My best friend went to this facility for rehab after breaking her back in a fall. THIS PLACE POSITIVELY BROKE HER SPIRIT. Were this a children's care facility, the state would shut it down. Apparently, it's acceptable to leave the elderly or disabled sitting in soiled diapers all day, to feed them foods to which they are allergic, and to blow-off their needs or complaints as the imaginings of senility. •By the second week, she gave-up on ASKING for a fresh diaper and resolved herself to the fact that they would get to her when they got to her. (Best I have been able to tell, she gets a fresh diaper about once per eight hour shift.) •Despite her food allergies - including eggs and milk - being CLEARLY marked on the "ticket" that comes with her meals, she continues to get eggs for breakfast and milk with meals. •When she actually made it to physical therapy, she was pleased with the therapist, but there were days no one ever managed to get her to PT. •After speaking to her on the phone one night, I knew something was wrong with her. When I called the desk and told them she didn't sound right, the nurse told me she was just confused, like she always is. I replied that she is, in fact, NOT always confused and that she either had a fever or her blood sugar was low. I was ignored and she was chastised for calling me! At some point the next day, the staff figured-out that she had an infection (and a fever). •When it came to getting her discharged (to go somewhere else), the facility physician didn't want to let her leave, then failed to conduct the required TB test in time for her to leave as scheduled. (Her doctor ordered the test several days before she was supposed to move.) She had to spend two extra days in hell waiting for the test results. She was so disheartened that I feared the extra two days might actually do her in.

My grandmother was recently sent here for rehab. And what an absolutely horrible experience she had. I am appalled by the “care” they give to patients there and I say the term care lightly. My grandmother text me multiple times that she called for help to get cleaned up and no one came for hours or would go in and turn the light off and not do anything to help her. So many times she would text me and I would go up there and get her cleaned up and just about the time I was done here comes the CNA. The excuses I was given are that they are short staffed. Only to find the nurse at the end of the hall watching tv and not helping what so ever. This is completely unacceptable and I fear for the people that don’t have family that can come and help them. The DON at the facility was requested to come change my grandmothers PICC line dressing after it was found with a folded up piece of gauze over the top of the dressing that was no longer intact. Off the jump he broke his sterile field when his gloves broke. Do you think he went and got new gloves? Nope. Continued on changing the dressing. I spoke with the DON about the events of the prior night and was told that the door to the end of the hallway would be closed. Next day…..same exact thing. I had my grandmother moved to an inpatient rehab that day after being told by Taylor the social worker that “it could take several days” she was kindly informed that it would be happening today. And it did. This facility is an absolute disgrace and in my professional opinion needs a very thorough visit from the state. I’ve been a nurse for 18 years and have never seen the amount of neglect of patients than I saw at this facility. I wouldn’t send my dog here. This facility seems to be extremely poorly run and needs to be swept from administration down.

I'm going to start with if you have a loved one that was recommended to Green oaks nursing home and rehab don't place them there ,and if you have a loved one there get them out ASAP!!! I'm going to start with my story now I have a loved one that was recommended to Green oaks nursing home and rehab someone was with them during visiting hours and started noticing that they do not go by protocol they don't change their gloves after picking up trash cans and going back to our loved one when the IV is done they leave it hooked up to them when we call them out many times to unhook the IV because it can cause sepsis to a loved one, no phones in the rooms if your loved ones my loved one had his cell phone for a few days before they did a covid-19 lockdown when placed there he was in good condition try to call him for days and could not get a hold of him about 4 days after lockdown someone did get a hold of our loved one and was told get me the hell out of here I'm not good I'm bad 911 was called to remove our loved one from Green oaks nursing home and rehab and taking back to the hospital where he was really bad shape I could go on more but if what I've said about Green oaks nursing home and rehab does not stop you and you'll see for yourself.Please don't place your loved ones here the staff at Green oaks nursing and rehab come across as they are only there for a paycheck not cuz they love their jobs!!

My mom has been here for almost a week and we’re about to get her out ASAP. We never planned for her to be there long term, maybe a month, as we are moving out of state. We’ve been caring for my mom since December 2018 complications following a lung transplant. Over last couple of years she has spent 14 consecutive months in hospitals, we cared for her at home for almost a year and decided to put her in a facility because she needed so much and we were playing every role (nurse, tech, pt, ot, speech, house keeping). Our goal was for her to improve a little so home care would be possible. Unfortunately, that has not been the case. She declined a lot in the first facility so we just moved her to this one. She got there on Wednesday (should’ve had dialysis Thursday) and we had the Plan of care meeting (via the phone) on Friday. We made sure to go over everything she had going on and asked if they had dialysis setup. At first they had no idea. Which was concerning bc the meeting was about her so you should know. But they checked and said they had her for T/Th/Sat. The next day (Saturday) my brother goes to the dialysis center to lay eyes on her (we don’t get to see her often bc of covid restrictions) and she never showed. Called them and the nurse’s excuse was she only works weekends. Total bs. I don’t care if you work once a week. On that one day you should get caught up and see what’s going on. When she looked they had my mom scheduled for dialysis M/W/F and T/Th/Sat but she didn’t catch that or ask for clarification. They couldn’t get her to dialysis that day but said they would on Tuesday. We decided to call the dialysis center to see if she was there on Thursday and she wasn’t. So she has now missed 2 treatments. WTF?! We told them to send her to the hospital. Trying to call them is a gamble. Today I spent almost 30 minutes on hold before hanging up and my brother has experienced the same. During the plan of care meeting they seemed to be in a rush and not really trying to understand our concerns. We understand that it’s hit or miss with any skilled nursing facility and then COVID has made their work more complicated but ppl still need to do their job I don’t recommend sending your loved one here

I visited this facility

The people were nice at Senior Care of Green Oaks. However, there was only one nurse to each pod and there was no aide, which is why people were sitting in their own urine. They were rolling around in wheelchairs with pee dripping down out of their seats. When you walk in the place there's no way to mask the smell of urine. It was so strong. I did a quick tour and then walked out. There was no way I would put my mother in there. They didn't have enough staff members to care for their patients. The only staff I dealt with was their marketing person. She was nice, but they're understaffed and the place was very dirty.

