL GIVE ME INFORM ME ON MY DAD OR WILL LOOK FOR HIM SO I CAN TALK TO HIM.VERY HAPPY WITH EVERYONE , THANK U 3FLOOR AND EVERYONE THERE. JUST NEED MORE PEOPLE TO VISIT PLS, CARING DAUGHTHER MY DAD IS MY EYES.:)