The staff here is totally professional, competent and attentive. I appreciate everything they provide for my parents on a daily basis. They respond in a timely way and they are sincere about helping my mom and dad. I am extremely impressed and grateful for the physical therapy they provide. An extra bonus is the food is delicious!

Our basic problem with Senior Care of Green Oaks was the response from the staff. My wife can't use the toilet, so she needed diaper change, and there wasn't enough staff to be responsive to her needs. The shared room she had was adequate. She had physical therapy. She had her meals in bed all the time, and the food wasn't bad. They had some good religious services, movies, and Bingo.

Where shall I begin...after my dad got checked in. They put him in a cheap low flat unadjustable bed with one thin sheet to cover him and he is not medicaid! It took several days for the caregivers to learn of his physical condition, meds, in ability to walk and incontinent situation. He was treat rudely by one of the caregivers who was also hateful to us (the family). I was kind and tried to reason with her that my dad has neuropathy, but she wouldn't listen. I was appalled! One day we found two large laundry bins hidden in my dad's and roommate's bathroom parked with stinky laundry. I was shocked! The dietician prepared spinach and the caregiver delivered it to my dad when he's not supposed to eat vitamin K because he's on coumadin. One of the rehab therapists left my dad sitting on the edge of the bed without assisting him into bed. The wheel chair they gave him was filthy. The caregivers were tossing the call button off the bed down onto the floor until we complained about it. And upon several occasions we have found my dad laying in his urine and feces. I don't recommend this place. I gave this place a 2 star because after we complained several times, they are starting to get their act together and take better care of my dad. Oh yeh, not only do the caregivers hide stinky laundry bins in the resident's bathroom, the whole crew will often hide out for long periods of time in one of the rooms and no one will attend the calls at the nurses station.

Green Oaks is good. The rooms are small, and the food is average. The residents and the staff are fine. Overall, it's above average. We haven't really experienced much because my uncle is bed-bound and limited to the room, but having been in several nursing homes, we see it's clean and maintained. There are three CNAs per hall. It's a good, clean place, and the people and the staff are friendly, so that's good.

I visited this facility

Green Oaks was wonderful, and I was very impressed. Everyone was wonderful, welcoming, and doing their job. It was like walking into a hotel. It was very clean, there was no odor, and there were many patients. They keep them very active. I went into the cafeteria, and the food looked wonderful. It was the kind of care I want for my father.

This place needs to be SHUT DOWN! This facility is TERRIBLE.. If you or your love one needs to be in a rehab center DO NOT go here. THE night nurses & aids don't know their jobs, during the day it's ok, but from 2::00pm when they have a shift change of staff thats when this place is CRAP! The food service is HORRIBLE also, meals are always COLD, Coffee that taste like they get it out of Lake Arlington, they provide a daily menu to choose your meals but they always get it wrong, they don't knock on the door before they enter your room they just barge in and turn on the light when your'e sleeping, laughing & talking loud, most don't speak good english hard to understand, don't follow Drs. orders of how many & when to give you your prescription medications, night staff is also not friendly at all they are RUDE PEOPLE, and boy are they DUMB & STUPID!!!

Do not put your loved ones in this facility. The employees are unfriendly and have to be prodded to do their jobs. If you don't have someone there with the patient they are left sitting around alone or in the dining room. The food, if that's what you want to call it looks disgusting. I found myself feeling very sorry for the other patients I was around. There are a couple of decent nurses and physical therapists but for the MOST part seemingly very uncaring, unfriendly and unwilling to go the extra mile to be kind and helpful. A smile goes a long way... if you are unhappy in your job of caring for the ill, get out of this line of work.

I am a friend or relative of resident

During my parents stay at this facility, we went through 7 administrators and more nurses and aides then I can count. Out of 70 nursing homes in the Tarrant county area, this facility ranked #70 with a score of 19 with DADS. This should be a clue as to how bad the care is. All the glitters is not gold. If you really want to know about the facility, then talk to the residents and family members you see there. I would never recommend taking your loved one here.

I would like to start on a positive note. My grandmother was placed in this facility almost a year ago due to problems with memory. She was not able to take care of her self and would often forget to take her medication, which only made her situation worse. I am thankful that I feel like she is finally getting the constant care that she needs, that my family was unable to provide due to work. That being said, we have had several issues with this facility. If it were not for at least one member of our family being up there every night, I'm not sure that she would be getting the care she deserves. They have attempted to put her on sleeping medication numerous times, though we have signed forms stating that she is NOT to be put on them. They only worsen her condition. My grandmother routinely needs to have fluid removed from her abdominal cavity due to congestive heart failure. They failed to notice after one such procedure that she was bleeding though her clothing. It was not until a member of my family pointed it out to a member of the staff that they sent her to the emergency room. That should not need to be brought to their attention. It is their job to take care of the people at this facility. The reason most people are in a facility such as this is because they need special care that can not be provided for them otherwise. It is sad that someone who is not educated in the field recognizes when someone needs to be sent to a hospital or not. It really makes me feel for the people that are in the facility who do not have someone to be an advocate for them. Who knows if they are being cared for properly.
